There are various reasons for folliculitis and it can be difficult to manage at times. All the treatments you have mentioned seem very reasonable. We may need to consider pityrosporum folliculitis as a possible diagnosis too. Yes, there is a possibility of recurrence of hair loss once stopped. It may need to be continued long term. There is no good data on this at present.
