I was in ER and they asked me the pain question. I think I said ‘6’. My wife immediately said “don’t listen to him, he doesn’t think like other people”. So the doctor asked me “what would a ten be?” I answered “if I was on fire and you tried to put it out with acid, that would probably be a ten”.
    They gave me pain meds.

I actually find the 'immediate family only ' thing a little weird and sad, even though I know they have to limit people one way or another. I remember being in a hospital and seeing a sign that said when visitors are restricted to family, they define that as 'whoever the patient would consider family', which I liked. Some people have chosen families and have friends who are much closer to them than their blood relatives.

The "family only" thing varies from hospital to hospital. And f on the ER and Operating room to the ICU to the general and intermediate care floors. At my hospital, we let any visitor stay as long as the parents is ok with them staying (unless the patient has requester a restricted visitor list or they are under a trauma name)

Hospital in my city allowed me to enter the ER for my friend's little sister, but only after confirming with my friend that he wanted me there. I feel like that's the right way to go. You can't just let everyone in, but at least let the people in that are needed at that moment.

if I was in the ER and found out they turned away my friend of 25 years because they aren't my "immediate family" I would be kind of pissed tbh. I get that people have to make a call somehow, but we're as close as immediate family and I would want them to be able to visit.

Never really got to experience the full 1-10 pain scale shenanigans as a veterinary professional (since, you know, animals don't talk), but I feel like it would be just as crazy if they could answer that question. Like a corgi or a poodle mix "OMG It's an 11 please doctor make the pain go awaayyyy" when the presenting complaint is annual wellness and all that happened was the doctor used a stethoscope. Or a happy golden retriever saying their abd pain is 6/10 despite being seemingly unaffected, but then it you take an x-ray and their spleen is actually about to explode and they were just so happy to see you that they forgot about how bad the pain actually is.
And then there's always that barn cat that fell from the hay loft who'll say "Pain? Why would I be in pain? Well, this bug bite on my ear might be a .5 out of 10..." meanwhile their tail is paralyzed, their hind leg is twisted 180 degrees, and that "bug bite" is actually a gigantic hematoma

I have several horror stories from our ER but another instance of why you shouldn't just blow people off. I have a friend that lost her arm over being blown off by the ER. She was working out in her yard and felt a terrible sting and had been bitten by a spider. The pain had gotten so bad she knew something was wrong . When she got there they made her wait for 4 hours then blew her off as trying to get pain meds and sent her away. She had to drive another hour and half to a different hospital to get proper treatment and they had to cut off most her arm. I get the Er deals with a lot but just going on an assumption like this can lead to serious damage and even death. My 2 year nephew almost died because he was blown off at the same Er. Thankfully my sister took him to another hospital and they got him in for immediate surgery.

On the flip side, there are doctors that don't realize they can talk directly to my non verbal daughter. Non verbal doesn't mean non understanding. She's quadriplegic (CP, not paralysis) and one time a doctor came in and was asking me if she could do this or that. Can she move her feet, can she lift her arms, can she blink on command, that sort of thing. I said, "why don't you ask HER?" So he starts asking her, and she shows him that she can do these things. He was absolutely fascinated that this profoundly physically disabled child can understand him (most can, and I wish more doctors knew that instead of assuming they can't), so he came back later to question her again, I guess to make sure it wasn't a fluke. After like 3 questions, she turned her head away from him and refused to interact anymore. I said "that means she's done". Then he came back the next day and tried again, and this little girl, who has almost zero use of her body, managed to swing her arm around and HIT HIM. I couldn't stop laughing. She's not a monkey that performs tricks for you, sorry. 🤣 Assume competence. Physically disabled and non verbal doesn't mean they don't understand and/or can't communicate. My daughter communicates just fine. The most efficient way is to just ask yes/no questions, which is often very easy to do in a medical setting, and I just fill in the gaps with details.

