Great video Andy. The legal, moral, operational trifecta in organisational risk assessment is a difficult balance to achieve. Why do we do risk assessments? Not because we have to or the standard says so but that we want our people to be operationally effective and safe in achieving objectives (personal and organisational).

A number of years ago, about 40, I was invited, as a chemical process operator, to get involved in my company's move towards working under a Total Quality Management Philosophy. I accepted the invitation and then studied, presented and put into practice the philosophy of TQM and its tools and techniques at company training and education sessions. One of the main drivers behind the TQM philosophy I was involved with was that organisations (and most likely individuals and domestic households) were wasting up to 25% of their turnover. There were 5 main reasons that were identified for things going wrong in organisations causing this massive amount of waste. This waste of resources came in the form of wasted time, effort, materials etc in not getting things 'right first time'. The top 5 reasons for not getting thing 'right first time' were identified as (I can't remember off hand in what order): agreed requirements not being communicated, agreed requirements not being understood, agreed requirements not being taken seriously by management, people in the organisation not being trained and the process not being capable of meeting the agreed requirements. 'Everything is a process'. Much of what you have covered in your videos and in the way you explain the reasoning behind strategies, tactics etc takes me back to those early days of being blown away by how much sense the TQM approach made and how it turned a company at risk of closure into a profitable and safe company that I was proud to work for. As a TU H&S Rep I use this 'list' as a guide to my investigations into workplace accidents and incidents as a means to aid in identifying the root causes. I find it extremely beneficial when demonstrating poor management practices and failures as a significant contribution to an accident/incident as opposed to the often initial claims of 'operator error'. Thanks for some great informational and educational videos.

Thank you. That goes to show Lonnie that TQM in itself is a powerful tool that can be very useful! The 'Quality' perception is very much overlooked I find,... when in fact, taking a Quality perspective in both a wide and focussed sense will ensure we cover risk assessment at most levels.
As a Trade Union H&S Rep I really am impressed and in agreement with your deeper understanding of what approach and commitment is required from organisations that claim to be 'safety first' in everything they do, but are avoiding serious accidents more by good luck rather than through their efforts at good safety management. Thank's for another clear, useful and understandable video.

I don't mean identifying and doing something about risk is crap. Obviously not. But the way that ISO 31000 is written is crap and all these 'helpful' vids and texts are pure crap.
