Had I known I was going to get PC by some kind of test or scan, I would have happily had my prostate removed BEFORE the cancer developed in it to Gleason 4+5, but nobody told me of the possibility or risk as my PSA slowly rose from 1.3 to 4.5, or even after that for 5 years when it dipped slightly below 4. They just kept sticking their finger in my back door and telling me that my prostate was "large but smooth". The ignorance and/or indifference in parts of the medical community is appalling, and it is killing men and subjecting others of us to painful, physically devastating, and in the end, unsuccessful treatments as the cancer spreads, becomes hormone resistant, and eventually kills us. PSA screening accomplishes nothing if the doctors don't ACT in response to a negative trend before it is too late. Love you guys!

Removing the prostate is not a perfect solution. Many time the cancer returns. removing the prostate can have a number of complications: wearing a urine bag, impotence . The PSA is a poor screening test but necessary. You can have a low PSA and have cancer and a high PSA with no cancer.

I discovered my PSA was 17.4 at age 62 in 2020, but biopsy did not find cancer. When my PSA reached 30.4 two years later, MRI located the Stage 3 lesion in my anterior transition zone base which is a rare area to get prostate cancer , and biopsy found grade 6 and 7 in two of the 10 core samples taken. We decided on active surveillance. A year later in March of this year I was having a lot of pain, my PSA was 37.2, MRI determined my lesion was now Stage 5, and PMSA CT scan determined my cancer was still contained in my prostate. I underwent 6 months of hormone therapy along with 28 sessions of external beam radiation therapy of my prostate, seminal vessels, and pelvic lymph nodes. My first survivorship checkup in August showed a PSA of 0.11. Due to the severe hot flashes I am experiencing, hormone therapy ended at 6 months, but I could end up back on it if my PSA rises by my next survivorship checkup coming up in February 2024. I found the hormone therapy to be wicked. But with a 37.2 PSA I was lucky it had not spread into my bones yet.

Hind sight is always 20/20. Many people have high PSA and never have cancer. Mine is high, my brothers is high, no cancer. My dad had advanced prostate cancer and his was removed. 10 years after his was removed his PSA was 20. Lupron put it under 1 but he was old and made him fall apart. I had a fight with his doctor to use that stuff intermittently and had him use 8 to 10 mg of topical progesterone. This put him in remission for 7 years until his passing at 93.

My psa was as high as 7.5. I use dhea and now anastrozole. When I stop for a month its drops by about a point and 1/2. My free testosterone I pushed up to about 20. My brother is athletic and fit. No hormone replacement except now he uses progesterone_and his PSA has been as high has 12. He is 5 years younger. He has had random biopsy which is 12 to 14 needles which was negative. About year later he had a t3 MRI which was negative. I had a t3 MRI it was negative. The T3 mri is better than the random biopsy. Its more accurate.

I had an enlarged prostrate for years. I had been diagnosed had a biopsy , got my prostrate removed, still had a psa number had radiation and hormone therapy. My hormone therapy has stopped my psa is undetectable and my testosterone numbers are going back up. I feel better every day ( less tired) and i am slowly improving my interest in sexual activity I am 3 years out from surgery. Radiation therapy was done less than 6 months after surgery. I think it was well worth it . I had a family history of problems but my dad lived 40 years after his treatments.

It's driving me nuts trying to figure out the best diet. Vegetarian whole food plant based diet or the new trend of Keto that eliminates all sources of sugar and piles on the fat at 80%, meat and some veggies. Wish there was a study that can say which is the best way to stop or control prostate cancer.
At 68 my Dr waited till my PSA was a 4.18 before he set me up with the MRI and it came back with PIRADS 5 with a possible bone spread. So here we go on the cancer journey.

I agreed with this 99% of it. I would strongly suggest to be more specific about life styles that those that are more prone to developing prostate cancer be detailed specifically those that have been diagnosed. The male sperm/semen contains fructose and glucose which cancer needs to survive and spread. Compiled that with a high carbohydrate diet, consumption of processed and ultra-processed foods, a sedentary lifestyle, no or limited exercise, obesity, frequent eating, and so on, would be a recipe for the development of prostate cancer.

Do I regret not seeking help sooner??
Absolutely not. I had 15 years of good life that would have been hell if I had gone to a doctor. There was an interesting German study that used MRI and PMSA with no biopsy. All (100%) had significant cancer and the prostate was removed. It’s rare in medicine to have 100% accuracy. It was a very small study with only 24 men and 52 lesions. I believe this speaks to some of the comment when men have significant cancer that needs to be removed sooner than later. And isn’t that the goal, to find the significant cancer early and treat it and not treat the insignificant cancer. I also think that biomarkers could also assist as another factor to skip the biopsy and treat, or not treat.

    • @hn5460

Myself, I have a fast rising PSA level of 22 without symptoms, negative digital exam, negative MRI and PSMA scan results, but my doctor still wanted a biopsy and I agreed with his request after doing my own calculation. My own calculation showed that a combination of MRI and PSMA scans will only have a 90% negative predictive value and a 90% positive predictive value for clinically significant prostate cancer.

If all men are destined to get PC, no matter what they do, it seems like the recommended course of action should be for all of us to have our prostates removed, before they become cancerous. Also, don't treatments often work better for those who have taken on a healthy lifestyle (vegetarian diet and exercise)?
