  • Medical oncologist Mark Scholz, MD, answers questions from our UA-cam comments on whether Firmagon is a safer and more tolerable alternative to Lupron.
0:03 Can you share your thoughts on whether Firmagon is safer or more tolerable than Lupron?
1:54 You said that you tend to start people on Firmagon and then switch to a more convenient option like Lupron, Trelstar, or Eligard. Is Orgovyx also a good option to switch to?

No effects at all apart from some hot flashes but it keeps my PSA below 0.01 ng/ml and if it stays this low for 2 more years I will be totally recovered God’s willing 🙏

I am very sure that it had a lot of hidden bad effects on my body my heart I gained 30 pounds I am always tired and fatigued. My Doctor tells me that it’s saving my life and I have to stick to it. To Zytiga and Firmagon. I hope that I will snap out of it one day. Thank You for your concern.

Can one choose what hormone therapy to be on, or does the doctor choose for you depending ? From what I'm reading on Firmagon there are less side effects as well as less long term effects. I have to take HT for 18 months. PSA 7 Gleason 8, doc says I'm high risk. Going for proton therapy. Thank you

      Hello,
That probably depends on the doctor. If you would like more information, you are welcome to contact our free helpline at pcri.org/helpline.

My advice to you is to take Zytiga 25mg 4 pills a day and a Firmagon shot every 28 days that’s the best most desirable hormone therapy and your TPSA will drop dramatically and you shall recover totally in 3 years

Doctor detected advanced prostate cancer in my father after PETSCAN and Biopsy. The PSA was around 160+ and I was scared like anything. I lost my mother in 2020 and I was totally blank & blind after listening doctor words. Doctor started degarelix-120mg ( monthly), XBIRA - 250 mg ( 4 tablet in the morning and precisely 2 hrs before breakfast) , wysolone - 5mg ( twice: 1 - 0 -1 after food), ccm - 250mg ( twice : 1 -0 -1 after food) . With this method, the PSA score come down to 8 then increased till 14 but doctor started "Abiraterone" and this medicine increased PSA from 14 to 50 ( today its 50.14). I talked to doctor and he told my father to get PET scan done asap. He told me about chemo and I am not too comfortable to listen this. I don't know what is the best approach to do now.
I have seen "Abiraterone" causes PSA flare and I am wondering that why my doctor has suggested it.
Can I get any help here?

My husband was diagnosed with stage 4 oligometastatic PC a year ago, PSA was initially 24. He started with Firmagon and his PSA dropped to 2.0 within a month. He's been on Lupron injections and Abiraterone for the past 10 months and also had 26 rounds of radiation last summer, no chemo. Since then, his PSA has been negligible

For patients with stage IVb PCa, what determines if the patient goes on chemotherapy or ADT? Is there any research showing the efficacy of one over the other?

It differs from doctor to doctor, but the most recent research indicates that using them together produces the best results. Here is our 2021 ASCO update in which Dr. Scholz discusses a study about men with newly diagnosed metastatic disease:

I started with firmagon which knocked my psa from near 400 down to near zero and basically eliminated the metatastic growth. The price was incredible pain and soaking night sweats. Would I do it again. Hell yes.

There are a few different types of medication that essentially do the same thing as Firmagon and are less often associated with pain from the injection itself than with Firmagon. A lot of doctors like to at least start with Firmagon because it does not have a testosterone flare like with Lupron; however, a person could ask their doctor about changing to Lupron or some similar medication after the first month or two of the Firmagon injections if the injection itself is it is causing pain or if it is too expensive compared to the other options. There is also now the option of a pill, Orgovyx (relugolix), which has some unique considerations that we have videos on.
Hopefully you will not have to do deal with prostate cancer again, but if you ever have any questions, feel free to contact our helpline at pcri.org/helpline.

Do I understand this correct, Lupron only spike the first time administered, and only if the levels are not managed? So Firmagon first shot and 1 month later Lupron and then just every 3 month Lupron?
