Iodinated Contrast used for CT scan can induce thyroid storm in patients with pre existing thyroid disease or with thyrotoxicosis. (Something like Jod-basedow effect but it takes 2-12 weeks to appear and stays longer) Therefore, MRI may be preferred in these patients.
Sir if we won't attempt for thyroid suppression therapy in such a patient of retrosternsl goitre extension then won't the surgery done in this hyperthyroid status will result in thyroid storm during surgery?

It tells about show much it has extended into the thoracic cavity Ina case of retrosternal goitre, as in this case
Also non iodinated CECT needs to be done. Best do MRI to also grade the extension.
