    Sciatic nerve flossing is introduced and an at home exercise to help with sciatic nerve adhesions is taught. Dr. Pedersen teaches how to check your shoes for wear patterns that can help you identify imbalances in your feet and arches. Foot issues can many times cause sciatic nerve pain and is not usually looked into by doctors and patients.
    Leg length inequality and the difference between anatomical leg length difference and physiological leg length difference is discussed. Then common solutions to anatomical leg length issues are mentioned.

      I totally agree with you. Been to 5 doctors so far who failed to diagnose me, had to fly out to state to get an accurate diagnosis with a doctor who also did ultrasound to diagnose me. Too many dumb doctors in the USA, do your research

      I would not feel so all alone, most ppl I know attribute life changing debilitation to decisions made by their doctors when they still trusted them. I’m one. If you must have a medical doctor, I say find one who is older. The young corporate drones live by the hypocritical oath: “the patient!? Fuck the patient. This is about my career!”

    Was a week into the most debilitating sciatic pain I’ve experienced and my physiotherapist wasn’t sure what the cause was and suspected a disc issue. Found this video last night which led to a few more other videos describing how to release the piriformis. Tried it this morning and my sciatica has pretty much disappeared in the course of a day! The feeling in my left foot has even started to come back. I know the comments section on videos like this are always people saying how much it helped them, so I’m SO thankful that I get to add a comment like that!! Thank you so much!!

    Sir. You are saving my life. 20 yrs, a specialist, X-rays, MRI, physical therapy, 5 chiropractors, orthotics, EVERYTHING and not once did anyone mention a trocanteric belt. In all my research somehow never came across it!
    I am angry and sad, but mostly THANKFUL to have finally found YOU. Hearing you say “don’t give up” brings me to tears.
    So many years, so much pain, not being able to trust my own body to do simple things or enjoy activities without fear of the pain to follow…

    There is also a connection from sitting for long hours and hamstring muscles getting shortened , pulls on quadradus lumborum which pull on the c4 backbone that pinched your sciatic nerve or the other nerve that runs through your secrael foramen , both causing similar pain to siatica

    My GP back in my home state diagnosed me with Sciatica many years ago, an old dancing injury plays into this a lot amongst other underlying issues.
    Today has been exceptionally painful, and am currently laying on my bed with my legs propped up against the wall.
    I just checked the soles of my shoes, and have noticed the left foot has worn through faster than the right.

    You have so much sensible advice, and you really seem to care about people, which is quite rare nowadays. I have been struggling with si joint for years, but a belt always help, now i have bad balance problems which is getting better with walking a lot. I have flat feet and walk peculiar, i will definately see a podriast to get specially made inersoles. Thank you once again, Diane

    Thank you so much. You would be a great elementary teacher, as you explain things so well!
    I’m 65 and have had knee pain, knee surgery, PT for severe hamstring pain, SI Joint Dysfunction., IT Band Syndrome, Bursitis, etc, etc. In 2 days I go for another injection in my hip. The orthopedic surgeon suggested Pain Mgt Doc. Aleve and Tylenol don’t help. I won’t give up! Thanks for your great video. I’m going to watch some more!

    Thank you doc.. dont give up is what inspired me.If I have to rely on my doctor I wouldnt be here watching this video.I have worked out what exercises are good for me.Understanding the SI joint has helped me greatly and using pelvic block idea was an easy fix and so after watching your video I used a pillow and went to sleep..a good deep sleep after a long while.
    Wake up and the pain has gone down considerably and burning sensations is fading. I will look into this seriously and the collapsing arche in my feet.Thank you so much doc.

    Thank you . I learned so much and have realized both shoes wear on the inside . One side more . My hips seize up and tender points on the inside of my knees . Plus a fall on my butt on a bus 25 years ago and injured my coxic apparently . Lived in great pain since . I also feel the tight facia in my glutes and hips . I’m going to show my chiropractor your video 🙏🏼

    I just stumbled upon your video and I think it’s the linchpin! I have been diagnosed with SI joint instability but have never been able to fix it.
    I’m going to Europe in 2 weeks: I’ll diligently do your recommended exercises and wear the belt. I’ll report back in 2 weeks, but I have hope for the first time in 4 months!

