i think the leatt neck brace I was wearing really did cause my collarbone break just looking at its location - but that being said the dent in my helmet and brace told me it couldve been much worse than a collar bone - so i still definitely wear my neck brace lol, id rather break a collarbone than a neck

@@ryanwest9267 sorry about your collarbone. You would know but it’s very low probability to be a cause. If you put your arm out or had a sideway impact to the shoulder that’s far more likely to be a cause. If the helmet or object struck your collarbone the brace it would have protected it- providing it was setup correctly.

Thank you for the video! Happy that your are going to be okay and still able to ride. Almost 10 years ago, I also got very lucky. Came off a drop that I wasn't prepared for and landed poorly. Ended up in a sitting position. My friends that were with me asked if I was okay. I said I think I broke my neck. Did not move it, but just did not feel right. Did every thing wrong after that though. Got up, tried to walk. Still about 3 miles out.
Luckily, 3 people who had just finished a wilderness training course were eating lunch just below and realized what might be happening. One person pulled a neck brace out of his pack and put it on me. They talked to my friends and convinced them that I needed immediate help and I ended up being helicoptered out.
Long story short, I actually did break my neck. I think I was ridiculously lucky. It ended up that I had broken my C2. It was a clean break that ended up needing no surgery. Wore a neck brace for about 5 months and was off work for about a year. I was 55 at the time. Now retired and nearly 64. Took some riding classes after that, ride more sensibly now, don't do much air, but ride much more technical features and I am much more aware of when I should walk a feature, or to give it a go.
Not sure how long I can continue to keep riding like I am, but life is short and you need to enjoy every minute you have available.
Thanks again and hope all is well in the future!

Thanks and, first, soooo glad you’re OK. I have had my neck fused at C5 and ride hard and am also a medical doc able to review clinical papers such as the one you quote. I’m also qualified to review critical appraisals of such papers. Due to self interest, I undertook exactly the same assessment as you have and was lucky enough to use my skills as a medical journal reviewer of a well known medical journal. And here are my conclusions to consider alongside yours.
Stay still until somebody can stabilise your neck. Go and get a possible neck injury reviewed pronto. You said both these.
Firstly, there is no definitive evidence that a neck brace is dangerous. It may be and perhaps it isn’t. We just don’t know. Secondly, there has never been a controlled study of sufficient ‘power’ to settle this debate, which is a shame of course, but in fact it would be almost impossible. The study you quote is an observational study with little control and the observers (the EMS techs) did not have a suitable protocol to follow to be able to draw the conclusions many have done from this study. Thirdly, at least we have this study which is food for thought and should not be dismissed. It covers a lot of what needs to be considered, but neglects the impact of rider skill, the exact nature of the mode of injury, rider fitness and fatigue and lastly it takes almost no account of the other safety equipment and care taken by each rider.
And it was nowhere near large enough as an observational study to say, ‘No, that’s not correct, just look how big the differences are between the two groups (neck brace/no neck brace)”. It’s so tempting to draw that conclusion: visually the figures are compelling! But, sadly, it simply isn’t a big enough study to say that, given its methodology.
Fourthly, it does seem wise to wear a neck brace doesn’t it? But remember, two things:-
    1. Practice riding within your ability wearing it until you are completely certain what your new limits are wearing it, especially when tired or ‘on the edge’. Without a doubt it diminishes our ability to ride ‘At the limit’. And, when I was younger I’ll admit that when I put on a full face for the first time, I took more risks…. This doesn’t matter to us sensible people who would never do that, obvz,- but hey, look at the decision you took and you seem very sensible… And this is the reason DH UCI racers give for not wearing them: they occasionally have to ride at the limit and there are other injuries that can be fatal or devastatingly life changing lower in the spine (yes I know what the paper says about that, but I’ve explained that conclusion simply can’t be safely formed from the data presented in the study), the liver, spleen, the pancreas, and the head.
    2. Get one that fits. This is not easy. Not at all easy. It simply must fit correctly to bring benefit rather than bring risk and benefit. Are you going to attach it to your chest/back protector? Do you know if it’s safer or less safe to do that? How would you do that? Buying a ‘set’ is an expense some might fight shy of (don’t!). The thing is, its not like its possible to go into a store and try on all the options from the quite large range of manufacturers and having an expert fit it to you, to your helmet and to your chest/back protector (by fit I mean integrating together safely rather than creating a leaver that actually increases the chance of cord damage). Perhaps there are one or two retailers in the US that offer this, but to the best of my knowledge many are bought on-line or the only thing in the LBS is bought through frustration and then immediately worn and so can’t be returned. My opinion is that many of those who have set aside a purchased brace have done so for this reason - so that’s complicated.
So, thanks for this. I, like you, can powerfully relate to the apparent benefit from the EMS study and would encourage anybody who wishes to get one. But please do so recognising that the data isn’t there and may never be (Becuase the necessary study will not only never be funded, but is effectively impossible) and, above all, get the correct fit and get used to it at the extreme of your competency if that’s the direction you go.
Obviously, it’s completely irrelevant what I chose to do. This is a personal decision because it’s so subjective. So, I don’t want to influence anybody by my choice, just my understanding of the science and the sport as a shredding medic.

