I have a doubt about the biopsy question, for more then one biopsy we use one primary code and add on codes for remaining biopsy hence the answer to the exercise#1 should be 11106,11103*2 -59? and not option D please clarify.
Hi Ms. Jay how are you? I have a question about the hierarchy of biopsies. You have a sample question that makes me confuse if the correct answer has an add on code no need to apply modifiers am I right? Thank you very much.
for biopsy codes example in answers options have two primary code acc to guide lines only one primary codes is reported additional type of biopsy is coded with add on codes so answer will be 11106 ,11103*2 --no modifier 59 required for add on code
Hi Praveen: Good Forensic effort. The answer is Correct 11106 ( Incisional biopsy), 11102 (Tangential biopsy first lesion), 11103 (Tangential biopsy additional lesion), it is appropriate to add modifier 59 for separate lesions(see CPT appendix A for a full rationale). Remember: Add on codes are exempt from modifier 51 not 59.

B for the delay of the flap
Hello 🙂
Is it right to use modifier 59 with add on code .as in question tangential biopsy code 11103-59? Please clarify.
Modifier 59 on an add-on code is not necessary, because an add-on code implies that the procedure is distinct. Great question.
