in the Irish public system we have care plan templates. For example, if your pt has chest pain, go to the cabinet and take out a copy of the chest pain care plan. It will say the symptoms your patient has, the issues they present, and the goals of treatment. There's blank spaces for any extras or unusual specifics relating to your pt. All we have to do is sign them off!