    Excellent coverage of these issues. Do you have a video with considerations regarding those with osteoporosis/degenerative disc disease? I have had 2 spinal fusions (w/plates) and still have bad pain in lumbar spine and legs. Thank you for such comprehensive info.

    I’ve been dealing with intense sciatic pain for close to 10 years now (carrying three babies & a couple of falls have led to instability in muscles and ligaments).
    My doctor was testing for a SI dysfunction but it’s never been fully determined.
    I have a pelvic MRI coming up. Which I’ve never had.
    I know i have a posterior pelvis tilt with a hip hike on the left which seems make my left leg “shorter” (all pain is on the left side).
    Along with the belt, ice, heat and rest I’ve been doing my own trigger point release on the piriformis muscle which has helped greatly. Most inflammation meds don’t touch the pain.
    I will try the flossing & avoid shearing.

    I found your video very helpful and an excercise to help my siatic nerve pain. I too have dropped arches and wear orthotics. However during summer a full shoe is far too hot to wear. Hence my back pain increase.

    Wonderful presentation! I appreciate your soothing tones and the simplicity of your teaching method; the neutral background and simple physical models made it so easy to understand. You with a skeleton and pointer. For me, the blue muscle with the pointer demonstrating the piriformis running underneath was better than any video. It is brilliant that the viewer can skip past chapters they don't need, although I am glad I didn't skip anything. I'll bet people leave your videos on just for the calming effect.     This was perfect, doctor Pedersen. Thank you, so much for this thorough presentation about this matter of sciatica and SI joint dysfunction. You presented everything so well, with pictures, demonstrations, and clear and concise descriptions that I followed you along totally engaged 100%! Everything you described is what I have been living with for many years, especially these past two years. I have returned over 100 pair of shoes in two years, trying to get the right supportive fit for my dysfunctional right hip, low back and right ankle pain with flat feet. I have watched this video two times in a row, and plan on returning to it again and again taking in everything and hoping to get help with this. Where in Northern California is your practice? I want to say that you did an excellent job all the way around, and haven't met any other person or video that explained these things as you did, and I thank you! It makes sense, and takes away a lot of anxiety and fear about not knowing what is constantly going on, especially when the surgeons say that I am not surgical.

    Your information is amazing I have been suffering with SI joint pain from a work accident in the year 2000 my left leg is now much bigger than my right and my left hip just aches with pain that radiates down my leg. I have a Harrington rod in my back for the scoliosis operation in the 88 and I have had other surgeries on my back and still have another in the next 90 days. I have suffered in pain from a young age and now I am 50 the pain has taken my life away but anyway there is plenty of people worse off than me thanks for the video mate. 🇦🇺🙏

    This is what happened to me. I was ultra fit throughout my twenties and got piriformis syndrome at 29/40 at the same time I got hallux rigidus/osteoarthritis in my foot. The constant pain from both pushed me over the edge. I ended up lying in bed with depression and went from about 14 stone of pure muscle to now being 25 stone morbidly obese. Not good

    thank you for this video found what is irritating my sciatica nerve. Observe my my flaps the other day are wear down the back watching my walking So happy I already have orpedic shoes my calfs and inner knee on the sides hurt a lot and sometimes sensitive to the touch. And from the ankle to the top of my foot feel a burning and irritation

    Thank you very much for this. I had watched numerous videos to alleviate my pain, but I found valuable information in this one. My problem is in my feet caving. I will start being conscious about that and see how it goes.

      I totally agree!! I have watched so many videos on sciatica that my brain nerves are numb but this video is by far the best. I have two left toes numb and pain in what seems to be the priformus muscle. Can’t sleep at night because no position is comfortable. Seeing a Dr and right now waiting for physical therapy next week. Hoping PT can fix it but so much info in this video and greatly appreciate this video and might figure out the cause.

