Mad Chainsaw Mode is a hidden difficulty that shows up every so often in the demo, and will not be in the main game, it's very hard and took me around 10 serious attempts to beat it once. If you'd like to try it yourself, quit and start a new game repeatedly until you get the challenge window you see at the start of the video.
Did they confirm it won't be in the main game? I was hoping it was 😢 hopefully the hardest difficulty is still good.

Think there is also a well you sneak into to get special weapon with tons of ammo, at least before the event is triggered so might have to crouch there. To the right of where the body is burnt.
Saw it on a video. He deleted or discarded his entire inventory before opening up the village gates. I may try it tomorrow.
