To find the ghoul lair, you can pull up the map and look for a brown circle. There should only be 1, but sometimes it's hard to spot because the lair spawned on the edge of the map, next to town, or by some other POI that distorts the brown circle shape.

That ghoul layer is one of the most terrifying things to go into. I'm just glad they fixed the wall ghoul glitch

You need to do the barman quests to unlock maps. The quest to unlock makeshift is called "Towards Makeshift Camp"
