How We Fix Botched Dental Implant Treatments
Prosthodontist and Implant Dentist, Dr. Daniel Tan, reviews a case where we re-treated a patient who came to us with botched dental implants. When he came to us, he had already had all of his teeth removed and many dental implants placed, some poorly placed and in inappropriate positions. He had not worn any teeth at all for several months. He wanted to have FP1 implant bridges, and the case was challenging for many reasons. Dr. tan gives a slide presentation showing every step of the case from beginning to end. The patient ended up with full mouth (upper and lower) FP1 implant bridges.
Many patients looking for full arch or full mouth implant bridges are told they cannot have FP1 restorations and that implant dentures, better known as “All-On-4” restorations, are the best (and only) option for them; that those cases are few and far between. MANY times this information is simply not true. Our practice does these FP1 cases every week. While it’s true that the “All-On-4” is a very popular and commonly done treatment, this treatment protocol yields a FAR SUPERIOR final result. Our UA-cam channel has numerous videos on this subject.

    Hold on a sec. You say that the procedures were designed for people that had already lost their teeth. Yet the picture of the angled implant posts were in a mouth with teeth and the angles looked like they were going to 2 different spots to replace individual teeth . Also the angled posts there make sense to me for that . Like they would hold better and have more bone surface to adhere to. Can you clarify?

      My point about All-on-4 being for patients already missing teeth AND BONE is those patients only have bone in the front of the mouth. All-on-4 bone removal (alveoloectomy) procedures turn everyone into the same patient (those missing teeth and bone for years). Hence, the need for angled implant position to increase the distance between implants (A-P spread). Most patients with some or all of their teeth usually have bone in the back of the mouth which can be used for parallel implants without bone removal. So. instead of four or six implants with some or all being angled those patients with teeth (80% of those presenting for this treatment) can have 6-8 parallel implants that will simply replace the roots (without bone removal) and result in an "implant bridge or FP-1. If this doesn't answer your question, please rephrase.

    Do either of these procedures have any impact on the natural fricative or sibilance of the voice. Air passing through the gap between the prosthetic and the gum line might be an issue for a singer, perhaps, dunno, just asking.

      Excellent question, and yes, one does. The All-On-4 would be likely to eventually cause the issues you mention. The implant bridges (as shown here) fit intimately up against the gums without gaps between the prostheses and the gums, therefore, eliminating this space and air flow issue.

    Man oh man it brings tears to my eyes. Im so hurt and sad. I wish there was something i could do now. I sold my house to go to Cancun to get 3 on 6 I woke up from anesthesia and they cut almost a inch of bone off. Yes i said 3 on 6 but i knew right when i seen what they did. I would never have what I want. My heart broke, I hate myself now. I can go on and on im day 5 with my all on 4 and they did everything wrong with this to. Im miserable and now I'm broke as i could be. They robbed me of everything.

      Another "All-on-4" tragedy. I am so sorry to hear this. Like I say in the video, many All-on-4 dentists will call their treatment by any name possible to make the "sales'. "3-on-6", "implant bridges", "implant reconstruction"... whatever it takes to fool the unsuspecting patient. They also will not disclose the "alveoloectomy". Did they tell you about that? Where to you live?

      @davidmcfadden7927 my story is really long to put all of it on here. I talked to them for over a year. I have hundreds of messages between me and there team. I would love to tell you about it. It's 3 years of hell. I studied for almost 10 years before getting this done I knew everything that went into all the possible procedures the 3 on 6 the all on 6 and the all on 4. I knew what they did to make the all on 4 possible that's why I waited so long to get the 3 on 6 so I could payoff enough of my house so when I sold it I had enough money to cover the 3 on 6. I paid almost an extra $10k my first of three trips to Mexico to have all this bone graft done. My first trip i had a sinus graft done both sides and I paid for every tooth that they pulled to be grafted. I woke up for anesthesia to me it didn't look like they did any grafting. Then 6 months later I went to get my implants put in. That's when I woke up and they had cut all that bone off. My heart sunk. Also they only put 4 of the 6 implant in. They had the nerv to tell me they can still do the 3 on 6. I knew they couldn't cause I've watched thousands of videos and knew everything. I gave them everything. I can go on about more of my story, but I know you're a busy man. I feel like I will never be me again.