😅 the "family only" segment made me laugh so hard. Storytime: I had suffered a traumatic brain injury three years prior. I was working at a summer sailing camp 16+ hours from home, and we were about 9 weeks in. So I got a concussion when a kid accidentally kicked me trying to parkour over a catamaran (yay). I was losing consciousness, vomiting, and slurring my speech. The camp director's son was a paramedic and he drove me to the ER. When we got there, they said family only, but I couldn't see and could barely talk. He goes, "she's my cousin" and whispered under his breath, "what's your birthday, cousin? How do you spell your last name, cousin?" I handed him my medical ID / info bracelet, I could hear the triage nurses laughing, but they let him in!

I find these videos so funny! I relate a little too well. I worked several decades in an entry crisis unit. I'm finding the "Don't say it" captions especially gratifying. Although I am a generally calm clinician there were times when that patience was tested. We were an adult facility. Sometimes one adult can be as challenging as a room full of runny-nosed crying, diaper-filled pre-schoolers. At one point they put four lines on my phone and had told me I would have a staff for screening and assessment. They then decided that as I was experienced and working on my doctorate I could singlehandedly handle the incoming calls and intakes WITHOUT the promised staff. One perceptive nurse said it was my own fault because I was always so kind and polite. It was hell! I couldn't ever leave to go to the restroom without hearing my name loudly over the speakers throughout the building saying that I had a call on line one, then line 1 + 2 and so on. It became my job to screen every incoming person, to conduct a full intake, field all calls, sometimes a suicide call, all because a local news reporter did a story about the "mean nurses" in our detox unit. So because I was polite I was punished by having to screen everybody! That was dangerously unsustanable. My final straw was a patient who was mad because he had to wait, entered my small office, and communicated his dissatisfaction by turning his back to me, then bent over to serve me one loud and pungent fart! THAT was IT!!! No more Mrs. Nice Guy! The Lion had ROARED! They got me that additional staff immediately!

The pain scale thing is frustrating to me because I'm a very stoic person, and I really don't even know HOW to 'look like I'm in pain' like normal people do. I also have a very high pain tolerance because I have chronic pain (like I had a major surgery, one which they prescribed me narcotics for, and not only did I not need them, I didn't need the tylenol either. And I wasn't trying to be tough about it or anything, I was just...fine) so if I'm saying it's bad IT REALLY *IS* BAD! And I legitimately have experienced 10s on the pain scale, multiple times, which for me is 'if you threw me down the stairs right now I literally could not hurt worse than I already am hurting right now, because I CANNOT process any more pain than this'. At a 10 I may have trouble walking and talking completely normally, and it's very hard for me to think, and mostly all I'm doing is sitting and focusing on breathing because I feel like I have to consciously breathe and also it helps to have something to focus on. But. Because I'm not crying, and because my face looks like it always does, and because I can have a conversation and answer questions (even if I'm having trouble answering sometimes because I can't THINK), and can get up and walk and do things...my pain is NOT taken seriously. I went to the ER during one episode and I could tell they just did not believe me. I tried to explain that I have chronic pain and a stoic face, and that I really really mean 10 like I would not even care if someone hurt me right now because I cannot hurt more, but I don't think they believed me at ALL. They rushed me out as soon as possible. With a diagnosis of...acid reflux. (I've had plenty of acid reflux throughout my life. Terrible acid reflux even. And heartburn. This was not even REMOTELY like that.) I've had a lot of bad experiences with doctors and hospitals, but sometimes they really do continue to surprise me with how shitty they can be.

Lol I had to go to the ER when I was 23 for severe stomach pain that turned into collapsing on the sidewalk, and I was also asked if I was pregnant. I told them no, but I was on my period and I expected that was the problem because the cramps were just really bad and I was so pissed that I had to call them over it.
They pressed me and were like, "Are you *sure?"* And I was like, "Yes". And they kept pushing and were like, "Because it could also be a miscarriage." And I was like, "I'm 100% sure. I'm a virgin, I've never even been kissed." Hell I'd never had someone even want to touch me. Never even *held hands* with someone because my hands were gross and sweaty, and middle schoolers say the meanest things, so I was too scared to touch anyone. So no. I wasn't pregnant or having a miscarriage.
Sometimes I wish we didn't have to explain our whole sexual history to get them to do some testing that isn't specifically pregnancy related. Like I could be literally dying but you are hung up on the pregnancy part above everything else. I get that people lie, especially kids, but I was 23, and had no reason to, and those people really kinda f*** things up for the rest of us.
Turns out I was severely dehydrated with heat exhaustion, and *could* have died. My electrolytes have never been the same since. 🙃
Also f*** periods because that's usually when I just can't stay hydrated no matter what I do, and they just feel like someone is scratching my insides out through my lower holes 😂😭
Good news is I did have my first kiss that year though lol

