    Doc, im from the Philippines, why in that position of my arm with my hand at the back of my head during my sleep my shoulder joint was injured? It has been so painful for both internal and external rotation. In my case, what muscle, or tendon injured? What kind of therapy should i do?

  • @Gatordptatc
    @Gatordptatc 7 років тому +2

    A lot of patients with pec minor length deficits have glenohumeral elevation restrictions due to the anteriorly tipped scapula position. Do you have suggestions on UE position if the patient cannot get the arm overhead without GH impingement? I would prefer to get glenohumeral mobility via scapular mobility rather than vice versa.

    • @BrookbushInstitute
      @BrookbushInstitute  6 років тому

      Oh sure.... Jim, with all techniques you have to work with what you got. I don't like working "through" pain. If someone can't put there arm up, just leave it by or on their side. Sometimes I just have my patient reach up and put there hand on the side of my back while I control their arm from the humerus.
      One you know where to palpate, how to find a tender point, what not to pinch, and how you can pin the tissue... everything else is the "art" of physical therapy.
      Dr. B2

    Hello Brent,
    A great video. In 4:50 you said that this tissue that you had your hands on is the anterior deltoid. It seems to me that maybe and mostly is the pec major muscle. But maybe I'm wrong.
    Thank you

    • @CXS1181
      @CXS1181 8 років тому

      I agree with you. The anterior deltoid would be touching the surface of the table since her hand is up behind her head. Definitely Pec major

    • @CJFranciss
      @CJFranciss 6 років тому +1

      I think he was referring to the superficial fibers of the anterior delt that the pec major runs underneath. Remembering the pec major is deep to the anterior delt in that area. Might be the camera angle that causes a little uncertainty either way in that area the tissues are quite closely related thats for sure.

  • @sharkrancher
    @sharkrancher 7 років тому +2

    Hey Brent
    You might try accessing the pec minor in the shortened position.and yes in sidelying as previously suggested... It takes tension off the muscle and decreases the discomfort for the client. In my experience it seems to help. Lifting humerus vertically creating a slight traction, putting finger pads in place as you bring the arm across the chest slightly. Give it a try. Keep up the great work!

    • @BrookbushInstitute
      @BrookbushInstitute  7 років тому +1

      I will try that, thanks.
      I think with all these manual technique videos we should probably create a video that says something to the effect of... if you can accurately palpate and administer the appropriate force there are likely multiple "correct" ways to perform the technique.
      I try to find the simplest/easiest way, but there are other great techniques and therapists out there!

      Good question,
      I am not sure I can give you a single word or description, but pain and uncomfortableness would certainly be expected. You may feel a nodule or "string" that snaps in and out from under your finger.
      Dr. B2

    • @georgekoutsis
      @georgekoutsis 7 років тому +1

      You mentioned a "string" that snaps in and out. This can be also the vertical fibers of the pectoralis minor right ?
      Thank you

    As a personal trainer, I have used smr on pec minor by applying compression on the anterior side, pushing through pec major. Would I be better off using something like a theracane to mimic the technique used in this video? How much difference would you say there is going through pec maj as opposed to under it? Thanks for the excellent content!

    • @BrookbushInstitute
      @BrookbushInstitute  7 років тому

      Hey Superfit,
      You could use a theracane, or even a softball in a side-lying position with arm over-head. I would say that going "under" the pec is more intense.

    Have you heard of the pec minor getting so tight it pulls the ribs up (thus slightly twisting them) and causes knots to develop in the rhomboids?

    • @BrookbushInstitute
      @BrookbushInstitute  7 років тому +5

      No, although that sounds more like hypothesis than an actual diagnosis.
      As the scapula is far easier to move than the ribs, I would imagine that the pec minor being tight would result in scapular anterior tipping and that would cause rhomboid trigger points. Rhomboid trigger points may also be related to thoracic spine stiffness, which may have an impact on motion of the ribs.
      In short, pec minor hyper-activity, thoracic spine stiffness, rib dyskinesis, scapular dyskinesis and rhomboid trigger points may all be related, and often are.. I just doubt the ribs are the thing connecting them.
      Hope that makes sense.
      Dr. B2

  • @rachkate76

    Jist curious as a pleb (no intention of doing this or performing it on anybody) with muscle imbalances etc, what are the symptoms of having a tight pec minor?

