    Sam will be truly missed by the Rebuild Rescue crew and community. He touched so many lives and has a lasting impact of those that he has met. We talked as a team and feel as though Sam would like us to finish the 401 project so that it can bless others along the way. We had filmed this episode of the 401 a few weeks ago and will be putting this video, and some other pre-recorded videos, out while we work through all of this.       @rebuildrescue Thank you for the quick response to the comments. The news of Sam's passing was a shock, since he had appeared regularly, and only later I saw on the news that the cause was due to a tragic accident. Sam was loved by all those that enjoy watching Rebuild Rescue, and will always be missed. We can only imagine what you are going through as a family/team, and our hearts go out to you. Please take the time you need to recover, and we will all be waiting for you when you're back to uploading. See you in heaven Sam, you will surely be missed.

      Watching Sam was such a joy. He was a great part of rebuild rescue and he will be missed. My thoughts and prayers go out to all his family and friends. I'm sure he would want the 401 finished and flying again.

    Absolutely gutted... One of the biggest moments I was looking forward too was seeing his reaction when the 401 took it's first flight again.

      Yeah me to crazy to think that he's gone now....I think Jason should put something on there saying in loving memory of Sam on the 401 when it's done because....it's like he will still be part of the build....and never forgotten

    i remember in the first videos he seemed like an incredibly grumpy old man. The longer this channel and project went on, the younger he became.

      Because he lost his passion. That’s where the plane came from. It was sitting abandoned. Sam owned it. This project quite literally changed his life, and sparked that love he had again for flying. He was Happy again 😊. This is so tragic too here about. Atleast he passed doing what he loved.

    I have also read the NTSB preliminary report and I have to say, water in the fuel is a serious oversight. You simply cannot ever skip the fuel check, especially on the first flight of the day, especially after refuelling and especially after rain - even if both fuel caps had their seals intact which, apparently, one didn't.
    With that said, I have also lost a dear friend in a seminole PA44 twin following an engine failure immediately after takeoff.
    In common with this accident, the speed was allowed to decay below Vmca leading to loss of directional control and impact with the ground.
    One big difference in the training between US and European pilots is the requirement in Europe to perform a simulated engine failure after takeoff which (for safety) is performed in the climb above 300ft with takeoff power.
    The emergency drill includes applying rudder to control yaw, ensuring full power on both throttles, full mixture and full props, identify, verify, feather failed prop and ICO failed engine, gear up, flap up and pitch for blue line (Vyse).
    Any twin aircraft is certified to climb on one engine if these procedures are correctly followed yet the US training fails to include a realistic practice or test of this exact scenario on the basis that it is too risky.
    Every European pilot will have done this multiple times and are therefore much better equipped to deal with such an emergency in the air if it should happen.
    It is tragic that Sam lost his life for a sequence of events that could have been prevented at so many missed opportunities.

    Sam was instrumental to the success of Rebuild Rescue. His enthusiasm was contagious. You can honor him by completing the 401 at the earliest instead of doing 20 projects at once.

      Don't forget to check everything you can possibly find in the beginning of every single flight. Water was found in the fuel tanks 😮

      You know that the NTSB is saying it was Sam's fatal mistake of not checking for water in the fuel that caused his death. Water in the fuel was present at the crash site. The cover for the fuel tank had no rubber seal, which is something that YOU should have checked. The preliminary report has witnesses saying Sam was "in a hurry" and did not check for water in the fuel tank.@@rebuildrescue

      They can honor him by learning from the tragedy and changing the safety culture around what they do. Aircraft are *not* a place for quick junkyard rescues, and it was only a matter of time before disaster struck.
      I’m very sorry for their loss. But it didn’t need to happen, and it doesn’t need to happen again either.

    I hope you learned also from mistakes in aviation. Reports are out and they saw the mistakes. If you don't want see more people are going in the trouble, including yourself, you must learn from mistakes, word "hurry" may not on the list to skip some steps. Never do it again. Just follow all checklists. No skipping.
    Water in fuel tanks... that is a serious problem that can easy checked.