Everything was black and I couldn't hear anything, but I was dimly aware of screaming coming from somewhere. THAT is a 10. If I can't stop screaming, but can see/ hear, that's a 9. Unfortunately, this realistic pain scale is apparently far from the normal, so I was left sitting in the busy ER with a 5-6 level chest pain for 3 hours... and it turned out to be a heart attack. Immediately postop from a major orthopaedic hip surgery where my pelvis was cut in three places and I had four screws holding me together was the worst pain of my life. I think I was screaming and grabbing the rail on the bed and desperately trying to get the nurse to understand it was my low back that hurt (my muscles were spasming so bad I was arched off the bed). I passed out right after and I’m not sure if it was from the pain or if they pumped me full of pain meds. That was 10 out of 10.
A number of days later I got a new roommate who was constantly whining about everything, demanded meds and even threw a tantrum. When the nurse came by in the morning to do vitals and asked my roommate what her pain level was, and she responded 10 out of 10 (the calmest voice I had heard her use ever). Bare in mind i moved to a different country to be with her so i have absolutely no family there.
So she rushed to the hospital immediately only to be told that she couldn't go in because only immediate family was allowed. So she explained to them that i have no family in the country and that she was the only person i had there besides some friends from work.
    They didn't care.
She then tried convincing them by saying she was pregnant with my baby.
    They didn't care.
So being the smart ass woman that she is, she had a ring on her index finger, which she had swapped over to her left ring finger during the conversation and then told them that we were engaged while showing them the ring...we weren't
    That one finally worked though.
When i got out lf surgery, one of the very first things she told me was that story. And now that has also become the story of how we actually got engaged and i found out i was going to be a father 😂

I hate the pain scale question with a passion. I always avoid saying 10 because I know that's supposed to be the worst pain you can feel, but there have been a few times I've said 8-9 and looked relatively okay (not as nonchalant, but close I guess) and gotten dismissed as exaggerating. I really was a 8-9 but I'm so used to some pain somehow that I've learned to just deal with it, especially as someone with endometriosis who has to deal with abdominal pain all the time. I was on my phone in the waiting room to distract me from my pain, not because I was bored. The worst time this happened I had an ovarian torsion (which means my ovary wasn't getting blood/oxygen and was literally dying). It took the ER forever to even scan me because the triage put in her notes (I could see this in the visit summary) "claims level 8 pain, but seems relatively relaxed and no signs of injury". I literally passed out and woke up after surgery after 6 hours of waiting. My ovary died and had to be removed so I wouldn't follow it. I literally lost an organ because of this stupid pain scale question.

I’ve seen a few articles that say the pain scale isn’t useful for people with chronic pain. Maybe the question should be, what number would you be on a regular day when you go to work or do the shopping. That might be a 4 or 5, but you can still function. If a person comes in with chronic pain and says 8, it would be higher than 10 for a person without pain. Or, we just have to act in pain instead of calm 🤷🏼‍♀️ We spend so much time masking pain, it’s hard to take that mask off and allow the real feelings be expressed.

As someone who had endo (technically a bit is still there, but I had a full hysterectomy in 2021) I absolutely get this. The number of internal ultrasounds I've had, the amount of pain I was in, and all I'd be given was paracetamol.
One time I got kicked out of the medically acute unit, where I was waiting to be seen because I couldn't eat or drink, because I was using my phone, and got told "it can't be that bad." It was, the morphine I'd been given in the ER had worn off and I was using my phone to distract myself, messaging my wife to reassure them that I would be okay that they'd had to go to work and leave me with a friend, and desperately hoping no one would tell me I was just exaggerating or drug seeking.
Three months after that doctor discharged me, I was in hospital for a week before someone agreed to do an exploratory laparoscopy. Turns out my left ovary was adhered to my bowel with endometriosis, and my surgeon was like "how were you functioning with that much pain?"
I wasn't, sir, but no one seemed to care.

