So,uh,yeah, EP12's ending gave me the feels.... gosh, I love this anime. Mirai and Aki are both damaged individuals trying to love each other, and the anime is all the more sweeter for that.

If you're talking about the anime's conclusion....
well, she disintegrates and vanishes after they defeat the Kyoukai no Kanata, but not before saying farewell to Aki.
As she's crumbling away, he manages to grab hold of her power limiter ring. 3 months later, Akihito is still devastated and feeling destitute, and he's staring at the ring one evening, when the ring suddenly disappears before his eyes. He realizes what's going on and runs up to the school roof where they first met in EP1. She's there, her back turned to him. Hearing his footsteps, she turns around and smiles, and he hands her the red glasses she always wears. The anime ends.
