I agree with you that doesn't make sense -when you're taking estrogen you need to be on progesterone, and generally it's given continuously (orally once a day at bedtime ) so that we don't bleed
Might be good to seek a second opinion

Dr. Susan, I’m 65 and just started HRT because I have osteoporosis. I did not have any hot flashes, weight gain, anxiety or insomnia. I use vagifem for vaginal dryness (it’s just ok) but I was afraid of HRT until I started watching you and many other experts. So my NP started me on Estradiol and Testosterone drops. She didn’t add Progesterone until one month after at 50 mg. After two weeks I dev vaginal bleeding and she asked me to stop Progesterone. She said we need to build up my uterine lining before restarting Progesterone. Does that make any sense? I thought we need Progesterone to stop bleeding. Thank you.

That doesn’t make sense. Go to a NAMS certified doctor.

How are your most resourced sophisticated patients not understanding this concept? It's not that difficult. My background is dietetics, so I'm used to this information. But there are way more complicated biochem processes in the body than this.

Because lay people, even if they have degrees in other subjects and may be very bright, are not experts in metabolic processes. My Dad put it succinctly: "When you get old(er), your body processes/cellular machinery breaks down. True. And our western diets (and stress) don't help.

My patients are 45-65... totally not what we were taught growing up! So it's hard to shed old ideas !
