- A 13-year-old girl with a severe dairy allergy died after drinking a Costa Coffee hot chocolate following a "failure to follow the processes in place to discuss allergies", an inquest has concluded.
Hannah Jacobs, who had been severely allergic to dairy, fish and eggs since she was a toddler, died within hours of having a sip of the drink that contained cow's milk.
Lawyers for the family said allergy training was "not taken seriously enough".

If I was severely allergic to dairy to the point it could literally kill me, I just wouldn't go to coffee shops that serve hundreds of drinks a day with milk. I'd also make sure I have medication on me.
Same. In fact I would not be eating at any restaurants at all and always carry my epipen. And if I felt any signs or symptoms of anaphylaxis I would seek proper medical attention IMMEDIATELY.
If I was about to go to the dentist I wouldnt have a hot chocolate.

It happens regularly where a busy coffee shop might use the wrong milk, it happens allot maybe every 30 coffees I have in a month they get 1 wrong. But why would u eat or drink out without an epi pen if ur allergic to dairy and nuts! Hot chocolate powder usually has traces of both.
And then the chemist didn’t have the right epi pen! This is a terrible thing but blaming Costa isn’t exactly fair, the blame is obvious when it boils down

Drinking chocolate is simply a mix of Cocoa and Dried Milk.
People with severe allergies ALWAYS KEEP THEIR EPIPEN ON THEM!
That doesn't mean that the person will survive this kind of ignorant comment really doesn't help the situation. What's better? Is people not being given stuff that kills them in the first place
Sometimes Epipen isn't enough. Some people keep 2 with them in case one doesn't work. Usually 2 will work but depending on how bad it is and the circumstances, people can still die even when they have their Epipen. Nothing is 100% guaranteed.

Something I do not understand when I hear of these tragic events is why someone who has a life-threatening allergy would even consider consuming something not prepared by themselves. It is illogical. It is LITERALLY like walking through a den of snakes and expecting not to get bitten. Personal responsibility is the prime consideration in a case like this.

She was allergic to the protein in cows milk not the soya milk which is rare to have an allergy to

@roseanncampbell3168 the chocolate powder used at Costa may have contained milk in it. So really the ingredients should have been read thoroughly. Some brand of chocolate powders contain milk whilst others do not.

@@CappellanoWith regards to Costa Hot Chocolate Powder. Costa states that it's not suitable for consumption by those with dairy allergies due to the risk of cross-contamination with milk during production.
Even if a request to make up a drink using soya is successfully communicated to the barista, a sizable risk still remains. This is presented as stated in the 'allergen bible' available to Costa staff and customers alike.
There was a breakdown in communication and both parties were at fault IMO.
This is reflected in recommendations of the report.

They didn't follow the allergens protocol.
Very sad and regrettable. My view is that with such a dangerous allergy people should be prescribed pharma grade food. Don't think it's fair to put such a burden on food companies and workers.

I know that Costa staff screwed up monumentally but If I had a family member or me who had an allergy that severe to something I've got an epi-pen on me 24/7 and so have they. Why not carry one just encase something like this happens. Its so sad and stupid that she died from something that was totally unavoidable by both parties.

If my daughter has an allergy that is strong, I would never live my house without the medication to react to that!!!!People can make a mistake but I'm shocked about the mother to not be ready for that kind of situation.

Usual stuff. Let’s take one side and rant at the other without knowing all the facts. Both mum and Costa stuffed up. Mum was her daughter’s first defence and probably didn’t take enough precautions. Costa had an obligation and failed.

Look I get the lack of sympathy for a little black girl but the family survived 13 years doing what they do. Clearly based upon the coroner's report and Costa's response there was a cock up at the store. UK comments section showing their true colours again. Disgusting.
The Costa should’ve had their own stock of Epipen as many other food and drink businesses should as part of health and safety, also it would’ve been best to write down the order or put it on the cup and employees should ask if someone has any food or drink allergies. This is a wake up call to all so this sort of thing never happens again.
Alas, Catering Establisments don't even have a duty of care to employees - let alone any customers- with regard to access to epiPens.
Also, the onus is not on the service provider to 'ask', it is on the individual customer to 'inform'.
Not that any change in legislation would have helped in this case. The cause was 'lack of comnunication' according to the inquest.
