Do not resuscitate (DNR) vs do not treat: there IS a difference.

  • Do not resuscitate means that in the event of the heart stopping (cardiac arrest) the patient is not resuscitated with CPR. If someone is DNR, this does not automatically mean treatment is withheld. They may still undergo treatment for medical conditions. It ONLY means they don't want resuscitation attempted in the event of cardiac death.

    Perhaps this should be taught to healthcare professionals because I have actually seen hospitals stop IV antibiotics after they convinced a patient to sign a DNR.

    But if DNR in place, docs will not perform certain procedures even outcomes seem favorable. If heart stops during a procedure docs need to be able to shock it back. Also, hospitals are pushing DNRs even if have Do Resuscitate in place, can do this in hospital bed without family, pcp, or clergy present while patient is in pain & on pain meds, this is not respecting wishes.

      The decision to perform a procedure is separate from a DNR. In the United States, a DNR simply means that if a patient loses their heartbeat, they do not want resuscitation efforts. Typically if patient is undergoing a major procedure, the surgical team may ask the patient if they want the DNR reversed during the procedure in the event of complications. In the US, a DNR can only be placed by patient or their medical power of attorney/next of kin. If the hospital is not able to find family and the patient cannot speak for themselves, then the default is full resuscitation. I am not sure if this is different in different countries, but this is how DNR works in the United States.

      I have a DNR and a DNI (do not intubate order) in my home state AND he the state I vacation in. She's right.
      DNR only means "do not resucitate". In most states, the order is ONLY valid if issued in that state. In some states, the order covers ONLY heartbeat. In some states, the order covers heartbeat AND brainwaves.
      If you want something more specific, you likely need a living will. Otherwise, EMS and your doctor will transfer those decisions to your next of kin, and you will not be able to speak for yourself if they decide to violate your wishes.
      If you suspect that your next of kin will disobey your desires, make sure to give your living will to the local hospitals you will be most likely visit upon a medocal emergency.
      Some states require your doctor to sign off voluntarily. Some require your doctor to sign when you ask. Some place specific restrictions that must be met before the doctor CAN sign off. Some require that EMS or doctors respect specific brands of EMSA bracelets.
      Basically... if you want specific responses from a doctor or hospital, those need to be stated clearly in the fashion accepted by your state's department of health. And those ALWAYS need to be in place BEFORE you are in a medical emergency. Otherwise, your dlctor will act in your "best interest" without your input, or rely on your next of kin or lawyers without such a legal order.
      TL;DR: if you have specific answers to how you want to be treated, get a legally binding living will while your brain is legally in good health.
