    I cared for my friend who is HIV positive, she had a baby and breast fed the baby exclusively for 3 months. She stopped at 3 months because her supply was becoming low and so she needed to supplement with formula and we didn't want to take the risk. He baby is 10yrs old, HIV negative and very healthy. You got thus Nozi

    I am a mom of two living with HIV and I exclusively breastfed my kids, my first born I did for 1yr6mnths and the second one for 6 months and none of them is positive, my heart is aching with all that you have said coz I can see and feel how all this affected you mentally and emotionally... I am so sorry

    As a Dietitian I cringed so much hearing the breastfeeding discussion yoh ha a we have a nutritional therapeutic program at Government institutions. Infants can get formula but the entry criteria to receive formula as part of the PMTCT-program states that "Mother has been on second/ third line ART for atleast 3 months and has a viral load above 1000". If you a mom that don't meet the entry criteria we advise exclusive breastfeeding for the first 6 months "breast is best" and explain benefits etc. However I believe the choice is still in the mothers hands all we have to do as HCP is guiding the mom in making the best choice for baby and herself.
    Having a c-section don't necessarily mean that you have less milk cause I had one and my milk supply was absolutely fine. And your research on mix feeding that's not allowed is on point cause it increases the risk of transmission due to the immune system; etc just as you've explained Nozi.
    Milk supply is a give and take. The more you breastfeed on demand is the more your body will supply to meet the high demand. Natural birth/ C-section if you don't feed on demand you'll have a low supply. There's obviously also certain foods that can help increase supply. I feel like the Dr shouldn't have went into the breastfeeding discussion so deep and should have rather refered you to a lactation consultant/ a Dietitian for further advise.
    My birth plan was thoroughly discussed with me; the pros and cons of both options were listed and the choice was left in my hands and I feel like that should be the standard whether you negative/ positive it shouldn't be any different. As a patient you have a right to make an informed choice. I'm so sorry about what you went through Sis Nozi. As HCP we have a duty to do no harm so why would the Dr even think that breastfeeding at Government level is advised cause it's cheaper what about the little one that could be at risk if that selfish act was practiced it would be a huge ethical dilemma.

      Hi Phaphama, i agree with your statements and id advise Nozi to get even more info. For myselft working at a PMTCT program, we heavily advise the mothers to breastfeed until 6 months. Again please look for mentor mothers from mothers2mothers, they will enrich with so much information that you need, specially because they are women whom been through the process.

    Also, Nozi, one thing I learned after going to a Gynae I didn’t like for two years is that you don’t owe any medical practitioner your loyalty ❤ You owe yourself peace of mind.

      I don't know how best to clap for this. If you are not happy with the medical practitioner change them. Your doctor is supposed to make you feel comfortable and supported always not bashing you and talking about your resources. It's not your resources that is mothering it is you.

    I am genuinely sorry, Nozi. It’s genuinely disappointing that there are still healthcare practitioners who approach HIV in this manner. One expects them to know better. I also understand the concerns this could raise to Skhu, and anxiety that could’ve been avoided. If it’s possible, please change that Dr. I doubt there’s anything you can do to make him approach HIV differently, and you need a Dr who is going to make this journey easy where possible. All the best!

    I got the shock of my life when I started working in a Private Hospital last year especially coming for Public hospital, having the knowledge i have regarding PMTCT. Birthing options and PMTCT is very much disregarded in the Private hospitals it somehow gave me the impression that they are not educated enough in that area and are not even willing to listen to patients or even what questions they have which is sad. Private hospitals are companies what's important to them is money and c-section is quite expensive that natural birth. There's so much research that has been done by the WHO regarding PMTCT there's nothing wrong with natural birth or exclusive breastfeeding. May God give you the strength in all this sis and lead you to people who will give you the right information and thus make this experience pleasant for you.

