How To Fix Lower Back Pain From A Sacroiliac Joint Problem | El Paso Manual Physical Therapy

  • Do you have back pain coming from a sacroiliac joint problem? It's a joint, otherwise known as the SI joint, it's all the way down, kind of like where the hips or the butt meet the lower back. If you have this type of problem, it's hard to fully know unless you've been seen by somebody who can diagnose SI joint problem, or sacroiliac joint problem.
    But one of the best ways that you can tell on your own at home, is if your back pain is really low, like not necessarily up here or higher. But kind of down here, right above the butt area or kind of almost at the top of the buttock area. Especially if it's off to just one side, it tends to be a sacroiliac joint problem. The sacroiliac joint is where the sacrum, the tailbone, and the pelvis bones on the side meet, and there is some big joints there.
    It's in the medical field a big controversy over whether they move or not, and how to treat them. There are all these different approaches. And I can tell you from experience and working with patients with these back-pain problems, they're very real. And when we treat them, they definitely get better. And if you've got one, if you're out there watching this video right now, and you think that you have a sacroiliac joint problem, and you've seen people, medical professionals that have told you that it's not that it's something else. It might make you feel crazy, it might make you feel like you're making stuff up or like the pains in your head and nobody wants to believe you.

    I had injections in both my SI joints for years and finally had my right SI joint fused. I also have spinal stenosis and scoliosis so that could have contributed. I'm working on getting my glutes and abs strong so I can continue walking.

    My SI joint is flared up bad. I can’t get in or out of bed. Should I apply ice or heat or both.

    Doctor, I have that SI pain and sometimes can't straighten up in the morning for a couple of hours! I want to try these exercises. However, I also have arthritis in my vertebrae (I've had a laminectomy and spinal fusion on L4-5), in my hips and knees. I don't want to cause a problem in other areas. The knee exercises I used to do, are not recommended for hip problems. Are these SI exercises okay if I have other problems? thank you!

    Is this safe to do if I have been diagnosed with sacroiliac pain due to a hypermobile joint? Not a stick or fixed si joint, but hypermobile. I have pain on both sides. I have a pelvis that is impaired and tipped forward, and am working on that with PT as well.

      Hey Anita, great question! Hypermobility of the SIJ needs to mostly be treated through bracing and muscular activation. Using the glute muscles to help stabilize the area would be beneficial. Hope this helps!

    Hey doctor, am back
    Not long ago you helped me confirmed that i had a si joint dysfunction
    And i don't my physical therapist is showing me any further difference
    Seem like the physical therapy exercises are not really helping me. Seem like the exercises he taught me was for more back issue than for so joint dysfunction. And he even said i felt like i wasted my time he taught me exercises that wasn't really specifically for si joint.
    Anyways
    Recently i have tried a physical therapy sample for si joint stability from youtube
    Exercise like
    Clamp shell with resistances band
    Sideway abduction
    Bridges with resistance band
    And it seem to make a difference
    So starts a good start and a sign that the right therapy applied will helps me go back to my martial sports and strength and conditioning training.
    So may i know if you guys have a rehab programee for si joint exercise to return back safely to my martial art actitvies and my strength & conditioning training?

      If i'm not wrong you told me it takes 6 to 12 months, so i'm confused.
      My injury has been exactly 1 year and i guess my phyiscal therapist didn't provide the right exercises
      As the 3 exercises i'm currently doing was in his rountine at all.

      But how do we know we are completely recovered and ready to stop the 28 day programme. So that we can resume my competitive sports?
      "Also" this clicking or popping sound that happens time to time is it something we should ignore it, is something to worry about or will it go away after this programme or should just ignore it?

    I have a very curious question
    Can one have an si joint dyfunction or si joint stability? Without any physical examination test sign being shown positive?
    Because i have all the symptoms of an si joint dysfunction.
    I have aching around my si joint, i have aching around glutes medius, and if i sit down for too long aching can increase
    If i increase my daily activities, the aching will increase.
    Even from time to time my do feel clicks/pops in my si joint area
    If i jerk and skip on my right leg, which is my affected side i feel like an almost sprain feeling. When i do "Child's pose" stretches i feel tightness and stretched feeling around that si joint area.
    But my physical therapist did si joint dysfunction tests on me, it showed no sign of positiveness
    So i'm very confused
    Can i deteremine i have si joint dysfunction, without relying on the physical examination test since all the symptoms above i mentioned links directly to si joint dysfuntion.

      Hey Blue, yes it is possible to still have an SI joint dysfunction in your case. The reason is that SI joint testing is highly unreliable in healthcare professionals. No disrespect to your physical therapist, as we know personally how much advanced, post-doctorate training it takes to consistently and accurately find and treat SI joint dysfunctions.
      But from what you're saying, you might have a loose SI joint which can present as normal in testing since testing is focused on stuck SI joints. There isn't much medical research on SI joints in general and there's even less on loose SI joints. It's all focused on stuck sacroiliac joints.
      It sound's like you've already diagnosed your problem. The next step would be fixing it!
      The best first place to start for this is strengthening your glutes and abs.

      Hey dr david you are a life saver!
      Thank you so much for helping me diagnose my condition
      That's why i was also very certain that it was and i kept telling my physical therapist for 11 months.
      Because i feel aching around my si joint BUT TO BE SPECIFIC a bit lower more towards my "BUTT" and aching around my glutes. And the one that really made me sure was due to my clicking and popping sound
      I mean clicking and popping is when we have instability due to loose ligament, aren't i right?
      My physical therapist was confused and he even thought maybe it could be cartilage damage, but i doubt it is that right?
      Anyways yes i'll check on your website for further treatment. I hope i'm not too late.

      There’s only 1 way to find out if it’s too late! Starting to fix the muscle imbalances will help normalize the pressures on the cartilage and ligaments so that they might heal.
      Be aware that it is a slow and long process (6 to +12 months) to fix a loose SI joint naturally. But it’s better than the alternative! Success rates for SI joint surgery is not very good.

      Sorry dr david, i don't get it when you said that "there's only 1 way to find"?
      Though it's been 11 months i had this injury
      I have been doing physical therapy for 5 months, but i guess it was "NOT" the right physicaly therapy exercise.
      Though it has some improvement, it isn't full healed yet.
      I have no problem wigh simple things like walking, climbing the stairs, it's just that sitting for longer than 20-30mins it starts to ache even more, certain jerking movement with 1 leg makes feel like my si joint area is gonna sprain.
      Or if there's too much daily activity.
      From this update about condition, i'm not too late right?
      My physical therapy himself is confused if it is si joint dyfunction or not. It was really confusing and frustrating.
      But thanks to you i have confirmed my diagnosis.

      @@Blue_Tsuki it definitely sounds like you need to dramatically increase your glute Maximus strength. That muscle directly stabilizes the si joint. More strength there should keep the joints more stable when sitting for any period of time and when being active. When that muscle this week the pressure of your bottom on the seat starts to shift the loose joint and can cause it to go into dysfunction. If you are too active with not enough gluteus maximus strength then the body tends to compensate with other muscles to be able to tolerate your activity. The other muscles could pull your SI joint into dysfunction as well.
      Does that make sense?
