He absolutely noticed her. A disruption such as a panic attack should be easily noticable to a sith of his callibre, and sana even said her name out loud when blocking his view. It sounded like he didn't know Rex survived just by explaining how he thinks the hooded clone was Rex's and how he survived wasn't trying to be rude or anything
Correction: Rex was not the old soldier on Endor. Rex WAS on Endor, but he is not the old man. The old man is Nic Sant and Filoni has said that he doesn’t want to make the retcon and erase the original character just to fit Rex on screen.
Darth Vader is on the flagship/headquarters of one of the galaxy's biggest criminal gangs with a bunch of criminals from other criminal gangs and mercenaries and bounty hunters working for other powerful individuals, any of whom might want to make a name for themselves by killing the emperor's right-hand man and second in command. No doubt Vader it was necessary for Vader to stretch his force senses to everybody in the area to protect himself from a sneak attack to the point he would not be able to discern an individual he would be looking for. A single distressed or panicked individual wanting to leave would not be worth the time it would take to concentrate on that individual to see if he recognized her.

This really shows how terrifying Darth Vader is, especially those who’ve survived a hostile encounter or several with him, pretty excellent.

Dr Aphra: *PTSD*