    Thank you for such an informative video. Just one minor observation, at 5:34 you describe the sciatic nerve running through under the piriformis muscle, but also the pen should have been inserted inside not outside the iliac bone, since anatomy diagrams show it that way. Blessings!

    So much great info in this video. I’ve suffered with chronic sacroiliac pain for 25 years and despite having every treatment known to mankind and spending a small fortune in the process I’ve never got to the bottom of what’s causing my pain issues. It’s really quite depressing but I’m still looking for answers 😅

    I'm so happy to have come to your video. I wish you were in my area. My chiropractor manually adjusts me in the wrong places. I getna tiny bit of relief, but I'm feeling my pain shortly after. I do yoga every day and just learned my favorite position is causing it to get worse. I just got a mri and have been waiting to see a neurologist in October. 2017 to 2023 of struggle.

    I've had si instability but also fixated. When fixated must have chiro. Very happy to be learning from these vids. Will be looking at glute med exercises and others.

    I just tried the movement to break up adhesions in the sciatic nerve and I can now feel less pain and referral down into my calf and foot. I am hopeful. Thank you for sharing this information. I had trauma to my hip and sciatica N this last March. It's been interesting and frustrating.

    I just found this channel yesterday and thank you so much! I apologize in advance for this long comment. I have adhd as well so I'm giving the background as well as asking a question (or two).
    I you do another video: I have had lower back pain since the last couple months I was pregnant 6 1/2 years ago so it's been going on maybe 6 3/4 years now. I was managing it alone for a long time. I went to my primary care who sent me to a pain clinic since I've also had 2 knee surgeries and have osteoarthritis in my right knee. I went to a chiropractor a couple years ago and he told me I had sciatica and my back was out of alignment. We just moved and it took a few months to finish the moving process, but the pain got MUCH worse during/after that. It's been massively hurting ever since. It took a fight to get accepted, but I finally found a different orthopedic doctor who would see me after my primary care had an xray done. The orthopedist did a few tests, including the finger point test, and he immediately hit the nail on the head. I thought the pain was mostly near the spine but as soon as he put his finger directly on the area, it registered with my brain (like my brain was confused lol) and I can clearly feel it's my hip now. I invested in a belt from Amazon (before seeing this channel) since we were going out of town and the difference it immediately made was incredible! My husband also had a Purple gel seat that he gave to me because i have massive pain while sitting. The orthpedist also gave me an injection which made my hip sore for 3 days and felt like it sometimes helped but sometimes I would still feel the pain. He now wants an MRI and fighting with insurance to get it approved so we can do that. Then 2 nights ago, I stupidly took off the belt thinking I would be fine just simply feeding the cats since I was only mildly uncomfortable. When I was done, I was walking up 6 stairs to get in the back door of the house and when I got up those stairs, I sat down for just a minute (on after thought I realized my legs were higher up than normal when I was sitting and I was also slightly leaning over as well) to pet one of the cats we have, and when I went to stand up, I felt the pain worse than usual about half way up, and when I went to walk in the door I realized my left hip, which I don't typically have noticeable issues with, became extremely painful to the point I couldn't move my left leg due to the pain for a minute. Once I gave myself a minute, I was able to take 5 steps to get to the bedroom where my husband was able to help me from there. I'm still massively hurting and was basically bedridden yesterday. I figured maybe if I gave it a little bit to heal from whatever I did, and didn't use the joints unless I needed to, maybe the pain would ease a little. I'm under a contract with my primary care for tramadol and a muscle relaxer so I felt going to the ER was going to be pointless as I wasn't going to be able to accept any pain medication they tried to prescribe me.
    My question is this now that Ive given the long backstory 😂. Since this issue has been going on so long,, is this something I'm going to be stuck with? I've been using the belt for over a month now and it helps while I have it on, but when I take it off, it typically comes back.

