  • Alex asks Dr. Scholz about a patient from our UA-cam comments who has a PSA of 0.28 while on hormone therapy after radiation, and answers his question whether or not he should get a PSMA PET scan.
0:09 Is a PSA of 0.023 considered undetectable? Is it safe for me to consider testosterone replacement therapy?
3:43 How long does it take for testosterone to come back after stopping hormone therapy, and does a second-generation hormone therapy like Zytiga affect that?
4:38 I am 72 years old with a Gleason 8 prostate cancer. I had IMRT and have been on hormone therapy. My PSA is currently 0.28; should I get a PSMA PET scan?

ALEX, this series is exquisitely produced and helpful beyond just information. It helps us in that because the system is so loaded that the oncologists and clinics drop us back to the care of our family physicians whom we seldom see and thus feel in a vacuum after all the visits for care. But You and the excellent Dr. Scholz keep us feeling "IN" the loop.

I was diagnosed with prostate cancer in 2016, 4+3 gleason score. I had a radical prosectomy followed by salvage radiation when psa was at .08. I was put on trt about 20 years ago. After having undetectable psa for 2 years I went back on trt. After a few months my psa is back up to .06. I stopped the trt. I was wondering if you had any advice regarding trt use in the future? I'm also curious if a PSMAT scan is too early at this point? Thank you very much!

I still worry ... My PSA is still 1.2 after diagnosed with PS 4+3 T3b N1 M0 7 months ago (ags 68); started Eligard 45mg right away, then radiation x 39. PSA fell from 27 to 12, 6.8 than recently 1.2 - Still not 'undetectable' after at least 6 months. All doctors I met (3) told me 1.2 it normal and ok. Just received 2'nd Eligard. How come my doctors says ok, even confronted with your PCRI facts and every PCRI video says like 0,1-0,2 and this video to to worry if it doesn't go down.. what to believe??

I'm an engineer, not a doctor, have familiarity with science in general, but not medicine in particular.
Most abnormal cell-growth, it seems, is either caused by improper synthesis of folic acid or by an exaggerated immune response caused by bacteria and other germs breaking into the body from the colon after they breed behind hardened feces.
Furthermore, the only amino acids with a methyl group that can alter the DNA are Methionine and Cysteine, which also lead to the issues that manifest themselves in the conversion of methionine into SAM, cysteine into homocysteine, and also issues with folic acid absorption.
What causes the hydrolysis of testosterone into hydro-testosterone?
I tried parsley to prevent the conversion of testosterone into H-testosterone, and was somewhat pleased with the results.
I also experimented with fat-reducing MIC Vitamin B-complex injections (Methionine Inositol, Cysteine and Vitamin B-complex injections) because people use them to dissolve fat in the body, so I wonder if they'd also help rid abnormal cells.
    Folic acid is a B-vitamin.
Finally, I tried the 5MTHF-type of folic acid, that is already processed, so doesn't need further processing by the body.
I even tried fasting and the cleaning of my colon through a blend of fasting and a diet of lemon juice.
The results have been mixed, my prostate issue is far from resolved, but the pain is bearable and I have managed to keep the PSA levels low, I would still, though, want to to opt for radical prostatectomy or a focal therapy just to put my mind to ease.
I suppose hormone therapy does pretty much the same as my diet, perhaps, better, but with more side effects?

I have recently completed radiation therapy for prostate cancer. I discovered that I had a mass in my underwear that looked like melted hydrogel. Is this possible that the ApaceOar gel melted during. Radiation therapy?

    • @johngorn268
      @johngorn268 Рік тому

I think that's unlikely. I would guess you may have expelled a mucous plug from your rectum, a result of proctitis caused by the radiation.

I’m 74 and had robotic surgery to remove my prostate in October 2020. I have 7 PSA test since then. The first one was 6 weeks post surgery and that was a .001 and I just had a PSA test a week ago. And that was a .26. It has been a gradual rise from .001 to .26. Now I’m scheduled for an Axumin Pet Scan. And follow up radiation. Is the PSMA Pet Scan a better choice for me and is 30 days of radiation my best choice. I’m located in Southwest Florida. Gleason was was 7, 3+ 4.
I was able to find a location that offers the PSMA Pet Scan. That is scheduled for February 15, 2022. Will be followed up with radiation at the least and hopefully not androgen therapy

I'm not with PCRI, but in Dr. Scholz' comments in the past and those of every urologist I know of who cares to comment, the consensus is that PSMA pet scan is now the best, state of the art scan for finding PCa metastasis.
