I'm a nurse & work in insurance. The problem is the doctors who don't follow evidence based practice & do unnecessary procedures or try to upsell procedures. e.g. an implant vs bridge for a 60 year old with osteoporosis, chronic sinusitis and the provider tells them the implant is the better option, which it is not. Insurance can be a safeguard for many patients to ensure best practice based on supported evidence is being done.
