  • In this article we want to answer, what does a high dhea sulfate in a female mean? This can come up on labs in a lot of females with PCOS. This leaves many with PCOS asking what does this mean and what can I do. This is not necessarily dangerous in and of itself but it can help us understand what could be going on in your body and more importantly why. There are additional lab studies to look at. We will look at the things that stimulate DHEA-s to be produced and also how it is being broken down. With this information we can triangulate what could be going on in your body to produce this level especially if it's very consistently like that.

    so look at the things that stimulate dhea to be produced and how it's being broken down. with that we can see what does a high dhea sulfate in a female mean and why it is high dhea in pcos.
    Let's look at what a high dhea sulfate and a female means [Music] so what does a high dhea sulfate and a female mean what does that tell us about what's going on in their body and if it shows up in your labs what does it actually mean what can you do about it things like this so that's we're going to be addressing and as with any system hormonal system etc you want to look at what's coming into that system and how it's how the the substance is being eliminated so what stimulates the production of the dhea sulfate and then how is it taken out of the system or broken down so with dhea the main source of the dhea is being produced from the adrenal glands and it's the innermost part of the adrenal glands that's producing that so what causes the stimulus for the dha to be produced mainly that comes from a hormone called acth and the acth it comes from the pituitary gland and it stimulates the adrenal glands mostly to produce glucocorticoids the acth is being stimulated when there's a fight or flight response so you get some activation whether you're in a car accident or someone's chasing you you're going to get more acth produced and that's going to stimulate the adrenal glands to produce cortisol along with that the innermost part of the adrenal glands are going to be stimulated and you're going to get increased production of dhea now there's always some background production of the acth and therefore cortisol so what we want to look at is what's you know above normal what's above your baseline level that could be a cause for increased dhea when we're looking at cortisol levels there's a few different ways to do that and the morning cortisol is a good option but when that's seemingly not reflective of what seems to be going on you can also do a hour cortisol to evaluate this so a spot check in your court am cortisol levels may show normal but if you look at the total load over hour period you may it may show like above normal levels so those are two ways that we typically use there's also salivary cortisol which is another option we don't tend to use that one as much so the question then is you have high dha sulfate do you also have high cortisol and how do you measure that so you got to make sure you might want to do it several times and look at different angles if you're not if it's not correlating with the high dhea so so this i know for sure can cause elevated dhea levels okay so what happens if you actually don't have high cortisol you've done several tests everything looks normal well then we want to look at what could be going on with the breakdown of your dhea sulfate now dha sulfate is turned into androstenedione and then testosterone there are some alternative pathways for this uh could be turned into estrogens as well they they're all dependent on one enzyme and that enzyme is called three beta hydroxy steroid dehydrogenase and it makes that conversion fairly readily but it is dependent on a few different things so if there is a high amount of androgen receptor and estrogen receptor activity then this enzyme can be down regulated so if you have high testosterone or high estrogen you may get a down regulation in this enzyme therefore raising the dhea sulfate or not allowing it to be converted into its normal path the other thing that regulates the three beta hydroxy steroid dehydrogenase is the cofactor nad your body is always keeping is trying to keep a relatively higher amount of nad to nadh and when the ratio flips when you have more nadh it kind of slows down the overall metabolism and you see this with in people with diabetes so your body's always in flux but people with insulin resistance and diabetes typically have low nad. with pcos often have insulin resistance DHEA-s

    Wow this was an amazing explanation! I have struggled for years, I even had a pituitary tumor at one point and was scared I had cushing’s. I first presented with slightly high prolactin and high DHEA-S (not over 100pts high) and I was out on cabergoline and both dropped due to the drug. I was on it for 2 years and was convinced I had something more so I went to a cushing’s specialist endo and he told me “you may have no Rotterdam criteria signs of pcos but it’s your ovaries causing prolactin to go high, and you have insulin resistance, don’t let anyone touch your pituitary gland.” I had never had any high insulin or glucose tests and I took them all. I knew intuitively I gad some form of it…and he used a CGM on me and saw I did have hypoglycemia. So he put me on metformin only, and my prolactin has never been high. I had to get off it due to high liver enzymes. So I came to Japan, went to the best endo thinking I would get a second opinion. My Cortisol 24hr was actually normal but DHEA-S and ACTH were high. Again, not crazy high. But I had really slipped on my diet due to moving to a new country and it’s crazy to me how the answer all along has been insulin resistance and adrenal/lean pcos. I also have never had high T, it’s usually on the lower side like you mentioned.

