- Trustsphere aims to support the complex journeys of children and families living with chronic disease by addressing data siloes and offering a trusted digital infrastructure to share critical information such as patient generated data, care plans, and educational resources, between patients and their providers. In this presentation, we describe the co-creation of TrustSphere with patients, families and clinicians, its core features and functionalities and the results of a pilot study of TrustSphere in the BC Children’s Hospital Diabetes Clinic.
