I was so thinking the same thing. Doing the theory behind the treatment In my mind should be done seperately. I used to teach this stuff for almost a decade. Theorum is done in the classroom. Hands on practical is done to show the steps. The steps should begin with the initial assessment, identity the threats to the airway breathing and circulation. And another thing, Why is she holding C-spine? what was the mechanism of injury that made that call? He need to stop the air flow into the chest and assess whether or not there is an exit wound. There are a lot of steps here he is missing and or skipping. This frustrated me. bad class material..., Just sayin.

Wow that's a top-notch training aid dummy you got there! I've seen models that were limbless slabs, or arms or legs, and even complete bodies. But I haven't seen an electric one before that simulates breathing and everything. Very cool.

I wish they'd do one about the back. When I was shot in the back, the doctor insisted that they immobilize my back and neck, I'm guessing b/c I couldn't feel my legs. I couldn't feel either leg, even though all the bullets hit me on the opposite side of my back.
I was assuming it would affect my left leg only but everything below my waist went numb and thing into nothingness. I was temporarily paralyzed but got the feeling back later.

right that's why I'm watching this too. i don't think I'll ever need it, but I'd rather be safe than sorry. I'm also learning some other stuff such as treating stab wounds, seizures, alcohol poisoning etc which could also be useful
My friend told me about his dark past, his friend got shot by a gangbanger and he died in his arms on the way to hospital.
Watching this made me thinking how traumatized that would be.

Years ago I watched a medical drama where a person is injured and a medic gives instructions on how to bandage the wound over radio. They use the top of a balloon cut to a square and taped over the wound by the sides. The medic says to only tape two sides and not all four sides, but the person mishears this as to tape all four sides. Since I didn't watch the end of the program, I never understood why it seemed to important that at least two sides of the bandage were left untaped. Watching this first aid video finally gives me an explanation. Thank you.
The military teaches to just tape all 4 sides now. The difference/benefits is negligible and it's better to know that you have a seal. Pretty much all occlusive dressings now have one side that is fully adhesive that completely seals the wound.

The military is now teaching that in these situations, you need to use a 4 sided tape job to prevent any possibility of air build up in the lung cavity that could lead to a tension pneumothorax. If you need to burp the occlusive dressing, then let up once side of the dressing then reseal. That is the latest instruction from AMEDD (US Army Medical Department).
If you need to prevent a tension pneumothorax, or even a pneumothorax, then you can use and NCD or a 14 gauge needle in the third intercostal space, mid clavicular.
There are these valves at petsmart for fix tanks that will fit on the end of the catheter perfectly. They let air out but none in. Look in the tank section at your local pet store. (Thats just if you don't have the occlusive dressings with the flutter valve built in).

@@sandeepvashisht7755No removal of bullet. Most likely the bullet will have passed through and there will be an exit wound that is treated the same way. This is just to stabilize the patient so they can make it to the hospital. Anything in the chest or body just leave it there and let the surgeons handle it.
I like that he covered the info about doing what you have to do. Not alot of instructors put that out there.

He sounds like a police officer, knowing that much about the bullet. I remember learning about stippling (tattooing) around the entrance wound, and whether it goes in straight or at an angle, but I don’t recall what oscillation. It seems more important for police to know about whether it enters at an angle (that tells them the height of the shooter) or maybe the surgeon needs to know. I’m not sure why the medic needs to know. I haven’t done this in a while and I was the ECG Technician/paramedic. I worked in cardiac care (ACLS) most of the time.

but serious question, i know about sucking chest wounds and the plastic bag trick, but how do address both the sucking chest wound and the bleeding from the same wound at the same time?
Keep in mind: entrance wound probably indicates exit wound. Once you've placed the occlusive dressing/chest seal on the front, you need to check the back for exit wounds. Both are sealed the same way. Being worried about disturbing their head/neck orientation is lower on the priority list than bleeding and airway. Follow the MARCH sequence:
M- massive hemorrhaging (tourniquets)
A- airway (ensure breathing)
R- respiration (chest seals, observe)
C- circulation (is blood pumping?)
H- hypothermia (prevent shock, blanket)
Prep for evac and monitor.

My niece was shot by a stray bullet when her brother and mom threw her in the back and were driving to the hospital when she stopped breathing she saw a fire station and they ran to help her. She's alive today but has severe nerve damage. She's half paralyzed. She can get up and try to walk but falls because she has no control over her nerve.

I know this is a very old video, came across it though. One big thing we do NOT do is cut right through the gunshot hole in the shirt. That is evidence, cut starting at the bottom of the shirt at the tires all the way up to the neck and make cute you cut away(around if you have to)from the hole in the shirt. Then you can expose the entire chest check for flail chest, JVD, can see the neck better see if there is a deviated trachea indicating a pneumothorax(popped lung).

The description of pneumothorax and tension pneumothorax are so identical that I could not understand how to differentiate one another even after asking other instructors. Even the books don't provide a clear definition, but this old video just described it with ease: a tension pneumothorax is when the pleural cavity is perforated.
So this is where the rabbit hole of researching bullet wounds takes me...

Should I put that Occlusive Bandage on if the bullet wound goes straight down? I ask this because it sounds like you can only use this bandage if the bullet goes straight down
I can tell this man has seen some things.
With the fluid from the nose and ears do you still throw Chad over your shoulder, not knowing the severity of the head or neck injury?
If the patient has had a gunshot wound, he or she will be bleeding a LOT. How do you go on with occlusive bandaging if he’s bleeding everywhere? Wouldn’t the blood enter his lungs? What do we do then?
Dec 2019,Stephanie did not have both or even 1 elbow planted to implement C Spine, she was covering his ears and d not do jaw thrust. However by the time you were doing your head to toe, you would have instructed your helper on the correct way of doing C spine and as Chad, RIP, was unconscious, I'd put in a nose hose or an oral airway to keep the tongue from shutting off his air. Just in case the helper lets go the chin.Also I'd roll him up on his side and check for an exit wound, if found put another dressing there as well.You may need to wipe off the wound area to let adhesive stick.I know this is just a training film. I still learned some things and was reminded about others.Thumbs up.

That IS showing concern about the patient. If you have no idea what's happening to the patient, then you're of no use to the patient as anything other than a risk.
In order to treat them, you have to do that. They don't comment on every aspect of patient care. When you're teaching people how to do it, they have to understand exactly what's going on and why they're taking the steps which also makes remembering them in the future easy to do.

At the end, when he speaks about putting a person w/ potential neck injuries over your back if you have to. Why not use something like your shirt for neck support? Something to add stability as you move them. Is that worthwhile or what? Have never experienced this sort of thing.