      The "Rush to get it home" Then it was said elsewhere it was on its way to being delivered to a buyer when it went down. Sigh... Remembering your R&R video with the International Harvester dump truck episode, You drove it back in the dark on a highway with a known profusely leaking hydraulic brake system. As a licensed professional, I was shocked by that poor decision. It never should have been on the road until those brakes were 100% . Not only your life , But other lives on the highway were also in peril if more had went wrong. Dropping from your subscribers list is well in order. Not angry simply disappointed. Have a good life.

I bet when Sam's family reads these comments they think WOW that adam guy sure is a class act. You know I read once whatever you put out you get back ten fold...theres a word for it but it escapes me right now. Anyways I wish you well and sure hope you get a lot of whatever it is.

    The best thing was watching Sam in the earliest videos. He seemed standoffish. He didn’t smile much and didn’t seem into the UA-cam thing at all.
    Rebuild rescue seemed to make him happy. We could see the evolution.

      Totally agree. Sam seemed to come alive the more he was involved with Rebuild Rescue. It was great to watch. I will miss Sam and especially not having him there when the 401 has its first flight. Thank you Sam for everything you gave us by being part of Rebuild Rescue. RIP Sam and fly high.

    Preliminary NTSB report says water in the carb. Sam sadly did not do a pre-flight, but a pilot's pre-flight is a last line of defense. I hope if Rebuild Rescue continues with operations, they do the proper self-examination and put safety front and center from here on out. From RBR videos, Sam seemed like an awesome guy and a great pilot, but even great pilots are still human.
    Frustrating thing about this is, this crash was 100% preventable, Sam did not have to die to learn this harsh lesson.

There are scenarios that would be outside of the aircraft manufacturer's, the maintenance crew, and the pilots control that can have potentially just as tragic outcomes. One example, bird strikes, despite our best efforts, this is something the industry still struggles with. So for your statement to be true in the context of my above statement, not flying at all, ever, would be the only answer and that's just asinine.

      NTSB report says he didn't check his fuel for water.
      Water was everywhere in the fuel system and right engine died left engine was about to as well.

      I think a reaction video to parts of what Blancolirio had to say about this incident, and some of the comments to that video. Water in the fuel is only the last part of the incident, but not how and why there was water in the fuel system. There were also questions about whether the failed engine was feathered. The gasket on the gas cap was apparently missing, potentially allowing water into the tank in the first place. Was a proper review of the engine failure instructions done before takeoff? One engine isn't enough to climb in that aircraft unless the failed engine was feathered. Was that a memory item? It sounds like there was big pressure to deliver the aircraft on schedule to the buyer to raise money for the channel, and a flight to make right after delivery. A slow fuel delivery was interfering with those plans. Many holes in the cheese lined up. I'm sure there are many people considering their part in this incident, and lawsuits may be inbound.

      @@benmac940yes. The “cheapest twin”, the second cougar they acquired, was the accident aircraft. The one they “had” to fix in 5 days. The one that had a double prop strike but they elected to not do engine overhauls to save cost. But couldn’t replace a filler cap o ring.

    Going forward, honor Sam by continuing the work and continuing Rebuild Rescue saving aircraft and by slowing down. Being rushed in aviation can be unforgiving. It's so easy to get rushed.     I dunno. I'll reserve my most critical judgements for the final accident report, but the preliminary report is showing that the gasket was not installed on the gas cap. I've been delayed on a trip, because the gasket on my pa31 was the last thing to get to the shop, and the annual could not be signed off on. I'm just a pilot with a degree in aerospace eng, so I am not an expert on the exact procedures/laws regarding this. HOWEVER, this missing gasket does seem like something that should have been looked at while working on his plane before it was handed off to him. And maybe it was mentioned to Sam and perhaps there is not a requirement in the US to have the gasket completed on signoff. Yes there is responsibility of the pilot as well to check the fuel, but still, I feel that theres some responsibility lies on RR here. I will agree with the majority of the experts in the aviation community that the format of fixing planes quickly on the cheap by under-qualified individuals is a recipe for disaster. Having a mechanic inspect/complete your work, but you are also his boss with a show/interests is also not really what these apprenticeship/self work exceptions were designed for.
    This video, kinda half dedicated to Sam, also reads as really disingenuous, and my gut is telling me something is very off about the demeanor and format of this video. I get that "he would have wanted us to continue working on it" aspect, but, he would have also rather not died. The creator here is obviously in a tricky situation, cant divulge much for legal reasons, and is probably struggled to find the right tone... I think RR is really lucky that the intricacies of this case are too complicated for the average social media consumer to really understand, and why this format should be seriously reconsidered...but more genuine apologies have been eaten alive on the internet, and I understand that this video isnt an apology, but this story and vid is really a ticking time bomb.
    Lastly, given all of this, I know there were some ideas to have this 401 used for angel flights, donating the plane for transporting kids for intro flights, or providing air travel for people who need /deserve it/cant afford it. I really hope more people aren't killed as a result of trying to get this plane operational hastily, being worked on by people who really are not experienced enough or qualified enough to be doing it, or being flown by pilots with little/no experience on these kinds of twins with little aviation experience. The 401 is a great plane, but like many light twins of this size and era, they are very complex designs (mechanically and piloting), not designed for longevity, not super forgiving, and are for the most part, antiquated. Im sorry if this is rubbing the fans the wrong way, or if this seems like a dissenting opinion here, but I had to put my thoughts out there.