    Yhuuuu!! I'm fuming as I'm listening to this 😡😡. As a Midwife, I agree that there's everything wrong with how the Dr said what he said. Plus according to the studies I've had an encounter with Breast feeding is actually very safe for the baby especially if the mom is suppressed. I mean, that's one of the reasons we give Nevirapine🤷🏽‍♀️. With the mode of delivery,bro! ! There's a number of HIV positive woman who went for normal vaginal delivery and their babies are well. Yes there's more chances of cross infection in comparison to C/S but they are slim.... Maybe the Dr is intimidated nguwe, your social status and all, that's why AGGRESSIVE 🙄🙄.. UbeRyt cc, whatever you guys decide on I support it and I pray you deliver a healthy GT❤️

They are breaking people's spirits. I mean a G4P3 will most likely have a smooth normal vaginal delivery, no tears or nothing... so nje benza imali without putting the patient first.

      Yoh sis Nozi❤ I’m sorry that he spoke to you that way. This video broke my heart because so much of what he said is not factual at all. It was not right. My advice to you is please also consider seeing a midwife for a second opinion and to have someone to advocate for you. I know in the private sector, at least in my region, there is a lack of knowledge and interest in HIV management in general, so it’s possible he’s not well versed in it and his fear spoke for him.
      With consulting a midwife, even if you’re doing CSection, you’ll have the benefit of an advocate who will be able to help you make well informed decisions regarding your pregnancy journey, your birth plan and how you want to take care of baby. ❤

    My HIV pregnancy Story: With my first child back in 2005, it was the AZT Era, had a natural birth no issues at a Public Hospital. Only bottletfed because I would not be available for breastfeeding because I had to leave my baby in EC. I learned about the Govt. Formula provision then but didn't need to. Baby no.2, totally undetectable for years. In 2017,long story short, though with MedicalAid, I ended up at Karl Brema Hospital (Public Hospital) best pregnancy medical care I received, BTW, another natural, very short but very painful labor,breastfed till almost two years old following all Dr's instructions ( taking treatment and gave baby treatment while he wasbreastfed) and Bob's Your Uncle!!

    With regards to feeding I was given both options, exclusively breastfeeding for 6 months or exclusively formula for 6 months. And I was told that as long as I haven't defaulted my baby will be born negative irregardless of whether I give birth naturally or not...and my preference was natural birth. I gave birth naturally and my baby was negative.

    If you are still far from birth i wish you strength with your gynae especially if you cant change him/her.... being a first time mom is overwhelming so you need all the support from your gynae and what you have related now does not look like support to me but someone who is just putting you down and judging you.
    I am sorry that you have to experience all this. Always know the "natural " way of bringing a child to life and "natural "!way of raising them does not make one more of a mother than others..
    The fact that you carried and gave life for me is a miracle... the rest are cherries ontop of a cake.
    After i had 4 days of labour and finally ending up with a c section when i was 8cm's dilated I realised the things we want when we still pregnant are not important, at that moment of labour you are faced with the stress and anxiety of bringing this child on earth alive... I remember i was even asking God if my child can come out of my nose i dont mind, i just want the alive!!... after all the trauma i did not even think twice to opt for an elective Csection with my second..

    As a Midwife Nozi my heart is so glad that you're empowered with so much information. It makes my job so easy. Kuwe whichever decision you take baby is safe considering your V/L. If you opt for BF please make sure the scheme provides the correct prophylaxis for baby ke and also make sure baby has all their PCR tests done. I'm saddened at my collegues response. You're every practitioners dream cc. He speaks like someone who is either not into Obstetrics or not updated when it comes to issues relating to the PMTCT programme. I'd advise you move to someone who is clued up. You can't be so informed and uncomfortable at your visits. Move to a safe space. Wishing you all the best ❤❤❤

    It's so disheartening that you went through that. As an HIV+ mom who recently gave birth my gynae gave me the option to choose what birthing options I want he just gave advice were needed and regarding the feeding option my gynae gave advice but told me that my baby's pediatrician would give detailed advice regarding that but again the final decision lies solely on me so I ended up choosing formula because I was scared of as I'm first mom and as people our bodies are different what if my milk supply is slow and the baby doesn't not get full and I can't mix feed I'll end up getting frustrated but after giving birth my gynae and pediatrician asked me what I want they understood my fears and respected my decision. My baby recently went for 10 weeks and did the test baby is still good. All the best to you and Abuti Skhu