    The info on high dheas and low estradiol and testosterone really caught my attention since this is what I have. Is there a blood test to confirm low NAD levels? If so is there a way to supplement and help dheas convert to downstream? Ive been researching for years....glad I stumbled upon your channel!

    Hi. I have had extremely high DHEA levels for about a year and a half.. the lowest being in mid 400’s and the highest being almost 800. Cortisol came back ok. They ruled
    Out congenital adrenal hyperplasia. I haven’t slept for more than 2 hours at a time in almost 2 years. I am 41 and have NEVER had problems sleeping. I’ve gained 50 lbs. I’ve seen several doctors and while they all agree my levels are extremely high, no one can figure out why. Please help. I’m exhausted and feel bad most of the time. I just want to feel ok again.

      Hi Hedi, I would have to look at your whole medical picture (symptoms test you done and have not done) in order to help you with this. It also depends if you are measuring DHEA or DHEA-s and many other variables.

    Thank you so much for your detailed description and explanation. I’m a type one diabetic(diagnosed at 15) w/ high testosterone/cholesterol/PCOS(diagnosed at 18), and recently had a full work up on hormone levels due to ongoing issues relating to my female organs. I never “hit puberty” and at 36, am still in the body of a 12 year old! No breasts, hips and very thin. My DHEA-Sulfate came back at 517, which now I understand correlates to the high testosterone levels. My question is regarding the enzymes that may link my conditions and if there are therapies available. I currently am on an insulin regimen and stopped taking metformin in my early 20’s after changing endocrinologists. Your input would be greatly appreciated!

      hi Selena, It may not be one single enzyme but I would need to look at the actual results to tell you. Some of these tests cannot be done through blood either. You may have an aromatase enzyme deficiency. More videos on this coming soon.

    This video explanation is super helpful. I'm on HRT and just had my six-month blood work done showing 337 DHEA-s which is high. Cortisol and all other labs in the normal range. I've recently lost 22lbs too. I am wondering if my HRT (including testosterone cream and progesterone) is the reason for this high number. Thanks again. Very informative.

    I have pcos and recently my period has stopped. Hormone levels were normal but dheas was elevated. Thyroid was normal as well. My obgyn said it wasn’t clinically relevant. Hair hair has progressively thinned so much my pony tail is no thicker than my thumb and I can see my scalp. I can’t lose weight. I also breastfeed my 2.5 year old.

    Interesting. Does that mean that high dhea is caused by stressed and that’s why I have so much acne ? My ex been stressing me out , that could make so much sense

    I have very high cortisol EVERY Time i have my period and if my period is Over i have high dheas always !!! I tested it for 1 year every Month during period and not during period alyways These Labs coming out ? What can Cause something Like this? (My acth is always normal by the way) the Doctors say there is something wrong but dont know what exactly … and i have very Heavy Symptoms !!!     what are any suggestions you may have as to how someone can lower DHEAs naturally or with medication/supplements besides trying to balance stress? mine are high and i’m in the process of being diagnosed with pcos, that was the highest hormone that came back after testing

    hi there! Great video. I have high Dhea-s with apparent symptoms (especially excessive chin hair, body hair). My estrogen is a bit low. Prolactin is a bit high. My ultrasound revealed my one ovary is larger and bulkier than the other one (suggesting extra follicles in one rather than the other ovary). what do you think this means? I went to my doctor but they’re not sure, so I’m being sent to an endocrinologist. Thanks please let me know :)

    Hey doc. I know you posted this awhile ago. I am on HRT creams. 4 yrs into menopause. Just drew labs. Test 83, free test 2.0 DHEA-S 500, Estradiol 11.9, Progesterone 6.6. A1C 4.8. Just had back to back total hip replacements. Blood draw approx 3 weeks after 2nd replacement. I feel stupid as I have been dosing myself with 5-25 mg per day of DHEA for the last 4-6 weeks. Would this be enough to cause such a high DHEA? Obviously Im stopping it now. Also for being on HRT it seems my est and prog are pretty low. I really hope you read this and THANKS IN ADVANCE IF YOU CAN ANSWER THIS. XO

    My Dheas came back at 451(21 yr old female), the lab report says it is high. However everything else came out normal. But I am concerned. I searched up high levels DHEAS and it said I could have an adrenal carcinoma.. I have been experiencing very high levels of stress. Im just so confused and concerned

      451 is a bit high but you should follow the advise of your doctor and see if they can repeat the test or evaluate you further to find the source. I have several other videos on high DHEA's so check those out .

