    So, I have a complete ACL tear and possible meniscus tear. I need to have total ACL reconstruction surgery. Orthopaedic surgeon has me in a TROM brace because my knee keeps "giving out" while walking with a store bought brace and crutches. My complaint is the very bottom joint, pressing against my shin just above my ankle is giving me unbelievable rubbing pain/pressure and is causing a blister after wearing just 2 days. Is there any way to loosen this particular part? I have loosened the velcro as much as possible but it still rubs. Could I put a cushion (like a baby sock) between it and my leg to prevent pain? I'm looking at 2 months AT LEAST before surgery.

      Thanks for the question. These are “off the shelf” braces and often require some adjustments. Adding a padded sock the way you have described may work and likely will not alter the function of the brace. If you obtained the brace from an orthotic shop, I would I reach out to them to help make any adjustments. Your surgeons office should be able to help you as well.
      Make sure that the hinges are right at the knee joint so that brace movement is minimized during knee motion.

    Thank you for explaining the exact use of the red lock. The instructions in the product were not so helpful in understanding when and how to use the range sliders vs. the lock function.

    My wife just had acl reconstruction surgery with bone patellar tendon bone autograft and her leg is completely straight her surgeon said to walk with the brace locked with full weight bearing how do you do do that? Whats the easiest way to get her need to bend?

    • @JeffreyWittyMD
      @JeffreyWittyMD  4 роки тому +2

      Sorry for the delayed response. If your wife just had ACL surgery and ONLY the ACL was reconstructed, we usually allow full weight on the leg and allow bending as much as possible right away. There may be some exceptions to this depending on specific things done during the surgery. Early after surgery the leg is often very weak and locking the brace in full extension allows support to keep the knee from buckling due to the quadriceps being weak after surgery. Using crutches early on with some swing through of the leg may make it easier and take some weight off for comfort during the early recovery. A therapist can give you the details.
      There isn't a "best" way to start bending the knee. What is important is that you start early and do it often. Feeling a stretch and then holding it without too much pain is ideal in my opinion. Avoid stretching only once and going very hard which will tend to make the knee sore and swell. I often tell patients to stretch 3 times in the morning, 3 times mid day, and 3 times in evening for 10 - 15 min. Volume over intensity here is often better. Check out the towel stretch for flexion video at this link:
      www.drjeffreywitty.com/knee-videos.html
      Also, don't forget stretching to get the knee all the way straight! As always, don't hesitate to discuss any questions with your surgeon.

    How too keep the knee brace doesn’t slide down when in the upright position?

    • @JeffreyWittyMD
      @JeffreyWittyMD  Рік тому +3

      Good morning,
      I assume you mean the brace sliding down toward your foot. This brace in the video is an "off the shelf" brace and not custom fit to a patient. The off the shelf brace are more readily available and do not cost as much as the custom braces. One of the downsides I have noticed is that there may not be a perfect fit and the patient will have to occasionally re-tighten the straps. Think of it like needing to pull your socks up from time to time through the day.
      Depending on the condition being treated, you may benefit from a custom brace. I would reach out to your physician to see if that is appropriate for you.

    Is this brace ever indicated for use as a splint for a high tibial fracture? My wife's doctor put her in one instead of a cast or splint, and it doesn't seem to be the proper device given that the strap below the knee goes over the fracture and the device itself doesn't seem to immobilize the tibia since only the ankle strap and the strap above the knee are above and below the fracture.

    • @JeffreyWittyMD
      @JeffreyWittyMD  4 роки тому +1

      Thanks for the question. Hinged knee braces can be used for certain tibia fractures. There are many variables that we must take into account when determining which device (cast, immobilizer, hinged brace etc) to use for non-operative treatment of the fracture. In general, for a fracture that meets non-operative criteria, a hinged knee brace could be used for what is called a tibial plateau fracture. A fracture further down from this area (tibia shaft) is more classically treated initially with a long leg cast (cast goes above the knee and down to foot) which can then be transitioned to a short leg cast. Much of the time, however, these injuries will be treated surgically. I encourage you to reach out to your physician with this question since the correct answer highly depends on a great many variables.

    *Question: I am a TKA revision patient. Should I use the brace in the night when I sleep?*

    • @JeffreyWittyMD
      @JeffreyWittyMD  5 років тому

      John K Lindgren
      Every surgeon has their own postop instructions. One of the reasons to use a brace at night during the initial postop period is to help maintain knee extension which can be a challenge to achieve if a flexion contracture (scar keeps knee from going completely straight) develops. This problem can alter they way you walk and cause problems. Please clarify with your surgeon any questions you may have.

      Much more information regarding your cousin's situation is required to answer this question. Hopefully you can reach out to an orthopaedic surgeon to help find some answers. If you are in the United States I can potentially provide you with some recommendation.
      The American Academy of Orthopaedic Surgeons is also a good source of information.

    my unlock switch is not working. any idea why?

    • @JeffreyWittyMD
      @JeffreyWittyMD  3 роки тому +2

      Sorry for late response. I am sure you figured it out but often the switch won't work if the bar of the brace isn't perfectly lined up. Try to rotate the bar slightly while using the switch.
