Great presentation and review of policy changes. There is one blooper included... At 3:46, Dr. Schmalz states wide complex tachycardia less than 40 bpm... Oops! It's bradycardia. In the end with the other bloopers, OMG... that train! 😂

if i'm not mistaken, at 2:01 mins: you have it conc listed as 100mg/10ml = 1mg/ml......incorrect....correct conc would be 10mg/1ml. What am I missing??

Why do 2 min of manual CPR? I'd say manual until the device is ready then put it on as early as possible in a coordinated effort while minimizing compression interruptions.

I broke my femur almost a year ago. I was given a choice between a half or full dose of fentanyl, at least for my size. I was scared of overdosing so I chose the half dose. Also I was subsequently given a full dose of ketamine in my other other arm. Seconds later I couldn’t even speak English for a few minutes, super crazy ambulance experience. Recently became curious about the perspective of my EMTs and came across this vid. Love it, very educational and I appreciate the service all of you provide for the public.

The algorithm met criteria to omit leads 3 and 4 due to a QRS discrepancy (Glasgow 12-lead ECG Analysis Program). Due to the evolution of the ECG during an infarct, serial ECG’s should be done. Limiting yourself to “one good 12 lead”, limits the paramedic to observe, treat, report, and transport in the patients best interest. Instructing paramedics it’s only a stemi if the interpretation says *** stemi*** is an amateur method of 12 lead interpretation. computer assisted interpretation is a valuable tool when used properly. No automated analysis system is completely reliable, however, and interpretations should be reviewed by a qualified paramedic or physician before treatment, or non-treatment, of any patient.( Glasgow 12-lead ECG Analysis Program)

When I’ve tried IN fentanyl my patient INSTANTLY started tearing up and coughing violently. Eyes instantly red and nose/eyes tearing. Then dry heaved. I’m scared to try it again 😶

Are we only supposed to use it for external bleeding or is it ok for external bleeding?
I think there's a typo Jessica. It is designed for all kinds of bleeding, including postpartum hemorrhage, epistaxis, and trauma. In Sac County, we are only approved for trauma. Does that help?

Can you explain why someone would be injected with ketamine during a police arrest?
Most commonly the reason is agitated delirium
combative patients, it knocks them right down. Great drug and its usually given intramuscular and only takes a few minutes to achieve therapeutic effect on the patient.
Are you using the Titan 2 or 3 gateway? Titan Gateway 3 actually has audio feedback also recording feature on the 15.

Could you please explain the reasoning as to why you wouldn't advise the use of ketamine and opiates on the same patient in the pre-hospital environment ? I have, and have found they work quite well.
The combination, especially in the individual who is opiate naive, has a greater potential of respiratory compromise. It was a built in safety measure required by the state EMS authority's scope of practice committee.
