You think about how to solve their problems. Most companies in need of courier services are in need of someone who is reliable. Since you know that, you market accordingly. Here's an example of what that looks like, "Yes, Mr. Johnson, I know that you may have had a difficult time with couriers showing up late or not showing up at all...with my company we make sure we drive later model vehicles to prevent technical down time...and if we are delayed more than 10 minutes for any reason we reduce your fee by $20.." This is how you approach ANY business that you want to do business with. YOU BECOME OBSESSED ABOUT WHAT PROBLEMS THEY HAVE AND HOW YOU CAN SOLVE THEM. That's how you differentiate yourself from the pack.
Providing medical courier services is going to give you the most bang for your buck based on my research. Mileage is the most common way to price your courier services. Then charging extra when services are outside of your normal geographical 'service range' and or outside of your typical 'business hours'. Step #1 is to register your business at Sam.gov using the appropriate NAICS code for the Coach Bus/Limousine business; then obtain an SBA Certification (i.e. Woman Owned Small Business, Veteran Owned Small Business etc.) While you are doing that, you want to register your business as a vendor with your state government.

This is a good question; but I don't have the answer for you. I can take a guess and state: Make sure that your business is set up properly (i.e. LLC, Corporation etc.), Make sure your business has a professional online presence. Make sure you determine the target audience of doctors offices who need your services the most. Create marketing that talks directly to these doctor's offices that speak directly to solving their problem (whatever that problem is). You will know their problem better than anyone because you are in that industry. Just ask yourself, 'What do the doctor's offices struggle with the most when it comes to getting things samples from point A to point B?" Is it speed of delivery? Are the labs being delivered to only open during certain hours? Are their existing couriers dependable or are lab results being delayed because the existing couriers are not dependable? Ask yourself a slew of questions like this. Once you hone in on the thing that most doctor's offices are struggling with, that's when you know that you have a solid marketing message. Use that message to get in front of the Doctor's Offices, and that's when you'll be able to offer your services on a contract basis to them. I hope this helps.