Utilizing a highly focused and talented team like the crew at McFadden Dental Implant Center of Dallas, you can be confident that you will receive and greatest of care tailored to your needs and condition in order to achieve a better smile/outcome than you could even imagine. Traveled from Florida for a full mouth restoration ~ ZERO pain through the process, a little discomfort on the days of surgery, but honestly no real pain, the combination of sedatives and gentle surgery really works wonders! ~ 100% satisfied with the results, with both how my teeth look and even better yet, the function of using them to eat with. The final results truly do look, feel, function and clean as real teeth!! FULL DISCLOSURE - The day the final sets were installed in my mouth was the ONLY time I can say there was any pain.     I think everyone needs to brush up on proper nomenclature. Anything that is fixed in the mouth and can only be retrieved by a dentist is a bridge. A removable prosthetic is a denture in some variety (technically all of the above are dentures but let's keep it simple for now but ask if you have questions). Dr. McFadden is right on that an AO4 is ideal for someone that has tried everything else and lost significant bone. The type of prosthetic is referred to as an FP-3. If a bridge is "tooth only" it is an FP-1, if the tooth appears long, that's an FP-2, and if it has any pink at all..... ANY AT ALL..... It's called an FP-3 but all are FIBs. FIB is a fixed implant bridge. This nomenclature was established by Carl Misch back in the 80s and widely accepted worldwide as a way to classify a treatment approach. FP-1 or what he is calling an implant bridge, it quite difficult and everyone thinks it's more esthetic but think about where the strength comes from. An FP-1 must have large connectors from tooth to tooth to provide strength where an FP-3 gets strength from the pink portion so the connectors can be smaller and the teeth look more "tooth-like." What Dr. McFadden highlights is how the big chains treat AO4 type of patients (AOX) and it's like a cattle call. Lot's of misinformation in the industry and Dr. McFadden is dead on in that we need to craft a treatment plan to fit that patient sitting in the chair. Sometime their risk profile is conducive to an FP-1 but sometimes it's FP-3. We have to craft the treatment to that person and not like many of the big chains are guilty of which is one size fits all. But let's get the names right..... All are fixed bridges.... If it has pink, it's an FP-3. If it doesn't, it's a FP-1 or 2. Hope this helps! Dr. B

      Yes, there are. THIS type of treatment is not for people who simply have cavities. For those patients, we simply fill their cavities or make crowns for their teeth. This full mouth restoration treatment is for patients with much more advanced needs than simple fillings or crowns.

    THERE IS A DENTIST ONLINE THAT IS SAYING THAT IMPLANT BRIDGE GUMS WILL NOT HOLD UP NO 10, 20. 30 YEARS FOR NOW. THE GUMS WILL FAIL. IT WILL NOT HAVE THE SAME APPERANCE. THE PERSON GOING TO HAVE BLACK TRIANGLE AND THE PERSON IS GOING TO LOSE THE PAPILLA. HE SAID THE GUM TISSUE WILL NOT LAST MANY YEARS. THAT THE REASON WHY HE IS AGAINST THE IMPLANT BRIDGE MOSTLY. HE SAID OF A PERSON GET A PF2 THE PERSON GOING TO HAVE SUPER LONG TEETH AND WILL BE LOOKING LIKE A HORSE OR OTHER ANIMALS IF THE PERSON IS A FEMALE.       Well, I don't know how long he's been doing implant dentistry or how many cases he's seen 10, 20 or 30 years later, but I can tell you this: I've been doing this for well over 30 years and have numerous patients I did this treatment on 20 years ago that I still see and theirs are holding up extremely well. Good oral hygiene habits play a big role in this!

    Is there any chance that even with the all on four treatment having been done, you are able to do something along the lines of this? It is so unfortunate that dentists are selling the all on four and patients find out after the fact what they have to do!!

