    The disturbing sound was killing, the imbalance and dizziness, PPPD was alarming,and every other symptom was breath taking.     Walked into a low hanging sign a few hours ago and am now feeling dizzy and tired. I have a history of concussion so I am worried. I feel a bit unbalanced but I have longstanding vestibular issues related to previous concussions. Worth pointing out that it's hot outside and that I walked for quite a long time, which may also contribute to the fatigue or the dizziness. Should I worry?

    I had post concussion syndrome for 4 years until I found an OT who told me to stop eating ALL sugar. Once I stopped eating sugar, after 3 months all of my symptoms went away. All. Of. Them. My brain healed, then my gut healed, and after 10 months of no sugar, my arthritis is gone and weight is beginning to drop. Sugar is poison.

    What if you have no headache...i accidentally hit my head on Thursday about 14:30 pm...maybe there is dizziness or lightheadedness...but i dont think i had a headache...its now 3 days later sunday...i did not do any workouts after that...only walking

      There are 22 symptoms on the post-concussion symptom scale (PCSS), and dizziness is one of those. Headache isn’t required for concussion, it’s just common. 🧠

      Symptoms MAY increase with physical activity after a concussion, even if you’re symptom-free at rest. It’s not a “they will” statement, but it is very common for folks and “may” happen.

      @@drmarkheisig I accidentally hit my head against an iron door...I didn't have any symptoms the first day...I have a weird feeling or something like slight dizziness or something or maybe it's something else... approximately 23 hours later,and I guess nothing else...I hope it's not a concussion....

    I have a question regarding the temporal arteries in PCS. I have post concussion syndrome and my temporal arteries swell and become painful when I do prolonged cognitive activity. Any insights why this is happening and what I can do to stop this? Thank you in advance!

    The plastic on helmets are designed to slide/ bend on impact that soft cap takes away that function and the impact is directly into the players head inside or into the helmet. Since the helmet can’t slide on impact the hit force goes to the head. This caps are some money grab/pr move by the nfl but not for players safety

    Thank you for taking the time to respond previously, despite your busy schedule. Knock, again I have a question !! About the distribution of mechanical stress during a concussion. Specifically: 1. Even distribution:-Does the mechanical stress exerted on neurons during a concussion affect each neuron equally, causing uniform damage to all neurons not a single neuron out of billion in that area undergo more than that degree of damage due to equal distribution?🫤 Or 2.Uneven Distribution of Stress:- Is the mechanical stress distributed unevenly, with areas closer to the point of impact experiencing more strain and damage than neurons further away? For instance, in a mild concussion, are there focal areas of the brain where some neurons suffer more damage due to their proximity to the focal impact site, while others areas just adjacent to that experience minimal damage due to less proximity to the point of impact ?😨 If the second point is accurate, could a mild concussion, even with a lower G-force (e.g., 60 force), still potentially damage neurons that are particularly vulnerable due to their location relative to the impact? Could this result in severe injury outcomes, such as axotomy or severed axons, for a few neurons?😱 In summary, does the damage to neurons result from unequal mechanical strain, or is the force causing a uniform degree of damage across all affected neurons? Thank you for your insights.👍

      I’m not sure the benefit you’re getting from this answer: but it’s likely all of the above - an unequally & widespread distribution with a consistent autonomic effect due to rotational forces impacting the brain stem. Neurons are unlikely to die in mTBI/concussion.

    if someone temporarily yet gently removed my head from my neck and shook it around to collect the loose change inside my skull, then reattached it to my neck without moving it at all, should i be worried about whiplash? hypothetically

      The scarring/repair from reattachment - even clean - would do a number on your neck likely resulting in headaches requiring manual therapy, ONBs, etc. Best to check your couch cushions and car seats for change first 🤙

      We have data (from cats) - no neck movement, no concussion. BUT we can get more severe forms of TBI secondary to external skull trauma. So you found the loophole into, potentially, more severe injury 🤷‍♂️🧠

      If I had to guess, your neck endured a “compression injury” while your brain “went for a ride.” Also note: there’s no such thing as mild, severe, grade II, etc. concussion. In 2024, it’s the “subtypes/profiles” that we use to categorize concussion.

I'm concerned that even if a concussion doesn't occur, inflammation could still damage brain cells. ☠️ Additionally, do subconcussive hits or subconcussion also lead to BBB disruption and inflammation?"

      Hey there! Sub-concussion is incredibly hard to define objectively because we can't even objectively define concussion (mTBI). The DTI and fMRI studies aren't able to differentiate mTBI from orthopedic injury, heavy exercise, depression/anxiety, etc. So, again, it's really hard to define something "less than" something else we can't even define. All of that said, from what I know about things like COVID - there are metabolic factors that can impact BBB independent of trauma. So, ultimately, this question likely becomes an n = 1, patient-by-patient answer.

Angular acceleration (rad/sec^2) measures how quickly something is speeding up or slowing down its rotation, while rpm measures how many times it spins around in a minute. They are like the difference between how fast a car is accelerating and how fast it's going - you can't directly change one into the other. Stay humble, folks. 🙏❤️🧠

      "Delayed exercise intolerance" could point to issues within the neck (e.g., cervicogenic).

