I think it was called Kickass simply because the majority of people can relate to the main hero while Hit girl is just a physical out of the ordinary beast who humiliates bad guys. Pretty sure the main message of the movie was that everyone can be a "superhero" in one way or another.

If you look there's only about 7- 8 other cars on the road and they're keeping the camera tight to make it look like the battle is happening on a busy highway - the road is empty in the far background. The cluster of cars and the van are driving along quite slowly (see how the cars slew away from the van from time to time with hardly any body roll) and the "speed" is all made with fast cuts and wobbly-cam. This is a 2013 picture - these days they'd shoot the van on an empty road and put CGI traffic in later.