    Question: I have a problem with my Piriformis. No pain, just an uncomfortable annoying feeling in my right cheek down my leg, into my foot. I've never had shooting, sharp pains. Just a tickly feeling, makes me anxious at times. I'm seeing a chiropractor, next will be my 4th session. Using Pulsar, plus, manual Manipulation on the hips, breakaway table, and then an automatic roller that rolls from the sacral area to the shoulder neck area.
    The first session was remarkable, Second not so much, my tingling increased. Will it take awhile to get this all in order?
    You should be an instructor if not already.....Thanks for all the info....

    can confirm - spent years being mis-/cross-diagnosed for SI issues, piriformis, sciatica, until one day I went to get an MRI and it was a herniated disc at the root all along. All the other youtube videos tell you to clearly identify which issue you have, implying that being related issues is an impossibility.

      How are you going about healing it? I’m suffering from a herniated disc too and the road to recovery doesn’t seem simple. Sciatica seems to start somewhere around the SI joint area.

    Great teaching video Dr. Pedersen! I’d like to see one on lower body strength training that is less risky than holding heavy free weights, doing deadlifts, doing lunges, etc. For example, how risky is a heavy leg press with a machine?
    I am scrawny, naturally weak (life-long, not just because I’m old now), and finally found magic in being able to do daily activities as well as normal people do once I started hitting lower body hard in the gym about six years ago. That is now gone for me per my medical professionals, but they didn’t give me anything new to add other than Pilates.

      Resistance bands! Great for low impact legs and glutes. Ive been dealing with crippling sciatica off and on for about a decade. Recently discovered resistance band exercises for lower body, just gotta keep up with it because if your muscles cant hold up your body, leads to instability unevenness which can lead to pain.

    Thankyou …Dr Brant. I have been suffering with L4 L5 disc bulge followed by SI joint dysfunction and tail bone pain. The moment you said “Don’t give up” really brought tears in my eyes. The pain I’ve been thru both physically mentally and psychologically is just so miserable. It took me more than 6 months to heal and go thru all the pain. And finally understand my situation because not all doctors are so knowledgeable and not everyone gives the right advice. I one could heal by rest and physiotheraphy. Your video is one of the best videos I found on internet and so well explained. Other doctors should learn from you. Really appreciate. Thank you doc. ❤

      Im only 23 and I suffer from the same thing!! Doctors only want to give medication and just hope that i can heal myself because im young but it takes a toll on my mental more than my physical.

    Superb video!!! I had a herniated disc (with the sciatica as a bonus) about 3 years ago, completely got rid of the issue in about 2.5 months thanks to the McKenzie technique I discovered.
    with that I had some numbness on both legs, the right leg was worse than the left, the numbness did become a little better but never completely left,
    Will it ever leave me?

    I'm so grateful you're doing a video about combining the SI joint dysfunction and the sciatica. My SI joint was completely destroyed and if I were to use a pair of crutches I was paralyzed 😅myself and I'm also grateful

      Before few years ago I can’t control my leg 🦵, off course I’m very tired by my upper & low back pain!
      Most of the time my legs got numbness & shaking also !
      Few doctors told me I should make surgical operation , but I don’t believe it !
      I’m going back home by other person help & I rest few months with out stress
      Then I tried waking & make do something in my room , after few weeks that bad paralyzed spear-it is gone 🤷🏻‍♂️
      I’m looking my legs need to be walk again, with out any therapy or other waste of money services
      Currently I am running & walking , but I have little pain in my right side butt, I think this video help me very well!
      Did u see him 👉 Dr Said “ don’t gave up “ 💪
      So Mr or Miss , through ur crunch and try something don’t gave up !

