Worst case you're fully aware but have no control, your body is your prison. Best case? Your mind is in a happy place and they intentionally choose a happy place. They often don't truly lobotomize you because they need your brain fully intact and capable of accepting new information and to be adaptable etc. Also they do sometimes make it so you're fully aware of everything including pain as a punishment and you can't repent if you don't know you're being punished. Many for the Servitors here could be failed space marines. People who the process of becoming a space marines failed and becoming a servitor allows them to continue to serve.
