Yes, this song is about the 1988 air show disaster at the Ramstein (with one ‘m’) USAF base in then-West Germany that took 70 lives and injured around 500, and the lyrics refer to burning men, a sea of flames, screaming mothers etc. The flaming coat is a quite a literal symbol of that.
They had variations of this effect right from the beginning, in the tour that came right after this show they "enhanced" it up a little, Till's flames would get out of control and he would drop to the ground, Flake then would come up with an extinguisher only to turn out to be a flamethrower and make it worse, Till would stay in the ground a moment only to then stand up later and keep singing. They quickly retired this effect because it frightened the audience. The effect was used for the last time in Japan in 2005 because the venue was very small for the new effect for the song, by then they already had changed it, Till would enter the stage wearing some kind of exoskeleton arms with bearclaws at the end and shoot fire up from his arms, this exoskeleton effect would end up being repurposed as the wings for Engel, and you can still see one of the bearclaws in Till's arm.

At that time, I worked in an intensive care unit for severely burned patients and, almost without a break, spent 4 days and nights with my colleagues caring for the explosion victims just to cope with the first onslaught.
I was on their second show ever in Prague 95. It was very small venue, but Til has already few pyro things. Like flame thrower which he fired over the audience. It was scary to jump when the fire was 2m above us. We smell bad from gas all evening. The were supporting Clawfinger btw.
I saw Rammstein two times in 1994/95 in Dresden. They were dressed exactly as in the video so it must be from that tour. By the way, the second concert was support for Project Pitchfolk.

There are interesting details about why they became the way they are today.
There are a couple of 1996/1997 versions of "Rammstein" available for viewing here on UA-cam featuring Till wearing the flaming coat that are MUCH MORE INSANE than this one. Been loving your reactions! I worked as the west coast show handler when they first came to the states in the 90s...such nice guys and so hard working. Til got burned alot back then. He would just shrug and say it's his job. I know someone mentioned it in passing, but if you were anywhere near the stage in those early small venue gigs, you would lose your hair/eyelashes... notice their hair is slicked back, looking gray...its Desitin, the baby diaper rash cream, also fire retardant! I had really long hair back then...it was hard to get washed out, but worth it unless you love Manson's no eyebrows look!
Michal Afrin (INTF/Agent) had been made aware of Rammstein by Bob Biggs of "Slash Records" because they were looking for an agent for the group. He went to a concert in a small club that didn't really convince him that Rammstein could be successful in America and he wanted to get out of the whole thing. But Bob Biggs didn't let go and so he went to a concert of Rammstein at the Palladium in Los Angeles that started with the opener "Rammstein". After seeing that, he said on the DVD "In America", Rammstein came out, Til had his arms wide open, in a metal jacket completely on fire and he said to his companion that what he had seen was so real, so authentic, that America would eat it up and he would be there !
ok, can't really help you Travis with the "FIRST" ever pyro's, BUT, the incident which caused Till to get his own Certificate of Pyrotechnics so the band could perform more safely than it had previously happened in 1996. There's a video of it, it's blurry and not to clear, but look up "Rammstein Concert accident in Berlin in 1996", the apparatus fell on the audience, and you can see even though the band kept playing, you can see Till down there in the audience helping out.

That day was so damn hot, they wait till the sun goes down to start the show, I still remember there a rocket fly over my head to the middle of the crowd. Totally stunned and still feeling amazed after all these years!! Top!

You have just seen an old pyro performance, it is from 1996, two years after it was founded. It is simple and consists of only one asbestos coat, because the funds were missing. Pyros were there from the start.

I've talked to a former roadie of Rammstein (Tours 2013/16/17). He told me even in festivals, when they has to share the stage with other bands, Rammstein put 30 tons of equipment to the stage and under the roof. The roadie said: "It was so unfair" :-) And backstage.....all the hands are literaly RUNNNING during the whole concert, to make all this happen.
That's how they used OPEN their show! 4 of the 6 times I saw them that was the show started. The first time most of their pyro effects had been confiscated by the fire department on a previous stop on the tour. The 6th time, it was held off for the end of the show. Where they staged it to look as if it went wrong. So they replaced it with the flame throwing backpack and a chance for Till to shoot flames at Flake in a giant calderon.
Ah I miss the Big Day Out - I still think it was one of our best festivals here. There was always something for everyone at them even if they were a bit of a logistical mess at the beginning, by the time I went to my first one in 96 it was well organised with good vibes. I remember when this came out. I feel old as dirt now haha
I have seen Rammstein as a support act at a small venue. Can't remember the main band or the exact year, though. Must have been 1994 I think. They had a crazy amount of pyro technics even back then. But you weren't allowed to bring a camera and smart phones didn't exist. So I don't think that there's a video of this show.
