Kojima's view was already accomplished with the game, or so his many interviews have stated. It seems the game was a shell of what MGS used to be, we had no over the top villains, long winded speeches, no boss battles.. we all felt MGS's existence in the game, but it was missing. Just like a.... Phantom Pain. Directed by Hideo Kojima.
The actual ending is a copy paste repeat of the tutorial mission that can't be bothered to abbreviate the boring parts or even get rid of the button cues. And then you get a cutscene showing that the entire game up to this point was a slap in your face. So you're left thinking Right. ok... so what was Big Boss doing? Can we play his downfall now? And then the game ends.

Big Boss never turned "bad", Miller tried to turn Fake Boss into some bitter old man like himself but depending on your choices throughout the game, you could either be a "demon" or remain true to Big Boss. The end dialogue via the tape he had of Big Boss, they seemed to part on good terms, and then a flash forward occured where Fake Boss took control of the Outer Heaven which Big Boss had built, and was told about the Operation Intrude N312 / N313. There was a time skip there so we dont know if Fake Boss had become a demon or not but at this point in the scene he smashes the mirror as gunfire sounds in the background, possibly where he was getting ready to fight Solid Snake after his plans have started to fail. "Man Who Sold The World" was simply a reference to the song that was playing in the hospital to help him jog his memory of what actually happened, which lyrics also state that he saw his doppelganger. The song itself was a cover version since the original version was David Bowie rather than Midge Ure, which is another hint towards you being a copy.
Seriously... This SOOOOOO should've happened, I don't care that it's a Bear replacement, there still should've been a freakin' Boss Fight with him. He served in SAS alongside Zero after slumming as an assassin for most of his life and was eventually made into Snake's unseen backup during MGS3, actively on the field with an XOF squad. Ocelot said he was sometimes clean up, sometimes a mole, and sometimes backup, taking out soldiers before they could reach Snake. Skull Face himself doesn't need to be on par with the protagonist to make for a great boss fight. He has the skills of a soldier, the added ability that his skin can't feel pain, and has a really strong old school gun.
The point was for it to be anticlimactic. Revenge is pointless and that's why it's so awkward when Huey kills skull face, its because Hideo wanted the player to know that what they were doing was destroying something to replace pain with hatred but as skull face says "it never goes away. it makes a man hideous, inside and out."