Thanks for the comment! Sorry about your C5 injury, sounded really bad. If you're interested could you find me on Instagram so I can send you a few bits like the Atlas's study - I'm trying to get my head around this. Very different not done by a computer models like Leatt. The high profile lawsuits on Leatt for broken lower backs ended up favoring Leatt, doesn't say too much but yes there are no proper studies minus EMS. As you know unrestricted neck braces aren't like helmets they aren't required to go through testing. If you're open to the idea it would be nice to have a discussion and perhaps we could prerecord it.

my mountain biking physician friend explained to me 1 thing. it does not matter how good your technique, skill is or how much you analyze your mistake because the next crash site is always different from this one. for sake of argument=> lesser experienced mtb'er, means big injury 1 out of 100 jumps/hops/drops, better skills mean big crash 1 out of few hundred repetition. it is unavoidable statistics for enduro, or all mountain mtb. biking.( not for XC). To put this in better perspective, 96% of motorcyclists my doctor friend encountered had severe crash injuries. it is inherited risk with 30-80 mph on a bike among moving cars but on flat asphalt. but enduro or all mtn biking is just as bad because mtb is zooming around 20-40 mph around drops, obstacles with unpredictable uneven soil/terrain conditions. wish you the best recovery.

    • @guymtbski
      @guymtbski  2 місяці тому +1

Thanks for commenting! The main mistake I made was two fold; not looking and attempting something big early season. I didn't really want to analyze the feature just drill in the two mistakes. 1/100 jump is probably close to the 66.6hrs exposure to 1 injury I quoted at the start, which is very relatable. I really like what you said about motorbikes being on asphalt and mtb having many obstacles. So despite the lower speeds the obstacles make things equally as bad. I'm going to use this when preaching safety equipment to the next pair of gym pants.

I can say i have been very lucky after having some big crashes (i can humbly say that i've always ridden hard and fast taking big risks, specially with bmx freestyle) during the years and since a couple of months i started riding with a neck brace. I just woke up one day and thought about what could happen if luck runs out one day. Being paralyzed is not something i want for anybody in my loving circle to have to go through because of my own doing so i am way chiller now. For those wondering, go check some neck braces with your helmet so you can find something that fits well and straps that go under the arm pits go a long way on keeping that thing still (loose neck brace is the worst thing). I'll never understand why neck braces are out of fashion at the moment in mountain biking. Yes, very occasionally I knock the chin bar into it while jumping and sighting the landing, but it's not like I needed to rotate my head that far forward to begin with. (I've heard the lack of head mobility as a reason against them so figured I'd dispel it.) Might as well have the piece of mind of reduced risk of severe injury even if unlikely. The Leatt system is pretty nice since it's one of few that pairs nicely with a chest/ back protector which is also definitely a worthwhile piece of protection kit.

I’m stoked how well this video is doing to help spread awareness considering my small sub size. Many people are thinking about this now and I hope people can question opposing views.

The collarbone statistics are skewed right? Less collarbone injuries have happened with people who wear neckbrace but the amount of people aho wear neckbraces is alot lower than people who do. So the collarbone injury to neckbrace wearer ratio is higher for people wearing neckbraces than not.
Tldr: neck braces do not protect your collarbone.
Or am i thinking of it in the wrong way?
The same would apply for the other statistics aswell

@@nightfade2052 fair point. I don’t know how they got to 45% - I’m going to email and ask. I ran the numbers myself just now and get 22% which considers proportions. The EMS study seems quite independent so I assume the best intentions.
The trouble is you can’t have exactly the same falls with the same people with and without so it is going to be speculative. I am bought into the benefits as the neck braces can protect from direct impact from helmets or a branch to the collarbone - it sits above it. You can’t do anything with hand or sideways impacts.

I wonder if there is a difference in HOW you wear a neckbrace. Some people say you should wear it with a chest and backprotector, some say you don't need to. This has kept me from wearing one all along because I feel like a chest and back protector + neckbrace is ristricting my movement to the point I feel like I'm more likely to crash. A neckbrace alone with the fullface helmet etc. would be fine for me, yet I am not sure if its good that part of the energy might be absorbed by your upper back (which could be absorbed by the back protector).
Very glad to see you walking and talking about getting back to riding! Cheers from germany

@@DerTim930 you 100% do not need to also wear a chest or back brace that is nonsense. Get the 5.5+ and adjust. There is an ‘optimal effective’ level of height you must keep within these limits. In short if you have no movement you’ll more likely to get a head injury if you have too much movement a neck injury. You need to be in the optimal range. This is hard for me to explain but it’s between (50mm-130mm between bottom chin on helmet and brace 50mm - 150mm bottom helmet and back brace). If that makes sense you could see what that would feel like - maybe check manual for the 5.5 page 5. You do 100% need to wear a full face.
You look like a sender so please consider this. It is awful if something happens and you are in the hospital thinking what if. Please note the worse case of a spinal injury is very very unlikely, I’m not here to spread fear but I wish I wore one so could ride right now.
