If any steering column linkage has ever been disconnected, it'll be worth it to assure it was assembled exactly centered. Don't ask me how i know but yeah, if your steering wheel won't center after a meticulous alignment, check the steering linkage alignment too
Depends on the car. Best thing to do IMO is try and follow the threads with a pick or just visually to figure out if it's left or right hand threads. Or just try it in both directions lol
Measuring from a string inward, yes. If you were measuring the total distance from side to side the measurement would be a lot more than 40mm and it would be toe out=shorter rear measurement.

My front tires are wearing on the inside...When I loosen the lock nut am I turning it toward the front of the car or the back of the car ?

When you say the measurement at the front edge of the rim is 39mm and the rear edge measurement is 40mm your calling that toe out??? Seems backwards??

This guy doesn’t know what he is talking about. Longer measurement at the rear of the tire means toe in not toe out .
