Hey Dr. Gallagher, I have a question. About MTF bottom surgery, you mention in an earlier video and just with just common knowledge that hormones are asked to be stopped before surgery (1 week before and after surgery?) But isn't that mostly only based on old premarin and non bioidentical estrogen studies, no study has been shown that any modern bioidentical, especially injectables like estradiol valerate, have any risk of blood clotting? Or for recent studies, they don't differentiate between delivery methods and whether the patient was on spironolactone as well, a blood pressure medication. It would make sense that injectables have no increased cardiovascular risk compared to cis counterparts especially, as FTMs have been shown to not be at anymore risk than biological males, and most FTMs use patches and/or injections, but MTFs who prefer pills do somewhat have higher risk factors along with the extremely common spironolactone. I ask because you've said in your post OP depression video, that hormones being stopped is a big potential cause of post OP depression, so it would seem to cause a patient a lot of potentially unneeded issues at a point where they already are mentally and physically recovering from such a big surgery. Messing with hormones seems MORE detrimental not better just to save the 0.5% POSSIBLE chance that you might have a blood clot based on outdated and inconclusive studies.
I asked this on the peritoneal surgery video, but you may not have seen it. Is there a combination of penile inversion and peritoneal? does penis inversion still occur with peritoneal surgery? Is scrotal skin still used? I am looking to get my surgery soon and want to make sure that I am informed and it's kind of confusing when the differences are not very clear.