    I ve been going through this for 18 months it’s tingling into the feet, pain in my outer hip and buttocks and into groin. The central soreness comes from the SI joint region on both sides. If I place my hands on my hips and my thumbs over the SI area on both sides I get soreness, tingling in my feet and a sharp sciatic pain into my buttock. It goes away as soon as I stop pressing on that area. I struggle laying on my back even with pillows under my knees and sitting for a long time or on a hard chair all bring on discomfort and start the tingling into my foot. It’s worse by far on the right side. I have also had tingling into the pelvic region. I find stretching my legs straight and pointing my toes out whilst laying on my side make me feel better. I usually now sleep only on my left side. I tried the SI belt and as soon as I wore it it made the tingling worse. I press my back and my foot tingles. I ve had 2 MRIs and seen 3 physios. I had a nerve conduction test that came back negative for the cause direct from my spine. This all started when I arched back a few times over a few days. I did this very quickly and then the tingling gradually started. I m thinking I injured my SI joints or something. My constant refused to acknowledge the possibility of SI joint problems. I ve now been referred to a neurologist as my orthopaedic specialist can’t understand the cause. I feel like one side of my back where the SI joint area is sticks out more. I run my hand where I feel most pain on the right and there’s a hard raised area that sticks out compared to the left side. That’s where the pain comes and goes. I m wondering why the MRI would not show that. Perhaps my orthopaedic consultant didn’t look at that area. He was certain it was my discs or facet joints but there was no sign of neural compression from the lumber spine. I have not seen a chiropractor.

    Went to physical therapy and did all these exercises, it helped alot. I started water / arthritis classes in a warm water pool 2x a week and sitting in a hot tub.that worked wonders. Helps your tight muscles.

      All of those actions that you noted are fantastic. I really believe that water aerobics and fitness classes are such an amazing option when available for rehab of arthritic joints. The hot tub sounds nice also. Thanks for your comment.

    I fell 8 feet onto hip and that started 10 years of si pain. I had a chiro guy realign it once by literally pulling my leg. My sciatic pain is outside calf and ankle now. A burning string in the leg can get to intense pain but knee to chest helps gap open the si.

    Insightful video. I'm a 19 year old guy with no family history of scoliosis and I can't even sit properly anymore because my lower back/lumbar feels bent and very tight on the left side, extremely uncomfortable to sit without leaning to one side. No injuries or trauma to the back. I believe it started from slouching/leaning to one side while sitting for prolonged periods. Any idea what it may be? And how can I address this with exercises? I experience intense lower back pain when lying down on my back but no leg pain

    Thanks so much Dr Pedersen. Your video helps me understand more about my problems. I am a long term patient suffering from different spinal problems. 2years ago even got worse and I had to quit my job. I did want to commit suicide and your line "Never Give Up" make me burst into tears. I have seen over 30 doctors and therapists these two years but no improvement. One of my problems is sacrum misalignment which is always swollen on what side. May I know if there is any way to help ? Thanks so much in advance.

    Thank you so much for this information! I am struggling with correct diagnosis aside from some lower back issues though nerve test does not show radiculopathy 😮 but I do feel ever since am injury 2 yrs ago my feet and lower extremity calf, ankle(one injury) tibial etc which I was diagnose may still be from piriformis muscle/sciatica etc. need a good chiro. Back surgeons, pcp, neurologist are not helping. Though some recent PT had helped it didn’t make it go away. They also used dry needling and not sure if that just irritated my muscles. I do have fibromyalgia…
    I will purchase the belt though and see a foot Dr again this time get fitted for my own orthotics (I have flat feet) I’m sure that doesn’t help the tibial…thanks so much! God bless

    Glad to see you are back, I have been sporting the Serola daily for over 3 months now, and getting better slowly. I was bad, fighting it for over 6 months badly. Until I started using the belt daily getting out of bed was tremendously painful. It is getting better but still the most painful part of my day is the first moments of getting of bed are absolutely the worst, the next hour I just stand around until everything settles into my upright self and then I can begin my day. Thinking I will wear it to bed for a few nights and see if it can help me get out of bed without so much pain in the morning. Any thoughts would be appreciated.

      Glad to be back.
      Wearing the belt to bed can be helpful when someone has their most pain upon waking. I would look at sleep posture and see how that can be improved if it might be flaring things during the night. A pillow between the two thighs for side sleepers. I hope you continue to feel better and better.
