- A rotationplasty is surgical procedure that provides an excellent cancer treatment option for patients with tumors in or near the knee joint, such as sarcoma. This specialized cancer treatment is best applied in children who are still growing.
During this procedure, the surgeon removes the affected portion of bone, along with the knee joint. The major artery, vein, and nerve leading to the foot are saved.
The lower portion of the leg is rotated 180 degrees counter clockwise and then attached to the upper portion of the leg.
The ankle is now on the same plane as the former knee, with the foot and toes pointing backward and the heel facing forward. Because of the rotation, the ankle bends in the same direction as the knee.
The surgeon will take into account the age of the child and their growth potential. Initially, the surgically-modified knee center will be lower than the unaffected knee. As the child grows and bones lengthen, the knee centers will match.
In three to six months, after the bones have healed, the patient can be fitted for a prosthesis. The patient’s foot fits inside the socket of the prosthesis and the ankle functions as the knee joint. In time, the patient will develop greater control, function and mobility. Most patients are able to walk unassisted and return to their regular activities, including playing sports.

it's all about making it as easy as possible to use a prosthetic leg. Much better to have half your leg removed, but with a joint that lines up with your other knee, then to have a longer (but still shortened) leg you can't bend.
The Sad part is, it is the affected Area :/ Bone Cancer spreads quite rapidly untreated.

I did lmao. I legit just randomly thought of "is reattaching limbs possible" this morning and found a kid with rotated foot on his knee in my search. Then i went to youtube.
I got so upset when I saw the ankle being rotated 180 degrees, but I felt such relief when I saw they are getting a prosthesis. Mad respect to the doctor or surgeon who came up with this!

This is really an amazing surgery, I couldnt believe my eyes the first time I seen that news story about that young girl who danced and could still dance even after they surgery. Out of all the surgeries this one looks to affect live the least. Amazing job whoever designed it and those surgeons who perform it.

My dad had surgery for his cancer and had to go to the doctors today for a check-up and to ask questions. I asked the doctor what was the craziest thing he has done at his work and he sent me this video and said he did this surgery two times.

The fact that they design prosthetic legs with the idea that someone now has a foot reversed at their knee is absolutely genius. That way they can have a prosthesis that actually bends at the knee and they have full control over it.

Dealing with it on an actual body is just as bad. I spent a whole dissection trying to isolate a branch of the brachial plexus (nerve organisation of the upper limb) from a cadaver and seeing it just dangling out there at the end was an experience.

I had this surgery nearly 20 years ago, I was 11 at the time and I had no idea what they where talking about. But I was amazed how easy it was to learn how to walk again, no trouble at all. Next to no phantom pain aswell. Really glad I was able to do this, instead of just chopping the leg off.

Wow you my friend are a warrior, i just had a medical scare where i thought i might of had cancer but everything turned out to be okay. i reevaluated my life, i stopped smoking and improved my diet. Surgery is terrifying to me i wont lie, so my hat truly goes off to anyone who takes that hurdle and keeps fighting everyday.
I was actually curious about the phantom pain. Glad you don't have it and hopefully other patients don't too.

I went into this wondering why this procedure exists. Then I thought, oh ok because of malignant tumors but, how does this help the person walk? Then they explained the prosthetic leg and I was just really impressed. That's ingenious.

I already know this kind of surgery, so I just clicked to appreciate the animation of this video. I would like to make animation videos of this style in the future.
As horrifying a thought that is backward foot as my new knee joint is, I'm super glad that this not only works but is very effective, apparently. Still, I will never unsee backward foot as a new knee joint. That will haunt me until the day I die, and hopefully no longer.
To be fair, I believe this is rather cheaper than just connecting a whole bionic leg. Therefore I see this as an absolute win.

I genuinely felt a little sick to my stomach for a bit and cringed. It's really offsetting seeing something that could've easily been made by a horror movie mad scientist, in a real life scenario. they probably fuse the three bones into one junction
the small bone of the lower leg (the fibula) is pretty much useless so they attach the other bone (the tibia) onto the femur, just like that
The tibia (larger of the two) is cut and connected to the cut femur. The fibula is left whole and the extending portion is inserted into the thigh (unlike what is shown in this video).

As a childhood cancer survivor I forget that people don’t know about it. I saw two kids on my ward that had this done. It’s really so innovative because you keep a real knee joint and the prosthetics have to be only below knee so it leaves way more mobility. I thought it looked a little funny the first time I saw it but when I saw the kid walk super well on prosthetics real soon it made me so happy. And after seeing it twice it’s really not that weird anymore ^^

I thought this was pretty ridiculous at first, but yeah, I can see how it would be incredibly useful to have your foot, even if backwards, for the purposes of operating prosthesis. Whoever came up with this surgery was thinking outside the box.
I imagine it'd be hella awkward to cope with the fact that your ankle has been turned into a knee and is now facing backwards, tho.
I'm sure I'd be able to learn to work with it eventually, but it'd be super weird. Glad I still got both legs

My first thought was "this seems ridiculous, why not just amputate up to the mid-thigh?" But now I can see that with a rotated foot that acts as a knee joint, someone with a rotationplasty will have a better range of motion. That's actually pretty dang cool!
They also probably don't get the "ghost limb" that amputees get, where they feel pain for the body part that doesn't exist anymore.

@@effervescentrelief wouldn't they still feel pains and sensations for the part of the leg they removed? Like they preserved the main nerve but they still removed tons of branching nerves so i figure they'd still have it.
also it's easier to walk on since when they wrap muscle around the bone with the regular amputation procedure it's not ideal to put weight on basically raw bone with some muscle padding.
@@Myemnhk phantom pain is the brain trying to elaborate feedback that just is not there (the video where someone have his hand hiddend, than a prop came in, and after some specular interaction the brain believes that it's recieving impulse from the prop hand instead of the real one, then the prop get stabbed with a fork and for a few instant the person feels actual pain).
this procedure leaves a full set of main nerves there, so a lot of information is still the original feedback that prevents the "search" in the first place; it's unclear if it's completelly possible to remove phantom pain, but there's no phantom limb effect, as most of the limb, from a nervous prospective is till there and sending correct feedback to analise
Oh happy day! One of my students had this done, and she just got word today that her bones finally fused after months of setbacks! She’ll get to walk again! 💃🏼

I was one of the first patients ever to have any kind of nerve-sparing joint extraction. My sense of relief having it all over with was incredible! I felt like giving my surgeon a high-five but was still learning to use the foot he'd attached to my arm.
