That little tab with the hole in it on the pulley sheave is a safety cable guide, to route the safety cable through to keep it from getting tangled up.
The loop should not go over the hook bolt. It can jump the hook along with the spring if that's the side that breaks. The proper way is to wrap the safety cable around the upright and pass the other end of the cable through the loop. Then pass the end of the cable through the spring and tie it off at the other end. The other thing to note is never install safety cables with the garage door closed. It should always be open and proped up using Vicegrips or a ladder under the door. This relieves the tension on the spring and makes it a lot shorter and easier to pass the cable through.