      We can usually improve the prostheses significantly but that bone removal bell cannot be unrung. Meaning, the FP3 that is the result of All-on-4 cannot be turned into an FP1 or an FP2.

    I spent 12 grand for 5 implants in my top gum I didnt need any bone graft.my gums were healthy and I had plenty of good bone. My crooked fractured teeth were removed and I got a plastic bridge with 12 fantastic teeth a little longer than my original teeth. This is a tempory bridge and I've recieved 2 more temporary bridges each feeling a little more comfortable than the previous. I've paid 2 thirds off the 12 grand. Once the balance is paid I get a smoother bridge made from titanium and 12 porcelain teeth attached to it. Believe me it's not what they look like it's how they feel especially eating. I've never enjoyed eating I feel I've just got a denture in my mouth very thin yes but a denture held in place by 5 implants. Hope fully the final bridge on 5 will be a game changer.

    So with an implant bridge are you saying that every one of your top 12 teeth requires 12 implants instead of an all on 4 which is 12 teeth attached to a bridge by 4 implants. ?

    Very informative thank you for providing these slides they are very helpful. I live in 🇨🇦 10 yrs ago my upper teeth were removed I believe most of them were removed for profit! 4 implants were placed 3 months later. I was not provided with a treatment plan and was sent off to a dentist for a denture to be put on my implants.(it's been a nightmare for 10yrs) this was strictly a denture with 4 liner holes for the implant (awful) 2 of the implants failed due to bone loss. I am now starting the process for replacing the lost implants a 3D panoramic x-ray shows that I need a sinus lift at 68yrs old this is my one chance for a better quality of life. I have made a lot of notes from your video and will be asking my surgeon a lot of questions. I definitely like the idea of a one piece implant bridge verses all on 4. Thank you so very much for your video on this issue.

      It’s hard to advise anything without seeing a current X-ray, but hopefully, she’s a candidate for an implant bridge to replace the four missing teeth. If she has enough bone for implants, most likely a four tooth bridge on two implants.

    Dr. McFadden I thank you for your honesty. I have been listening to and watching the videos of another dentist about implants and I thought they were being honest but I have never heard them discuss bone removal. It is really unbelievable the greed and misinformation associated with all on 4.

    Can you please answer the dentists who have ask about classes & teaching this technique? Im curious why they don’t know though, is this not taught in dental school?

      I have given a few classes but, honestly, the technique is so advanced that the only people who can even begin to start this learning curve are those dentists (mostly surgical Prosthodontists) who have done dozens of large cases. I will release a video soon that demonstrates why it's so difficult which causes the treatment to be very rare. I literally just finished a surgery and I feel like a ran two marathon. The surgery is difficult but the real limiting factor that limits these treatments to Prosthodontic practices is the immediate (day of surgery) fabrication of the FP-1 (implant bridge temporary). If you haven't seen my "Why Some Dentists are so Much Better than others" please see link below. It probably sounds elitist and I don't mean for it to be that way. It's a really time consuming, delicate, precise surgery that most dentists (especially Oral Surgeons) do not have the patience to perform. A Prosthodontist must be involved for the entire day so it's also a logistic nightmare for that office. We have three Prosthodontists here. I hope this explains some of the reasons...

    What about patients that have some form of facial neuralgia pain such as Atypical trigeminal neuralgia or atypical odontalgia that was caused by a dental prcedure or dental injection. I have been told that inplants would be asking for more nerve pain. Not sure if you are familiar with how many people suffer lifelong pain from nerve issues that started with dental procedure but would like to know if you have any knowledge of this. Most dentists either claim ignorance on the subject or just do not want to admit that dental procedures can go terribly wrong and cause people to suffer from pain and or numbness the rest of there life

      I am aware of these complications. Fortunately, I have never had a patient suffer any of these life-changing outcomes. I would certainly proceed with caution. I would need A LOT more information to advise.