    Hey, doc. Im a male, 25 years old suffering with idiopatic intracranianal hypertension. I did all the tests and images exams that u can imagine, but i just got one test that wasnt normal. My dheas is high, and im together with my endocrinologist reserching for a cause. Ive got 588 wheres the range in the lab goes up to 492, just. Do u think that it can affect my icp? Some studies showed evidences that excess of androgens que raise the ICP in woman, but I'm a man... Also im a pharmacist and medicine student, ive been fighting for two years long trying to figure out the cause of my IIH...

      Hard to say but I would look more at the stronger androgens...DHT and testosterone.

    Hello. I am a woman and i am 30y old. I did some hormonal test and i have high Androstendione, high Progesterone and high Prolactina. The rest of the hormonal results are in range.
    I also have ovarian cysts I also know i have high Cholesterol in my blood and dehydratation.
    My doctor said its all normal but i dont understand what causes the big results and what should i do to lower those levels.

      Context is always important when looking at blood tests... so if you are having symptoms that match the results (or any symptoms), re-test or find a new doctor. The whole point of a reference range is to identify where the problems are coming from. That said, some people may normally run a little high or little low and that can be normal for them... context context context

    I’ve been dealing with facial hair and I’m extremely confused because it does run in my family for females.

    Does this effect fertility? All my blood work came back completely normal, only thing off was elevated DHEA-S but only a couple units. Doctor told me he could put me on certain medication to help me get pregnant but is there anything I can do naturally?

      High DHEA-s does not cause fertility issues but it depends on what is causing it to be high. Slightly high levels are usually not a problem but high levels are correlated with PCOS and therefore infertility.

    Im male 24 my dhea-s is 18,6. reference 4,34-12,2. had a very stressfull time lasts month where i lose 30 pounds. Anxiety disorder. Can this be the cause?

    Is my high result of DHEA sulfate related to my high levels of estrogen too? I’m 35 and the DHEA levels came to 379 (fasting) progesterone 10.9 and I can’t remember the estrogen levels, but I was told they were high.
    I have been under a lot of stress and wonder if all of those results are related, plus if also me being 17 pounds overweight for my recommended weigh has something to do. I’m al ready working on loosing weight and have lost 6pounds already, but still overweight and overwhelmed by my results. Hope to find some answers from you upcoming video, but also for my questions here.
    I really appreciate it. I haven’t find a lot of information on this matter. So again Thank you!

    I am struggling with pcos. Pre birth control my testosterone and dheas were both high. Now testosterone is in the middle and dheas is on the very high end. Can high normal dheas still cause androgenic symptoms? Why sometimes spiro which is supposed to block androgens lower dheas but sometimes make it higher?

      Hi Jelizaveta Kogan, these are good questions and I think the answer will be a bit clearer when my next video on how to lower dhea-s comes out. Some birth control can decrease the conversion of dhea to testosterone so this might be why dhea-s is higher.

    Hi doctor, I'm female 24 years old I have high dhea_s. Both free and normal testosterone are normal. cortisol pm, sodium, potassium and TSH are also normal . First test lab of dhea_s was 533 ug/dl my doctor gave me sipronolacton after taking sipronolacton 4 months it became 625 . I have pcos and lots of hair lose but my aldosterone is 46.9ng/ml higher than normal 3 degrees the ultrasound showed that I had few crystals in left kidney is that causes high aldosterone? and why sipronolacton caused high dhea_s?
    plllzz I'm soo exhausted 😣😣 how do I lower dhea_s level?

      I don’t have an easy answer it takes time to analyze all your hormone results to determine this cause. you probably need to work with an endocrinologist or a hormone specialist.

    Hi. I am 26 years old male. My dheas level is usually around 700-800 microgram/dL. Also i have high estradiol level like 70-85 pg/mL. No tumors was found in my MR scan. What could be my problem?

      Same problem. We may have some enzymatic issue either in 11beta hydroxylase, 21 hydroxylase, 17 hydroxylase, or STS ( Sulfur transferase synthase)

      I don't know if DHEA is associated with endometriosis. Endometreosis gets worse from high estrogen states and Dhea can stimulate estrogen receptors.

