Sciatica Won't Get Better Unless You First Fix These 2 Problems

  • Sciatica stays much longer than it should because of 2 problems that rarely get fixed. And most healthcare professionals won’t know to tell you to fix these 2 problems.
    If these 2 problems don’t get better, then any other treatment you do will only give you short-term relief. Getting long-term relief requires fixing these 2 muscle imbalances so that you can have long-term pain relief from sciatica.
    These muscle imbalances are foundational to body movement and stability. Without correcting them, there’s a very slim chance that sciatica will go away for good.
    In this video, Dr. David explains why sciatica won’t get better unless you first fix 2 foundational problems.

    Dude. I’m a physical therapist. I have sciatica from a yoga injury 7 months ago and have watched lots of videos to get myself better (yes I know what to do but I also DONT know what to do). It’s a complicated injury to say the least…lots of stuff wasn’t working. This video is sooooo good for patient education. The best I have seen and heard.
    Nice job!! 🙌🏻🙌🏻🙌🏻

    • @VanessaDanielle444
      @VanessaDanielle444 Рік тому +18

      @@vcash1112 I was in standing bow pulling pose in hot yoga (standing on left leg) on 11/5/22. Been doing it for years. After the one minute posture I was moving out of it and I heard an audible ripping sound, my left hip shifted, and I could feel the swelling. Couldn’t walk on it for a week. Had radiating posterior thigh pain, couldn’t straighten my left leg. Got an MRI order from doc in ER. Results: Hamstring tear, quadratus femoris strain (unsure of grade), hip labral tear. Sent to surgeon. She said: lots of edema in there. Give it six months to heal the hamstring.
      Now I’m sure I was set up for this due to an old left side back injury that would give me trouble (hence the reason I started yoga years ago). I figured it was just QL tightness but yes the QL would tighten up I think because of a disc problem? Also to note-I felt my back when I was walking just the other day and noted left side paraspinal atrophy, so who knows how long I had inactive lumbar stabilizers…). Long story short I went back to the surgeon. She sent me to PT again. I did it initially for a bit (November until January) and thought I could continue with exercises on my own. Ha! The radiating posterior thigh pain continued and due to compensatory movement patterns from walking funny, my back got so bad I almost had to take off work. I’m in an outpatient neuro clinic full time but also PRN at an ortho clinic every other week. So I Went back to PT. It’s helping…I guess. I know all the exercises to do and I do the. Biggest thing that helped? When I discovered self traction by leaning forward over a table or bed, reaching overhead and pulling, letting lower body relax. That would “unlock” a spot in left side lumbosacral area and then I wouldn’t have to walk right side bent and right rotated and it reduces the thigh pain/cramps. I keep having posterior thigh pain on/off consistently. It’s not the hamstring strain. It’s sciatica. PT and surgeon agreed I needed a diagnosic hip injection in the joint to see if that helps. In addition to these symptoms, I have very specific burning groin pain, a feeling of stiffness and pain after sitting, etc. As long as I avoid that anterior pelvic tilt my body prefers on that left side, it’s better. Unless I over do it and then my leg is just tight and groin hurts and posterior thigh hurts. Any suggestions? Hahaha! It’s a long story!

      Sorry to hear. But to be honest - and please be open-minded now - yoga is absolute fucking TRASH. The amount of people that get injured from yoga is crazy. I similarily injured myself doing yoga. 12 years of ice-hockey, 20 years of hard gym training, still, my two worst injuries (that I still suffer from today) are from only 2 years of yoga practice. Yoga has potential to be good, but most teachers and students have NO idea what they are doing. Also, I found after being involved in the yoga scene for a few years that most yoga practitioners have horrible posture, especially Ashtangis. Its just not a very well-thought out physical practice, its more of a cult. This becomes extra evident if you know anything about nutrition and hear what they have to say about it. God bless everyone, stay open-minded and trust your body.

      @rhiys 2 місяці тому +1

      @marbeeidk5892 2 months isn't a long time. Please hold on.
      Read the book "Back Mechanic" while you're on bed rest. So many people give in to the pain and get surgery. If you can get through the pain, you'll heal. The human body is designed to heal and adapt. Your back will do just that.
      Please give yourself time. I slipped a disc in 2019, and was in pain for 8 months until one day I woke up and felt amazing. Seeing my dad in pain after the surgery made me not want to go through with it.
      Glad I didn't and simply took the time to rest and read up on spine hygiene.

    Thank you for this. My sciatica has lost it's ever lovin mind suddenly and I can't even begin to even walk. I thought I'd broken my hip! Heard a pop exiting the vehicle and I've been in agony ever since.
    I lasted 4 days at home and ended up calling an ambulance, and after 3 days in the hospital, they couldn't find a single thing wrong.
    I'm in agony. No pain management at all...
    I absolutely can't sit or stand for more than 30 seconds, and now they want to do PT... and I'm game but once I move much it takes DAYS to recover enough.
    Earlier in the year I've had a number of surgeries (gall bladder and kidney) and have lost a significant amount of weight... and now this... I'm 66 and while I realize I'm too old for several things... being pain controlled and as comfortable as possible aren't two things I have ever fought with.
    End of the rope time... I don't sleep, can't eat and I'm fighting stage 4 kidney disease. I literally feel as if I'm losing my mind.

