This is "You Raise Me Up" performed by The international Staff Band. This track features on the ISB's album entitled "Together", produced with Universal Music and released 24th November 2008. "Together was nominated in the Classical BRIT's NS&I Album of the Year 2009 category which included the top 10 selling classical albums of 2008." (information taken from their website)
This has become a very popular song over recent years both as a secular song and within the church repertoire. The words written by Brendan Graham are very powerful - I will let them speak for themselves:

Such a beautiful piece, a lasting memory of my mums final journey from the citadel at Sale, Manchester, to her final resting place, treasured forever, how proud were her family to have the Salvation Army Flag before her.

This was played as my mum was carried to her final place by her son and three sons-in-law, my mum and dad are big salvationists, I'm not a believer and neither are her sons-in-law, but this piece gave us a lot of comfort as she made her final journey