      Completely agree. TV style "can we rebuild the house in just one week" dramas have NO place in the unforgiving world of aviation. Blancolirio features an excellent and sobering breakdown of this preventable tragedy.

      Very well said. I followed the channel for a few years and once the side rebuilds started as quick flips I began to feel like it was becoming a bit sketchy and even commented about a year ago I felt like his inspector shouldn't really be the guy who works for you. I took so much heat for that comment, yet here we are.
      My opinion, work on the 401 should stop, channel should be shut down and Jason go back to tinkering with cars.

      So what is the timeline you think adequate for 401 rebuild? I mean the first videos are 2 years ago and it still seems 6-9 months out before it's back together. I know they have other projects at various stages of work as well, but to say blame lies with RR while no less than 5 other repairs were done after it left them and RR didn't do the annual inspection seems a bit short sighted.

      @@Zappy1210 I have a slightly different opinion, but agree with you in general terms. Safety is a culture, and a choice. If you go back and rewatch most of the RR videos, safety and doing it right is mentioned, but it does not seem like a culture.
      Perhaps there are legal considerations, and a desire to “just remember Sam”, but he did not need to die to learn these lessons. Plenty of people have learned before them. This seems like a common problem on these planes. Was it left outside? Did it get rained on? This video does not even mention that Sam died in a plane crash in the Cougar that had been prominently featured as a flip on this channel. I find that shocking.

    Really gutted by Sam's accident and my heart goes out to his family and all at Rebuild Rescue. You have been one of the main influences in getting me back into aviation after a long break. I love the dream of rescuing aircraft and am so sad about the loss of the Cougar. I am sure you've spent a lot of time second guessing yourselves over the last two weeks. I am really glad to see you're posting videos again. Probably not now, but when the time is right I would appreciate a video about lessons learned that might help us all fly more safely.

    Beautiful tribute to your friend and the friendship all of you shared with him. When we lose people we love, continuing forward without them seems insurmountable and our grief is often an unbearable weight but I know for sure, they are with us, can see us and want us to walk, run and fly. So I’ll share with you what my wife left behind for me, “Fly, be happy”.

      Blancolirio channel put out a lengthy video explaining how this all went wrong using the most recent FAA update. Water and debris in the fuel...no gasket on the fuel cap. He blames the nature of this channel for the harm done. Cutting costs and time to get planes running.

      @@RickMcCargar give it a rest stop commenting on every damn video. All he’s doing is speculating in his video and that’s all everyone is doing leave the investigating to the NTSB and the FAA that’s their job. We will know what happen when they release their final report.
      All the work that was done, was done under the supervision of and inspected by a licensed A&P/IA who then personally test flew the airplane. And then took it on a lengthy ferry flight.
      Get a life and stop being a troll.

    I read the preliminary NTSB report on the Cougar crash. Very sad to think water in the fuel and a missing fuel cap gasket allowing water in the fuel tank may have caused it. It appears that checking for that condition is fairly simple but he was in a hurry to get in the air and didn't check. It shows that mistakes by those with a lot of hours and experience flying can have very bad consequences. A good man gone too soon. Slow down and be careful guys.

      Well water in the fuel wasn't from it sitting because it was flown 4 test flights and it flew 500 miles back to rebuild rescues hanger with no problems.

