Before pregnancy if a patient is on Warfarin for valular heart disease , during pregnancy, patients are often switched to alternative anticoagulation therapies which include low-molecular-weight heparin (LMWH) or unfractionated heparin (UFH), to avoid teratogenic effects from Warfarin. How to monitor it with INR require deep understanding to avoid an mishap.
When we do abridging of warfarin before surgery the patient must receive clexane 3 days before the surgery, is that including the day of the surgery? Does the patient receive clexane in the day of the surgery?
Heparin increase risk of osteoporosis because their is difficiency of Ca caused by Heparin, thus come out from the deposit in the bines. Am I right? By the way I'm not genius 🤠

IIt’s made in vitro, but can work in vivo. Unlike heparin, which is naturally made by the human body, warfarin isn’t made in your body.
