    Very good explanation of the normal apical impulse or the PMI. It has been written that only God knows the explanation of the E Wave. Why would there be a brief expansion of the LV in early systole when the LV should be emptying and moving away from the chest wall. A possible explanation is isovolumic contraction. During this early period of systole, the longitudinal fibers in the subendocardial portion of the LV are activated to prepare the LV for ejection of its content. The mitral apparatus being attached to the top of these fibers is pulled down toward the apex of the LV. Given the absence of long. fibers in the apex, the apex bulges giving rise to the outward motion of the E wave. The shoulder in the down stroke of the E wave is LV suction. Tony LaSala MD.