    • @BrookbushInstitute
      @BrookbushInstitute  8 років тому +6

      Hey Rach,
      Good question, although perhaps not the right question. We would presume that the pectoralis minor is tight if an individual showed signs of rounded shoulder posture or an inability to posteriorly tip or sufficiently upwardly rotate their scapula (which may limit shoulder flexion, external rotation and abduction). These impairments may lead to symptoms ranging from shoulder impingement syndrome (SIS), to thoracic outlet syndrome, to entrapment of the ulnar nerve, to just feeling a little tight on the outside of the chest.
      Hope that answers your question,
      Dr. B2

      I believe it can. I’m experiencing that now. Sort of a brachial plexus compression based on a troublesome pec minor (I tore it 6 years ago and never addressed it). Now that I’ve been hitting the gym regularly, consistently and hard (I didn’t the past 6+ years) I feel as though it’s come back to haunt me. I’ve been experiencing a dull ache, heaviness, mild pain to the entire LUE, mild tightness L c/spine, and tingling, numbness L arm into all fingers… this started 4 days ago. I’m a therapist but hasn’t practiced since 2015.. this has caused me to do extensive online research the past 3 days. I did an upper limb tension test which was (+) for brachial plexus compression. I have mild tenderness @ PM region, etc… I’ve ruled out thru various tests the referred pain coming from my c/spine (which is another story considering I do have compressed discs throughout)… I’ve been doing doorway stretches, Y-shoulder retraction and hold exercises and tennis ball upon a wall for pressure release of the pec-minor along with cold/heat modalities.. as of now it’s slightly better with less pins/tingling symptoms… let’s not mention the possible shortening and/or scar tissue that has adhered within the area the past 6 years of not addressing the problem from the get go! ..hopefully I can rectify this problem myself..

      I'm experiencing all the same symptoms for months now. I'm trying to do it all on my own, I have good days and bad days, just can't shake it. I get pain into the middle of my arm as well. I take no meds so it has been challenging, massage seems to help. I sometimes wonder if just totally resting it will work. I've try just about every stretch and exercise on the internet. I also have compressed disc's as well from all the brutal lifting through the years, fingers are numb every morning.

      I incurred a bicep injury along with this pec minor injury in Jan 2017 on a trip away with friends… my inner bicep bruised along with pain within the axilla area. I ignored it for I was on vacation and upon returning home didn’t wanna deal with doctors, sick days and missing work…. So I went about life. I didn’t even rehab myself! I was a fool. The bicep healed with no problem but I always had mild discomfort in the pec minor region deep within the axilla. Never enough to the point where it really bothered me and prevented me from doing things. I could no longer do normal push-ups though. I was working lots of hours (not in rehabilitation but in my main job). I figured when I retired in 2022 I’d be able to exercise and train in the gym adequately to over come this left upper extremity shortcoming.. I noticed since the injury that anatomically the left shoulder/pec insertion areas weren’t symmetrical with the right side. When I slowly abducted both arms I can notice the difference even more so. Upon retirement in June I did basic AROM exercises. Then come October I yearned to join a gym and get back at it!.. as I started implementing resistance training, weights and machines I would no doubt feel muscle soreness, etc from the routines (all good normal stuff). I did continue to notice the orthopedic problem within the axilla and pec minor but strived forward. But as of last week it turned into a whole new level. The whole left arm now felt heavy with a dull ache. Not a muscular discomfort after a good workout. This was different. Also, I’d feel a heat rush within my arm intermittently throughout the days along with too often pins/needles and altered sensations within all five fingers.. I will avoid routine pressing exercises for awhile and just stick to a few exercises for relieving brachial plexus compression and stretching and releasing pec minor. Along with basic shoulder/cervical postural awareness and slow, controlled, hold, scapular retractions….😃

    My massage therapist does some stupid ass pec release. Its absolute bullshit and i think that she knows it. She does this thing then get down low by my head and looks at my scapula's. She says that they are much lower and giggles. As if she took tension out of my pecs that allow my shoulders to fall back and relax. IT reminds me of some sheisty chiropractor.

      Hey Rach,
      I agree, we really do the best we can. Because we want to be able to keep producing videos at an aggressive pace we do not use a videographer or even special equipment. We set up 2 still shots the best we can using amateur photography equipment.
      We have certainly become a lot better at filming than we used to be, but it will likely be quite a while before we add a 3rd point of view or a videographer who could change angles mid-shot.

The fact that I have been able to record HD with a wireless mic, was a huge step up from our original videos which were done on a flip cam.