    What happened to me for the few months, I couldnt even stay awake even if I wanted to. Before the injury, I had energy over the top. More energy than mist people and was fairly fit. Now 10 years later, walking more than 30 minutes, i feel sick and need to rest. The symptoms are lingering on and on. I feel lost. My blood pressure is low but spicks high fast abd heart rate is 43. Any suggestions? Watching you through a ball in the air back and forth made me feel dizzy.

    I had a fall from a swing onto concrete. Rushed to hospital they treated my broken leg but never looked at other possible injuries. 6 months later I was having trouble walking, it looked like a line had been drawn down my body and my entire left side was paralysed. This has had a major impact on my life. If you have serious trauma insist on head scans and repeat if needed. If I had received treatment within weeks of injury I would not have a permanent disability now

      I'm sorry to hear about that! It sounds like you may have had an unrelated incident 6 months later - but regardless, your brain health should have been more accounted for in the acute stages after your impact. If any red flags were present on the initial assessment or within 48 hours, they should have performed imaging.

    1 month ago ,i hit my top of my head in the left side, no symptoms, just my head still in pain now , every day i can feel my head aching in the part that i Hit ,and pain reach in my left eyebrow, do u think it will go away time pass?

      While it's possible that symptoms may resolve on their own, the 2022 Amsterdam consensus guidelines recommend assessment/rehab for any symptoms persisting beyond 10 days.

    Hi Dr. Mark! I've had whiplash twice within 3 months of each other. I was diagnosed with a concussion for the second accident. I have cervical C-4-C7 & throatic spine T11-T12 bulged discs from my first accident & still cannot do certain exercises & had to stop P.T because of constant pain after going. I know the concussion symptoms are prolonging because of the cervical neck issues....I just can't figure out to get diagnosed properly to get the right treatment in South Florida. I have flavor type 1, 2, & 3. I am getting dti mri testing done & getting results this week & going to get a new evaluation from an orthopedist. I know they aren't going to tell anything different. I am having severe depression, etc, & memory issues. I still can't do simple daily tasks without causing nausea, headaches, or carry groceries or do simple exercises that cause delayed cervical pain & pinched nerve pain. I have looked into going to a concussion clinic for manual therapy, or structural integration therapy. I can't drive because of back, neck, & nausea and can barely do anything for myself. I am thinking of going to Complete Concussion clinic in Oakland Park, FL if not a similar place in the South Florida region. I follow The Concussion Doctor on youtube as well. Do you have any suggestions? Thank youuu! 🙏😊

      ​Great! I listened to him on the Neuro Wellness Institute Podcast. Thank you, Dr. Hesig! ​I was finally diagnosed by an Orthopedist this wk. Everything he told me I researched it all, it all makes sense! I am doing a new gym plan at home & it is helping a lot. I was advised for special PT therapies & other treatments.     Great video!!! Wish I watched this years ago haha :) But agree - on similar note but loosely related - I had a swollen / hurt ankle for 18 months - waiting for swelling to go down. I went on treadmill with light jogging for 30 min. My ankle hurt for a day (felt bruised) - but IMMEDIATELY the swelling went down (shape returned to normal) - and within two weeks I went from 3 mph to 8 mph without issue. I could imagine the same applies. Minor stress better conditions vs no stress / atrophy.

    Hit the top back of my head on the bunk bed frame when I was standing up 4 days ago. Didn't have any of the said symptoms. I just kinda feel a little pressure on the area. A month prior to this, I also hit the back of my head on a barbell bar as I am about to lie down to exercise. Also didn't feel any of the said concussion signs. Could the pressure that I'm feeling in my head right now a sign of concussion or just feeling the pain from hitting? Like some sort of soreness. Thanks!

    Okay so I got pulled by the hair off the ground, and slammed into the floor. The back of my head on the right side by the ear hit the ground. I just felt gushing in the head, room spinning, pain when I touch it, and a tingling sensation that lasted only for a little! When I walked it’s like I wanted to fall down. And also felt nauseous I did vomit hours later though. Next day my body is sore, headache, swollen kneck on the left side, and can barely hear in the right ear. Kind of hard to chew. Tired. What do you think? Is it permanent or not. Doctor said it’s a concussion, that it looks normal but I don’t feel like myself. Which makes sense

      Impossible to say via comments/symptoms alone. Proper physical exam & appropriate/active rehab is the best course to minimize the risk of PCS.

    Got into car accident not sure if I got a concussion but for two weeks felt sensitive,insomnia,dizzy. I felt better two weeks after just had the symptoms of insomnia, but 2 months in now and feeling still insomnia & some headaches but I’m thinking It can be due to stress because I’m unemployed so yea… not sure what you think I feel strong physically and got my balance.

      Hard to say. The work-up would include a symptom score, neurologic exam, vestibular-oculomotor screening, and Buffalo Concussion Treadmill Test. This would rule in/out the "concussion-specific" stuff, but beyond that, we would also need to look at your neck with specialized tests (e.g., JPET, SPNTT, Butterfly Test, etc..).     (F 25) it has been 6 hours since a wooden door fell on the left side of my head , i just feel slight light headedness/ a little sleepy and sometimes superficial pain because of the bump , should i still be worried ? Thank you so much ❤️ Edit : i've been having anxiety all day now i feel nausea i Don't know if it's from the anxiety or the injury help

      In concussion, your brain “stretches” and releases “brain chemicals” everywhere. These are what make you feel disoriented, nauseous, etc. When you keep playing/stay warm that keeps your brain warmer which may cause even more chemicals to be released & make the injury worse.