Just in case anyone is curious, the pain scale used in most hospitals is called a Mankoski Pain Scale (they combined it with the “Smiley Face chart”) and a 10 is pain so bad you’re nearly passing out. Nine is crying uncontrollably and moaning. I give this explanation because so many people make the ridiculous claim that their pain is a 10 or even crazier, an 11, 15, even 100. There’s no pain scale rating above a 10 because a 10 is passing out. It’s like when you lose a limb or have third degree burns over the majority of your body.
I HAVE had migraine episodes that ranked a 9 on the scale. My husband had to drive me to the ER, and I was literally in tears and unable to really describe anything aside from the fact that my head pain was extremely bad.

I've never found the pain scale to be super helpful because I can imagine a lot of really terrible things that would hurt way worse than anything I've ever experienced, like falling into a vat of acid, being eaten alive by rats, being slowly crushed by a road roller, having my limbs ripped off when being drawn and quartered, etc. I would never be able to say 10 no matter how excruciating the pain because I can only assume all those other things would be far worse.
Also PSA for people with chronic pain: if you have worse pain than usual, rate your pain about 2-3 points higher than you actually feel it to be because that'll be more equivalent to what other people would consider it. Dealing with constant chronic pain skews your perception of what's normal and you may not be taken seriously if you rate it at a 6 when others would probably say 9.

All these ER videos still remind me of the time I went to the dentist for some treatment when I was like 12. the dentist had to ask me some questions in private, and he asked me how bad my toothache hurts on a scale of 1-10. I asked, “wait are we going left or right?”
his response was “Uhh. choose whatever.”
being the degenerate I was, I replied with 1. he asked me, “so it doesn’t hurt?”
I can write a whole novel but long story short, I said “it does hurt.”
he said “okay, so are you able to go any higher than that?”
I said “you told me to choose which direction I can go in”
He looked so done after that debate was finished.

The pain scale patients are difficult for me to slam cuz in nursing school we’re supposed to take the patient at their word: their pain rating is what they say it is. And I get that. Some people have chronic pain that’s actually always severe but they’ve just gotten used to it, so they don’t look like they’re in a lot of pain even though they actually are. But I won’t lie: I am going to have some questions if I see someone presenting with stomach pain and they’re scarfing down Cheetos at the same time (though tbh if the Cheetos are actually helping then something else is definitely up).

Pleeeeeease don't forget that! Please don't treat people like me as drug seeking because we're not in hysterics. I had a severe hemorrhage while having a miscarriage and made jokes as I was regaining consciousness to help the ER staff be less stressed.
I got treated as a drug seeker with a kidney stone because according to the doctor it couldn't be one because "I wasn't curled up in pain and being hysterical." I told him I was curled up at home. He only did the CT scan because he thought I'd say no to the expense. Turns out I was right, kidney stone!
It sucks to present atypically and get dismissed and ignored. Mine was from abuse where showing pain made the abuse worse. Fast forwards years later, now I also have chronic pain on a daily basis (small fiber neuropathy, fibromyalgia etc) that is about equal to labor contractions (mom of 6) just spread out over my entire body and non stop. I rate it a 4 or 5 because it does get worse. Much worse. I'd rather give birth on a daily basis because contractions as least ebb and flow and doctors will offer treatment for the pain.

