    Im taking fin 1mg 3 days in a row and then 1mg dut. Once a week laser cap. I have stable hair loss but want to regrow my crown. Slight progression again of hair loss. I’m willing try oral minoxidil, 2.5 daily? And should I just go daily dut instead? Do hair laser work? Is there anything new to look into for hairloss?

    I have read that with MPB, with each hair cycle, the miniaturization of hair in affected areas proceeds more and more. So tellogen affluvimum shortening the anagen phase could also mean that it is shortening the hair cycle and therefore more hair cycles in the same period and could result in more miniaturization of affected areas? Donor areas won’t be affected as there is no miniaturization of those hair follicles?

    • @DrParissis
      @DrParissis 4 дні тому

      @@princejoseph8280 great point. Together they will cause a cumulative effect and worsen the overall appearance of your hair. An important point though is that telogen effluvium doesn’t actually permanently shorten the anagen phase like in MPB (there is no miniaturisation) - it just switches the follicle from the anagen to telogen temporarily which may be reversed by minoxidil INDEPENDENT of MPB. Therefore, if someone is losing hair rapidly EVEN IF they have pre-existing MPB, TE should be ruled out by asking the patient about recent stressful events and treating with minoxidil accordingly if so. TE can affect the donor area - MPB does not.

    I unfortunately was a guy similar to the guy in the last example and got a HT done in Turkey. The surgeon never explained to me the risks and what was meant to be a 2500 graft procedure to address my hairline and frontal area which had receded became over 3500 spread across my scalp, which ended up damaging the native hairs I had in my mid scalp. I basically ended up with less density that before the operation. Going into the surgery I was unaware of things like permanent shock loss and transection so I urge fellow diffuse thinners to be careful.

    • @DrParissis
      @DrParissis 12 днів тому

      @@MrMolotov69 I’m very sorry to hear this. The issue with these clinics is often transparency. Working into an area that sacrifices native grafts is also a rookie mistake - as the surgical process damages them and you can end up with lower density than you started with. Hopefully things improve for you in the future!

    Hi Doc! Thank you for the video 👏. My hair transplant surgeon doesn't want me to use Tretinoin, as he said it isn't good for wound healing. I miss the anti-aging benefits of Tretinoin. Do you think I could re-introduce the Tretinoin into my skin care routine, if I leave out the recipient area? I'm one-month post transplant, and wonder if a strong 0.1% topical Tretinoin solution on a larger surface area could lead to a higher systemic absorption and cause hairloss, given that Isotretinoin causes it? Thank you 🙏

    • @DrParissis
      @DrParissis 12 днів тому

      @@AyaneBKing there is only a 3-5% risk of hair loss from oral isotretinoin. Topical shouldn’t affect the grafts in most patients - but I think in large quantities it might. Taking a break from the cream as your transplants grow it is not a bad idea to minimise any risk.

I also look 12 years younger than my actual age. The point is the "feminizing" drugs work best. If you want to stop hair loss, this is the right way to treat it. It's time the dermatologists open their minds to this for men and especially those who want their hair back no matter what! I still look like a man despite the regimen I am on. Additionally kenalog-10 shots monthly can help deal with the inflammatory factor in androgenic alopecia.

      @@mbmart2005 hello. Regardless of the exact pattern of loss - if it is advanced Male Pattern Baldness in the form of diffuse loss for example, then the most number of grafts is needed. Not a clinician/work around pharma industry, but I don't see why there would be. The reason Fin and Min work together as a regimen is their different modes of action. Doing the same thing in 2 different ways just introduces uncertainty about what dose you're really getting, and ups your risk of side effects. A safer route would likely be to talk to your doctor about, say, going from 2.5mg to 5mg oral dose, then seeing how that goes for you. Again, not a clinician.

    Personally I think it's the skill of the surgeon that matters most behind the technique when it comes to the FUT vs FUE debate. Yes FUT was the gold standard and still has it's place (especially when a large number of grafts are required). However with surgeons today like Dr Pittella and Dr Sethi and Dr Zarev, etc, FUE offers some things over FUT. For one thing you can 'cherry pick' the type of graft required to create a soft frontal hairline, soft temple points, etc from extracting from behind the ears. For patients like myself who was a Norwood 7 with two previous bad surgeries from the late 80's that failed, FUT was not a choice. However for a first surgery for a patient it makes sense to start with a strip and if/when MPB continues you can always augment with a smaller FUE surgery to keep the integrity of the scalp donor looking as untouched as possible. Thanks for sharing.

      @@Merth667 nice, what side effects did you get with finasteride? Dutasteride supposed to be a lot more potent and prone to side effects according to some, like in this video says if you take it every day. But in some other videos they say chances of side effects are lower.. I'm thinking of switching to dutasteride but not everyday, maybe 3 times a week... If what this video says is true, then I'm good.. I heard dutasteride is better at stopping hairloss in the hairline area, which I'm trying to do currently.

      @@neometalx9 Finasteride gave me erectile dysfunction. With dutasteride my sex drive is only somewhat more muted I think which is fine because it was unbearable otherwise. The plumbing still works great. My hair recovered a good deal on dutasteride, I have more hair than my 14 year old cousin up top. In the hairline the damage was done, I had to get a transplant to get rid of the norwood 3.

    Based on this, would you only ever really need to take Dut 0.5mg 7x/week if you have severe balding? could you do DUT 3x/week and Fin 4x/week or if your doing the switch should you just get rid of the fin altogether?

      @@adamlapier4520 with dutasteride blocking type 2 enzyme 3x stronger than finasteride, there doesn’t seem logical benefit to continue finasteride on top of the 3x week dutasteride. The strength of 3x week dutasteride appears to render finasteride quite useless.

    Very interesting video! Thanks for the information, doc. Question: what do you think about combining both medications on patients who have not experienced any side effect from fin? For example, a person who has been taking 1mg fin daily for an entire year could add to the regimen 0.5mg dut caps twice a week instead of the finasteride pill. I think that could work as a little boost on the already well tolerated treatment and the risk of getting sides is very unlikely.

    Both studies have very low sample sizes.

      @@davidomeara4704 I agree. Although I felt 60 patients (in the second paper), is enough to make a few interesting conclusions. Of course more research is needed!
