In this video we are going to answer 2 questions.
1. When to do a semen culture and sensitivity?
2. When to NOT do a semen culture and sensitivity?
The seminal fluid is made up of a combination of testicular, prostate ,seminal vesicles and a small contribution from epidydimal secretions. Now because semen also comes out of the SAME penile urethra as the urine, when a semen culture and sensitivity is done, you would expect to see some bacterial growth anyway.
A large number of fertility specialists routinely recommend semen culture and sensitivity reports to male patients when they visit the doctor. This is absolutely not recommended. Professional societies and bodies have never ever recommended a routine semen culture and sensitivity testing for all men. A simple semen analysis is all that is required in most patients.
We also do not know the exact link between the bacteria in semen and its implications on sperm quality. Therefore routine testing of men with semen culture and sensitivity only adds to the burden of infertility and increases the patients stress.
Moreover, patients are also put on antibiotic therapy immediately after semen culture and sensitivity. This is also not recommended since, antibiotics in itself can drastically reduce sperm motility.
So, when to actually do a semen culture and sensitivity?
In specific instances where there is blood in the semen, burning pain on ejaculation or a sexual exposure, semen culture and sensitivity can help pick up probable infections. Only in these instances is treatment actually necessary. Of course further testing with more sensitive tests would also be required in addition to semen culture and sensitivity.

Dear sir, my husband has ejaculated red semen last two month ,he take higher antibiotics,( but not cure) then he take semen culture test, with another two test,as doctor advice.....

It depends on how the microbiologist reports the finding because for example e.coli isnt a normal flora present in the genitourinary tract... kindly clarify this.
I agree, if e coli is detected and if the patient also has clinical symptoms. Then I would definitely advocate a treatment based on antibiotics susceptibility testing. But not routinely without any clinical symptoms.
Dr,my semen viscocity is increased and abt last 1 month my counts are low and all are dead.i done a semen culture in my lab and insignificant growth of gram positive organism seen.i have no other clinical symptoms .can i take antibiotics?

Dr. My urine culture report came out to be E.coli bacteria: 10000 colonies/ml. As my symptoms are very similar to prostatitis, I usually have dysuria and burning at the lower part of scrotum and perennium after ejaculation which subside in 2/3 days, he told me to do a semen culture. Is it helpful to diagnose prostatitis? I am on antibiotics ( levofloxacin) and alpha blockers. I already took antibiotics a month ago, first cefixime for 8 days and then nitrofurantoin with pain killers but the condition seems to return after I ejaculate.