      @MaryCumbersnatch Рік тому +3

You just described my daily life with hEDS, cervical instability, cervicogenic migraines, and lumbar arachnoiditis. Childbirth is an 8; daily pain is always at least a 4. When horrible occipital nerve pain drove me to go the ER begging for an occipital nerve block, I told them it was a 7, 7.5. The local hospital didn't help at all, not even a Tylenol! I signed myself out AMA, went home, didn't sleep, waited until after my son's birthday dinner the next evening, and went to a large hospital in Nashville. They actually listened and did the nerve block, but nobody listened to me (or charted, in 3 ER trips) that my tongue/lower lip felt numb, like I had anesthesia, but I didn't. By the time I drove the 90 minutes home, the occipital neuralgia was back. The next morning half my face went paralyzed, so I drove back to Nashville. It was Ramsay Hunt Syndrome.
RHS is definitely up there on the pain scale. Not only that, but you can't get any rest because pain keeps walking you up. I don't think I slept more than a couple hours a night for 5 days until they got me on steroids and antiretrovirals.

For those of you like me who struggle to assign numbers to abstract concepts: i think of it in terms of how often your pain interferes with your day-to-day life, and assign that value a percentage instead. so if 0% of your tasks are impeded by your pain you rate it a 0, but if 60% of your life is messed up by your pain rate it a 6.
For me that means any pain that prevents me from sleeping gets an automatic minimum of a 4/10, because I count the secondary effect of being sleep deprived as part of the score.
"My pain is so bad, I can't work, take care of myself, or even get out of bed because of my pain" is a much more grounded description for 10/10 than "the worst pain you've ever felt"

They took me to another room and when the results were ready the doctor said "look, you are going to have to level with your husband that you cheated on him. I know it's painful, but he's going to find out eventually". The nurse handed him my results - I wasn't pregnant. Using the most common issues is just plain lazy and assuming your patients are lying is kind of shitty.

It’s the *emergency* room so if you’re in a doctor just brushes you off like that it’s specifically that doctor’s fault. Yet, I live in a different city (Gillette, Wyo) and I’ll say that at least here I’ve heard no bad stories from the ER about nurses or doctors.
Depends on your city and hospital for how they’ll treat you I guess

When I had glandular fever my throat was so swelled up that my tonsils were almost touching each other. The whole back of my throat was white. I couldn't eat or drink anything and if I did manage to swallow a few bites I would vomit it straight back up. My GP prescribed oxycodone hoping that it would ease the pain enough to eat and drink. It didn't do anything for the pain. So I went into e.d. they took my temperature and admitted me to hospital straight away. I was so dehydrated they had to give me i.v. fluids. I got put in isolation and had student doctors coming into my room in protective gear asking to see my throat. When I got admitted to the ward the nurses were negligent and extremely rude. The day before I was discharged they ignored my bell and "forgot" to give me my pain meds and I was in absolute agony.
The e.d staff were so much nicer and understanding even though they were as stretched as thinly as you can imagine. The ward I was admitted to was probably at about 50% capacity at the time and the room I was in I had to myself even though there were 3 other beds. It was then I realized that if nurses pick an issue with you they will treat you like shit for your entire stay. I had to go to the nurses station when they ignored my bell and they were all standing around laughing. I get that they are entitled to breaks of course! But all at the same time? That's ridiculous. I was so happy to be discharged. I've never had that experience any of the other times I've been admitted to hospital (there's been a few times over the years)

Anyway, I was on my own and drove myself to the ER, almost drove into the intersection because of the agonizing pains. I get there and I almost missed when they called because half the time I was tethered to the restroom. I went back, the nurse seemed nice enough, but then the Doctor came in and I explained the situation and he looked me up and down and laughed and said I get you some bentyl for IBS cramps and you shoukd be good. I'll still run other tests but I doubt it will show anything noteworthy. Also to nite I was put on one of those chairs to wait instead of a bed. I was rolling in the chair in agony and I remember him walking back up to me with his head down and not making eye contact. I said no words. I found out I had a severe infection, and toxic megacolon. He couldn't look me in the eye anymore, and said he'd put the paperwork to be admitted right away and for pain meds. Finally. I knew better before than to say something contrary to the doctor, so I simply let him see the results speak for itself.

It isn't fun and does need to be monitored to make sure its not ulcers. When I had stomach pain, I do several observations. Am I dizzy, where is the pain, how long it is happening, am I nauseous? Found out when I ate more protein and fiber, the pangs were under control. I also noticed I wasn't eating as much, only ate when I was hungry which is a huge change for me. Thus I was more hungry.