    4 month sciatica, recurring shoulder impingement, and recurring cervical nerve aggravation person here - I can't thank you enough for this video. I've tried everything short of surgery for my issues and never really gave related muscle activation a chance, due to not really feeling said muscles. Something about your explanation in this video made me finally sit down and focus on activation (just at home in a chair/on the floor in the morning). For the glutes, I even wiggled my legs and hit them a little with my hands to make sure they were relaxed... after a day or so of trying, it finally happened, finally felt my glutes give a hard contraction without my legs lighting up. Glute bridges feel totally different now, ab exercises feel totally different, and I even translated this to lower trap activation to help my cervical issues. This is a Come to Jesus moment that everyone in a similar position needs to hear - strengthening exercises aren't going to help if you can't talk to the right muscle, so take the time to sit down and say hi until they say hi back.
    Just had my first day back at light barbell squats and I'm feeling even better than on my best days.
    Thank you again and again

I'm not a PT, Physio, kinesiologist, or health care provider of any kind, so I wouldn't be comfortable advising someone else to try something I've extrapolated from information on other body areas other than stating that working on getting my lower traps to activate is what helped my issue - I'd look to @epmanualphysicaltherapy for their specific advice either directly or in their past related videos. I have 4 out of the 5 major bones in the spine that are misplaced and rotated. Ive had sciatica pain for 10 plus years. Ive had adjustments and went through physical therapy. Once i stopped stretching those muscles and started strenthening, weight lifting, and started using an electrode machine coupled with an ice pack. Im feeling much better.

    Thank you for this. Ever since my sciatica started like a year ago, I was unable to walk for almost 3 weeks when it pulled on both sides back to back. Right away I felt like my life had changed. I couldn’t even workout with out my lower back hurting, not even for 5 minutes, but I kept introducing workouts slowly. Little by little I started feeling like my self again, but at times I still feel a pull but at least I can work out and feel strong again. I can relate to so much of what you said in this video. Thank you for being so specific with your recommendations. I have been using both 1 leg and 2 leg bridges for years in an effort to maintain glute strength. Unfortunately, I did not avoid having my hamstrings dominate those exercises. Once I finally scaled back my work sets and truly worked my glutes without also activating my hamstrings I began to see improvements in my sciatic pain. Same with isolating my lower abs. My work sets invariably incorporated more upper ab work than lower and again, scaling back the work sets and being intentional to flatten the lower back has been a game changer. I feel like I owe you for a couple PT appointments!

    I went to physical therapy over 13 years ago and was told that since I had two emergency Caesarian sections (dr had to cut through the lowest abdomen muscles) I stopped using them. I’m order to get rid of my sciatica and lower back pain I would need to do the an exercise he mentioned in this video. My Therapist was so knowledgeable that she said if I work my way up to completing between 5k-6k of them that I’d be healed. I laughed but each week she added a new variation of the exercise using resistance bands and so I was adding more reps per day. By the end I was doing 100 per day and on the second or third month I was completely healed. It’s lasted all these years and now it’s wearing off so I’m about to start again on my own. It definitely works if you do them. They’re so easy you’ll think they’re not going to work but by day two if you’re doing them right you will know they work. Sore for days 😂

    Very helpful, I’m going through it pretty bad with my back condition but any help is a blessing at this point, I’m all ears and open minded to push the boundaries and limits of what the body is actually capable of. I’ll keep grinding and fighting till my last breath

    I've learned more from you in two videos than after several years of chiropractic and pt. I didn't realize that I was only making things worse with the stretches and exercises I was doing. This all makes total sense to me now. I'll be doing this for the next couple of weeks and we'll see what happens. Thank you so much for providing such informative and life changing content!

      @@epmanualphysicaltherapy Great news! My sciatica has really subsided and my hips now feel like their realigning properly. I can even get out of bed in the morning with out rolling myself off lol. This is honestly the best my back has felt in years. I really appreciate the information you provide here on UA-cam. It's truly invaluable and I will continue to follow your channel and advice. I would love to check out your 28 day program, but sadly I will need to wait as finances are tight at the moment. I will check it out as soon as I'm able. Thank you so much for all that you do!

      Darn, doing same, stretches & just made it worse after a month trying. I was thinking, I am just destroying myself. Dave's vids having given me hope. Using ice to numb the pain. Elevating leg on soft pillows has helped too.

    I'm 53 and I have had so many hard falls since I was a kid. I also majorly fractured my tail bone about 20 years ago and took years to *heal. When I had my daughter at 37 (all natural), my tailbone was again extremely painful. I also have hip arthritis and arthritis on my left hand. I'm not taking any medications or any treatment. I have a high tolerance for pain, but OMG the sciatica just makes me want to die! Thank you so much for this really valuable information. New subbie :)

      The same thing happened to me, a couple of really bad falls; and 2 major falls injuring my tailbone…bad sciatica pain ever since and I’m 70 years old.

    I found an interesting solution to 3 or 4 months of sciatica pain. I bought a complicated jigsaw puzzle. It was beyond frustrating. I spent hours STANDING at the table, slightly bent over the pieces, trying to make sense of them. About 3 weeks into this routine, I suddenly noticed my sciatica had disappeared. My problem had disappeared (and I ended up tossing the puzzle...lol)....maybe this is helpful?

      This recent time I hurt, yes! One of the best positions for me was to lean halfway over the sink counter, whether bathroom or kitchen...and just stand there haha. I can imagine doing a puzzle is the same thing...and I don't love puzzles enough to do one while I stand 😅