    Hi great video quick question I have a two year old daughter was able to get pregnant very very quick in a few months and hardly trying but yes I do have PCOS but I have perfect periods like a clock work only time it came late was when I was pregnant at 28.     I’m 54 postmeno been on HRT. Levels are within range considering supplementing with HRT; however, been having aggressive anxiety for the past few years. Finally checked DHEA, level is 779!!!! 😞
    I suspect this is what has been causing the terrible anxiety. Other levels are: Estradiol 185, Estrone 143, Testosterone Total 129, Testosterone Free 1.16, Progesterone 1.7
    Doctor thinks perhaps a tumor on pituitary or adrenal gland causing high DHEA?
    Any thoughts? The anxiety is a tumultuous storm!!! 😞

      Probably need to rule that out but also check cortisol. These levels of estradiol Testosterone etc are somewhat normal but depends on when you took them. If it had been more than about 18 hrs since you took them you are looking at the low values and high values can increase cortisol and with that dhea-s. Something I have seen empirically.

      @@swintegrative Thanks for the response. Labs were drawn 24 hours after hormone injections. Would it be reasonable to say that lab results were from peak levels?

      Are you taking them as injections, pellets, topical cream, or troche? If you had an injection 24 hrs before blood draw, this should be close to peak levels (sometimes it takes 48hrs). For cream or troche it is the trough level.

      @@swintegrative estrogen and testosterone injections…in the process of titrating progesterone. It has been most difficult to find balance of E and P. Currently on 0.26 ml E every 3 1/2 days along with 0.06ml T every 3 1/2 days (both injections). Just increased P to 100 oral plus 100 rectal (promethium). Still getting anxiety, but P was just increased yesterday. Does it take days/weeks for P to get saturated in my body?

      Hi thanks for your comments. It is hard to say but there is no direct link of high cortisol and breast cancer. There is of course the link between stress and cancer but that is too general to be useful.

    I’m a male, my test and estrogen levels in the normal range. However, DHEA is in the high 800s range . I’m not on any supplements either ... any reason why I’m so high? I scheduled a apt with my PCP to get my adrenal glands checked

    Hello! This is a very useful info! Can you please talk more on solutions to normalize DHEA?
    I’m 35 y.o., lean body type, and from the recent blood work my DHEA-Sulfate showed 559 ug/dL and DHEA- Serum 815 ug/dl, all other tests are normal (Androstendion, Testosterone, Cortisol AM, ACTH), my glucose normal too.
    The doctor referring me to do MRI on adrenal glands, but what is after? I cannot figure out why is this parameter has been off for me for many years now? I am not taking any hormones. Thank you!

    Hello, Thanks for the informative video. My labs show that my DHEA-s is high, Testosterone low, Estradiol low, Progesterone low, and Cortisol is pretty low. I have searched for answers and can’t seem to figure out what is going on. My doctor said that it isn’t possible to have high DHEA-s and low Testosterone. Is that correct? Thank you!

      Hi JustPlaneBiggs, there are a few scenarios where this is possible. For instance, birth control decrease the activity of enzymes that allow conversion of DHEA-s into testosterone. This scenario create high DHEA-s and low testosterone.

      Depending on how high the dhea-s is they may want to do testing to rule out adrenal hyperplasia.

      Did you ever get a diagnosis? I had an adrenal tumor removed with these symptoms, but even after my adrenalectomy, my levels are still high with symptoms. My endocrinologist even consulted with the medical board and couldn’t figure anything out .

      Your doctor is wrong. It doesn’t always convert from DHEA to T and that can be suppressed by many different things. What is your fasting insulin? it’s a matter of finding the cause of the DHEA not converting

      Hi Mualla, yes under-eating can cause you to loose periods for sure. I am not sure about the DHE-s but that makes logical sense for it to go up in compensation for your other hormones shutting down.

    i have high DHEA-S (347), high FSH (145), high Thyroid peroxidase AB (99 iu/ml) and high thyroglobulin antibodies (2iu/ml).. tired, not much appetite, lost 5 pounds in a month or two. what are your thoughts given all these high levels? thank you.

    Hi! I’ve been struggling with hormonal acne, weight gain and heavy periods my whole life. I recently went to the doctor and all of my bloodwork came back normal. My dheas was between 465 and 480 (I went twice) both times my doctor said that’s perfect and not causing my symptoms. Is this accurate?

      It's hard to say for sure but the DHEAs seems high. DHEA-s is a precursor to androgens which can cause acne. You have to do a detailed analysis of all the hormones and their metabolites.

      Hi Tess, stress is a common reason for high cortisol and high cortisol can cause high DHEA-s.

    I just got blood work I am 23 years old and my DHEA level was 475 I know that is high but is that considered really badly high? I was also on birth control for 4 years and just came off of it 4 months ago , could that have anything to do with it ?