      Was the aircraft stored outside?? , we don't know , so do not jump the gun on the preliminary NTSB report because as stated, it's preliminary!! ,
      Yes, the fuel seal gasket was missing on the filler cap, but the water could just have easily come from the fuel truck.
      However, I do think a sump test should have been done regardless of the recent test flights.
      What an absolute nightmare for all concerned 😟

    My heart goes out to you, your family and friends that knew him also. Today was my best friend’s birthday, whom I’ve known for at least 30 years. We lost him last year to covid suddenly and I miss him dearly. As time goes on, the pain lessens. But there are those triggers, such as reading my post on his Facebook for his birthday which brought back the loss instantly, setting me down for a while. Appreciate every moment there is, because time can be short. Good lock in moving forward. I always look forward to your videos and I have learned quite a bit about plane mechanics.

    When I heard the news about Sam’s accident, I went numb for a while. As a fellow pilot who has seen many accidents over the years, I always said there but for the grace of God go I. Rip Sam. I know he would want Rebuild Rescue to finish the job they have started thanks to him.

    I have just discover your channel yesterday and watched all episodes of Sam's old plane and his planes taking off. Then I can´t describe what I feel right now.

    Sam’s smile always made me smile. It was contagious even through videos. I never met him, but still considered him and the RR crew friends. He I’ll be missed by all of us, but the Rebuild Rescue needs or soldier on. Sam would have wanted it that way. Rest in peace, Sam.❤

    Cause of accident: failure to put a rubber gasket on the gas cap, as well as pilot failure to check for water in the fuel tank during preflight check.

    My favorite part of a Saturday morning is walking back to my PC with a cup of hot coffee to check out the latest RR video. This was the last thing I expected to see and my heart truly hurts to get this news. I knew Sam was a very special part of all this. I had once even asked how his life had changed since meeting you guys. No question he meant the world to you and likewise yall meant the world to him. His legacy will live on through Rebuild Rescue without a doubt. What you guys will continue to do will be a wonderful tribute to Sam. Blessings to all.

    Rest in peace Sam! You will be greatly missed. My favorite video of Sam was when the rebuild rescue team helped Sam out with his airplane and his hanger. He truly light up you could see it through the video. Keep up the good work Jason and the team. I know Sam would want to see the 401 completed.

      At some point the fans of this channel need to come to grips with the fact that the 401 project is never going to be completed. It's like click bait, they use it to get you to watch and support their other projects. Sam would have also wanted the crew to check out the seal of the gas cap on the right wing after the plane had sat outside for years.

    Very sorry for your loss. Let this be a lesson that aircraft cannot pull over in the sky for a $5 part that someone forgot to install and that a proper preflight is always important no matter how rushed you are to get somewhere. 😢 rules to live by written in blood.

      @@MartinMcMartin mostly agree but that is why you always take fuel samples! you never know when you will have an unexpected failure (or somebody not installing all the parts)

      You are right, a proper pre-flight is essential, but there is also the risk of over-confidence (I'm not saying this was the case for this crash). I am reminded of the tragic death of F14 veteran Dale "Snort" Snodgrass who was killed in his Marchetti SM-1019B because he did not check that the control lock had been removed before his takeoff. Regular use of checklists, despite them seeming mundane, boring and irrelevant, saves lives.

      Give the crew time to grieve and pay tribute to a really great guy. His loyalty and genuine excitement to see the 401 fly again was contagious. It is the same energy that keeps 700 K

    To Jason and all the rescue crew. A lot of us have been with you from the beginning of the 401 series and so we got to know Sam on here to a point where he felt like he was one of our friends too, even though we never met him. It's an unusual feeling to grieve someone you never really knew, but we are grieving with you right now. I'm so happy to see another video and want to encourage you to keep doing what you do and try to save these airplanes. Your fans need to still see you putting out videos because we know that's what Sam would have wanted.

    Watching Sam as he went from a little reticent to engage with you all, to being a pillar of the team was one of my favourite parts of this journey.
    I feel conflicted for (all), your loss of Sam. But also a feeling of “quiet happiness” that he, and his efforts will live on with all of you and all of us who care.

    I somewhat understand your feelings. I lost a friend of mine in the last year from an aerial application accident. He was more than 10 years younger than me and we had not seen each other in years. It’s a punch in the gut. He will be proudly watching and laughing along with you.
