This is an old video (no offense to the poster!). Anyone getting a biopsy in 2024 should demand a trans perineal biopsy which DRAMATICALLY reduces the risk of infection.

    • @9neil

I agree. I had both and whilst the TP is more invasive, as you say iis much less likely to cause an infection. The TR is of course cheaper. If I had to do it again ( and I dont. My psa is now 0 and a test 2 days ago came back at <0.05) I would definitely go for the TP biopsy.

I'm going to have a robotic prostatectomy in Austin, Texas on April 9th. Looking forward to viewing your videos. I've been concerned about the catheter, mainly about accidentally pulling on it and yanking it out either when sleeping, getting out of bed at night or walking... or just forgetting about it and doing something unintentional. It seems that it was no problem for you. I will be having a discussion about this with my doctor's assistant before surgery. Thanks for taking the time and effort to make these videos, and cheers from Austin!

Hi Joe. i wouldn't worry about that. The catheter is taped to your upper leg with sticky tape, so you dont really run the risk of pulling on it, and it never happened to me asleep or when walking the coffee shop next day -and we did plenty of that. The catheter is no big deal.

Hi Neil, thanks for the video. I'm hoping to avoid this path. However having an engineer explain it, has been helpful. I guess not everyone can expect no continence or ED complications, but it's good to here there is some hope at least. The stats don't look real flash.

I just had the Biopsy last Tuesday, now waiting results. I'm an electrical Engineer, so actually having another Engineer explain it all works well for me. Thank for that. With respect to the Biopsy, my issue was the dressing they used, it was stuck down really hard and getting it off hurt worse than anything else, and risked upsetting the fresh wound.

It would be really great to get an update of how your going now after your prostate removal. I'm an Aussie who is due for biopsy soon I think. I'm a public hospital patient in Australia unfortunately as I know private health cover ( which I don't have) gives probably better quality service in medical fields. How old was he and what went wrong in the private hospital? If you don't mind me asking. I'm still waiting to have my actual biopsy on my prostate done as I'm on the public waiting list. My mri scan picked up a very tiny 3mm lesion and that's why they suggested a biopsy to make sure which is understandable. My PSA levels have thankfully decreased which I assume is food sign. My biopsy was recommended for the exact reason yours was. I'm also 69 years old and my PSA score was starting to rise. My father survived prostate cancer but his brother (my uncle) died from it. Your detailed description was almost identical to mine. Although I was asked to give myself an enema on the day before the biopsy. I had this biopsy done yesterday at the VA Loma Linda, California. I was very nervous about it to say the least - causing high anxiety. I was so nervous (afraid), I took an unauthorized Xanax one hour before just to keep my blood pressure from going too high. The doctor was very good and explained every step as she was doing it. My conclusions: Not nearly as bad as I imagined. Uncomfortable? Yes. Painful? No. The whole procedure took less than 10 minutes. She took 16 very targeted samples from my prostate. Lab results are pending.

The 12 samples were done quickly, under ten minutes, and the only thing I felt was a tingling in the penis. She said all plumbing is connected, and that can happen. Well,Neil, all i can say is that my experience of a biopsy was totally different, here in Cork, Ireland. Mine was done over local anaesthetic and overall was pretty awful. Firstly,i was given local anaesthetic up the perineum. Then followed four long core needles, one of which really hurt. The camera probe up back passage was very uncomfortabe and found it difficult to sit down for couple days! Nearly three weeks later, I still pass blood in my urine. I did ring the hospital...nurse said keep drinking plenty of water! I am just dreading any treatments i have to contend with as its 99 percent sure its cancer

Hi Gerald, I'm sorry to hear of your experience, and i'm surprised that a transperineal biopsy was done under a local, and not a general or least with midazolam. Its all about the cost! (Midazolam is an IV tranquilizer and is safe and is magic stuff , in that there is no memory of the experience at all. I've had it 3 times now for other procedures, and have no memory of any pain or unpleasantness at all.) Passing pink urine for a couple of weeks after happened to me but was not an issue. If it is cancer I hope you have caught it early, and all the best with the future. ( good rugby games you guys played recently. Well done !!) neil NZ

My first biopsy was back in 2014 was done transrectal. Blood in the urine for over a month and in the semen for months. Then 45 days of radiation therapy. PSA remained low (.64) for about six years and then started to rise. PSA got up to 10 and I had another biopsy, transperineal done under anesthesia. Then I had cryotherapy. Last week I had my third biopsy. This time transperineal under local. And man did that ever hurt. Those needles were about 10 to 12 inches long. About six needles to numb you and then twelve biopsy needles. But as painful as it was, it was quicker, had minimal prep, and so far recovery seems quite a bit better. Haven't talked to the doctor yet, but have seen the pathology report. Many areas that had shown cancer before now are clear. However, one area that had shown no cancer shows perineural invasion.

These cause frequent hot flashes which are particularly debilittating in a warm environment or on a warm sunny day. The radiotherapy itself was fine except for bowel problems. PSA now down to .045 which is good news...now if only i could come off that Firmagon. Prescribed for two years,I may get off with 18 months as doing so well. Neil, my experience last October was almost identical to yours with the exception of the communication, or lack thereof, at the start of the procedure. I wasn't ready for the first snap and I jumped a little. The doctor asked what was wrong and I told him the noise startled me given I was a wee bit tense. I told him that I would appreciate him communicating what we was doing before he did it, and after that all went very smoothly. I was tense for a week or so prior to the procedure, and apprehensive as I sat on the table waiting for him to come in and looking at the apparatus, but afterward I told him that I'd do it all again in a heartbeat if he told me we had to. I'm almost done now with a month and a half of radiation treatments and it has gone smoothly.