    I live in upstate ny so obviously it wouldn't be Feasible to travel to Texas for treatment. is there any dentist in the northeast that you could recommend?

      Try as I have, I have not yet found another dentist in that area who follows the same protocol as we do. But with the information you glean from many of my videos, hopefully, you can interview some prosthodontists and know which questions to ask and be sure to ask to see lips retracted photos of their work.     I had a dentist recommend all on 4 for me because I have 5 teeth that need implants in my upper jaw. The problem I had with his recommendation was that all my front teeth are in excellent condition, except for a chipped front tooth. They are strong and healthy and perfectly aligned, yet he wants to extract them all and do all on 4 because I need implants in my back teeth. I said absolutely not. He seemed annoyed that I couldn't see the benefit of his suggestion. He even sent his assistant to try and convince me.🤔 Is it common to extract good teeth for this procedure?

      Because you’re only paying for each phase of treatment AS IT IS DONE, and not the entire fee for ALL treatment which, in some cases may ultimately take 12-18 months to complete. In other words, you’re not paying 12-18 months in advance for treatment that is yet to be done.

    Will the Interdental gingiva (papilla) eventually fill in the gaps if a zirconium roundhouse 12 tooth with no pink and 7 full size implants that were all bone grafted is screwed into place after wearing an interim denture for about 8 months while bone grafts healed?

      They are most likely as large and full as they are going to be. Some minor improvement may happen if the permanent has more pressure on the tissue than the temporary.

    Hello Dr / thank you for your upload / I am going through a terrible stage in my life - as I was assaulted last year which resulted in my upper two front teeth removed - and the lower four front are not in great shape as they were also impacted / so I will have to get them removed most likely in the coming years / it has destroyed my smile and ability to vocalise like I used to - which has shattered my life - and sense of well being - I have been diagnosed with severe periodontitis- I am wondering do gums have to be completely pink to receive a form of treatment?? Or can it still be possibly not to get a lasting effective result while still having poor gum health / as I think it may also be hereditary as I cannot really remember ever having pink gums - well maybe only as an infant to adolescent but even then I am not sure if i had great gum health - it has been about 18 months since teeth were removed - and i am also worried about the length of time it has been and damage done by denture - which i have been wearing for about 10 -12 months .. i have a partial denture for my front two upper which attach by hooks on my canines . but i sincerely hope to get optimal treatment to bring back my bite force and ability to vocalise - do you think there will be a possible avenue / that is not on the extreme end of costing / thank you for your time - i hope to hear back from you

    43 years old. On permanent disability. I only have around $16,500 saved, with 805 credit score. The problem is my income is only $963 a month. Had to pull all but 4 on bottom front teeth removed between age 25-30 my teeth started massively failing. Root canals with crowns were at least $1700 each. My only option, according to my money hungry dentist wanted to do a lot of root canal/crowns. He could have done fillings I'm sure on several of them. I ended up caving in and had to have them pulled. I don't leave the house hardly ever. I wish someone would run into me while walking down the road so I could be "deleted". I haven't had a life since my early 20s. Is there any hope for me?! By the way I live in Southern Illinois, so I'm 11 and a half hours at least away from you. I have headaches, and jaw pain everyday. All those teeth being pulled I just know is what caused my trigeminal neuralgia, which is so painful it's known as a suicide syndrome from the pain. Also, I have tried two sets of cheap dentures and I could not wear those things. I couldn't talk right, and they hurt so bad, fell out etc. and I also had to have at least 4 Alveoloplasty procedures years ago while trying to wear those basic dentures. Little splinter pieces of jawbone would poke through my gum. I haven't had to have one of those in many years. I was told about 6 months ago that I had a good amount of bone left from a dentist.

    So I went to see a dentist in London, and she airily proposed 'angled abutments' as if they were the most natural, obvious thing...Yet you vehemently oppose them. Are they bad because they aren't seen in nature, or because they have bad outcomes, long and short term?

      There was a book written in the 70s called, "Dentistry and its Victims". I have not read it... rather I have lived it. If I had more time, I would enjoy writing that book !!!
