    If at my country the foundation year is only 1 year ... can i get directly into core training ??? After filling the crest form back in my country????

      If you completed all the things mentioned in the CREST form while working as a full registered doctor in your own country and obtained (or expect to obtain) full GMC registration, yes you can apply for core training directly.
      You CAN NOT get CREST signed with your internship work. You have to work as a full registered doctor for at least three months under that consultant in the last 3.5 years from the start date of the training post you are intending to apply for.

      You will need to obtain the alternative core competency certificate first. You can obtain that if you have done the equivalent work back home, otherwise you'll need to work in a non training post in the UK first.

    Salam, I have a question, more of a misunderstanding perhaps? So if I complete my Core Training and want to leave it at that, would I have any sort of certification or does having completed just the core medical training and MRCP is of absolutely NO VALUE in wanting to practice as an 'Internal Medicine' consultant in a hospital?
    Another query, more of an extrapolation of previous question actually, is that after you do 3 years of residency in the US, you are a hospitalist (consultant), i.e you work on the medical floors, maybe in ICU's depending on what kind of hospital are you in and see outdoor patients all related to the subject field of 'Internal Medicine'. How can we be exactly this kind of consultant in England?
    Last one: Can you please enlist or direct me to a list of fields that fall under run-through training and the ones that fall under uncoupled training?

      Walaikum assalaam. A consultant in which hospital? It would be nothing in the UK. You would be at a registrar level. Internal medicine is not a specialty in the UK. We have a video covering run through vs uncoupled.

    If you have already taken the membership exams foreignly and completed non-training jobs in the UK, will you be awarded the fellowship of the corresponding specialist college? Or you just have the certificate of core competencies?

      Fellowship of royal college of physicians is by nominations - you have to be a specialist in your field to have that. Fellowship of most other royal colleges has exams to pass.

      As mentioned in the alternate pathway, you will have to invest time on getting your alternative core competencies and then higher specialty training. The higher specialty training is usually 4 years. Now the time required for you to get your core competencies is up to you.

      Yes, if you are able to cover everything in the CREST form, it can be signed outside of the UK.

    Did I understand it correctly in the video that you can enter the UK and work as a locum FY2 (non-training) to get the CREST form signed and as soon as you have the CREST form signed you can apply for Core Medical Training even though you just worked on a service post (locum) and not a training post in the UK?

      You need to be in a substantive (not locum) post, but yes, by having CREST signed and meeting all other criteria for a training program, one would be able to apply for the training of their choice in the UK.

    Is it very difficult to get an Alternate Certificate of Core Competence? How long can it take for a person who has cleared MRCP and has just started working in the UK? Appreciate all ur efforts in helping us understand 🙏

      It's impossible to give you a timeframe.
      One doctor might be really proactive and can gather evidences or convince their signatory consultant that they are competent in a six months time, another doctor might be working for years and years and still fail to grow that confidence and trust in their clinical work.
      So the alternative pathway is not structured and completely depends on the person's own capabilities.

    Hello sir, I am from Bangladesh currently studying in Bangkok. I am going to give my IB(alternate to a levels) in 2025. After that i am deciding to study medicine in UK. In every uni such as UCL, imperial, KCL the MBBS program is for 6 years. Just to be clarified after MBBS i will have to do foundation training and then speciality training right? Will universities such as UCL directly enroll me in the program or i have to enroll myself in but they are affiliated with NHS. Please reply thank you

    1st question- Can we apply for both run through and uncoupled training programs and choose the one pathway that we get in?
    2nd question- For the acute care common stem, it is mentioned in the website that it is 4 years of training. 1st 2years in ACCS subject and last 2 years in IMT training and that can be field of our choice? Please correct me if I'm wrong?
    Also please tell me difference in Acute general internal medicine training for both ACCS and IMT pathway?

    What about SHO ? I thought it was necessary for IMGs to complete FY2 and SHO if we are planning to get into IMT program. Can you clarify this for me?

      Senior House Officer (SHO) is a post used in previous nomenclature which in terms of modern nomenclature can be FY2 level or CT1/ST1 level. As you can see in the video I have mentioned about FY2 level non-training jobs for IMGs to get the CREST from signed which makes them eligible to apply for any run-through or uncoupled specialty w/o having to go through any foundation training.

    Hello Dr. I am a doctor from the Philippines . I am already a board certified internist since 2018 here in the Philippines . I am planning to migrate and work in the UK . I am on the process of completing MRCP UK. Eventually I wanted to specializein Gastroenterology . What do you think is the best pathway for me after the MRCP? Should I start from CT1 so that I can have a reintroduction/reintegration of UK based approach to management in internal medicine .or can I just apply straight to a ST3 position for gastroenterology?

      It's up to you- you will need CREST signed if you choose to go for IMT, or the alternative core competencies if you want to start with ST. So go through those forms and see what would be possible for you.

    Hello doc, i have done my fcps anesthesia pak and its a gmc approved qualification so after IELTS n gmc registration i will get enter into ST3 speciality training or will enter into core training St1 plz tell.

      To enter into ST3, you'd need the alternative certificate of competencies signed to show that you have ST1 & ST2 level competencies. In order to achieve that, you'll have to work in the UK in a non training capacity for some time first.

      @Atheist yes correct, but in order to enter training and becoming a consultant u ll need to give part 1 frca even though u have ediac and frca part 2.

    It is very confusing. Can you please guide if a person has done 4 years of training in their own country in OBGYN (Pakistan in my case) and they have done MRCOG then how would I enter a training program in the UK or what would be the type of training? Also, can you please elaborate, if I do MRCOG 1+2 and do 4 years of training in my country then join for a job or training in the UK followed by applying for mrcog 3 exam, is that applicable? And if I would be able to join at st3/st4 level with that?       To get GMC registration (before anything), you need to either complete MRCOG or pass PLAB. After that, when application rounds open, if you are eligible to apply for OB/GYN ST1 (according to the person specification- which will require a signed CREST form), then you can apply.     Thank you very much for this explanation. This is very helpful. Let's say I want to work abroad after being GMC registered specialist, it it possible? Because I know with plab I can only work in Australia, NZ, and the Middle East (EU countries aren't included)

    how about competitiveness in applying for CST /CMT, does IMGs will have less chance to get?

      The application is equal for everyone without any visa restriction now. So if an IMG is eligible and do well in the recruitment process - they can get in any specialty. Please check this article: roadtouk.com/blog/specialty-training-opportunities-in-uk/
