It depends on the design. Usually it looks good to have one type of alignment. But on a magazine cover there is usually the main title center aligned and then the headlines left aligned and right aligned.

It’s rare that designers are well established locally, though some do choose their target audience to be their local town or city, I have no clients from my city, all are across the country and in other countries too. For the second question, it all depends on how you’ve set up your brand message and marketing strategy. You could get international clients and I’ve seen designers do this when their messaging is crystal clear and they know who and where the right clients for them are and how to reach them.