    I wish I could send a voice note. My partner and I are both health practitioners and he happens to be a gynae. When we're were pregnant in 2021, the pregnancy complicated last minute due to hypertension. I cried and even though I knew the risks, I just couldn't hear them and he was siding with our gynae and it angered me so much, so I completely understand. As for your gynae, I fired my first 1 because we did not have a good relationship, I believe gynaes form a big part of your birth experience and I was not willing to have a bad one. I believe there should be a good relationship. It is not our choice as health practitioners to make decisions for patients, only to show them all the risks and benefits and let them choose. Breastfeeding is hard, and could or could not work, it's a tough and sore point but a lot of the time it does not work out and exclusively Breastfeeding for 6 months is Not easy. Don't be hard on yourself. Once baby is here all these anxieties will be taken away. I wish you all the best!! Congratulations once again.

    Sis Nozi maybe start consulting with a lactation consultant as well, these people are normally specialists about all things breastfeeding...and they can help you to advocate for your breastfeeding desires as well as wellbeing of baby! I once worked with one in a public hospital setting, and they are good🙌🙌🙌🙌

    I'm a healthcare worker working in mother and child stream; and I can guarantee that you have a choice sis Nozy. Women deliver all the time and breasfeed exclusively and their babies are fine. That's what I don't like about private, they're management of HIV is not the same as ours. But guidelines are there. Sorry Sis Nozy. Love your videos

      ​@@moemo4016 if the mother is virally suppressed and taking treatment well and the baby is on the prophylaxis for HIV then it is possible to mix feed, although ideally giving either breast milk or formula exclusively is recommended. There have been new Guidelines that allows for this.

    That's one thing I have come to learn about Private Healthcare and Public Healthcare.. at least where im from... with private it's about money and keeping the company afloat, unnecessary procedures being done just because it costs more and benefits them... they don't pay much attention to the patients mental health and personal preference... whereas in public, they let you express your preference and educate you on whether or not it is better for you without compromising your mental state.
    You were talking about public Healthcare in your last video and I'd really love to hear your opinion and experiences.
    Im a HCP at a clinic and educating, conversing and learning with and from my patients is the best part of my day.
    Approach is everything, bedside manners is the most important thing to learn in Healthcare... breastfeeding is recommended especially for a patient with a suppressed viral load... it's recommended for patients who have been constant and compliant with their medication and taking care of their health... a high risk and unstable patient isn't the best candidate for breastfeeding, because consistency is key and they might end up mix feeding when the going gets tough... the practitioner can make suggestions but not force you to do anything you do not want to do... take notes and make an informed decision, seeking other opinions is best at this point. Also take Bhuti Skhu with you on the journey as you research so that he also understands and is at ease with the decisions you both have to make.
    As a HCP I truly apologize to you and your husband about this experience, you should've left the consultation with more information rather than more questions and doubts. It's your choice as parents and at the end of the day, you know better than anyone else, what will work for you and your child.

    Hey Nozi I'm not HIV+ but my sister is. In 2018 she gave birth and I took her in for 2 months. She educated me a lot about birthing and breastfeeding when you are HIV+.
    She explained to me that the nurses told her that both natural or C-section are fine. She had a natural birth and the baby was given a nevarapine syrup for a week to minimize him getting infected. She breastfed him exclusively for 4 months. But then we switched to formular since she wasn't producing that much milk. According to the nurse's explanation, you can't mix feed because formula is much more heavy and at times it can thin the lining of the intestines making the baby vulnerable to infection. So inorder to minimize this, they ask HIV+ moms to exclusively breastfeed or exclusively formula feed. My nephew is turning 5 in a few months and he's HIV- and healthy. Just know Nozi that you are as able as any woman who is HIV- and you are within your right to choose your birthing option, breastfeed or formula feed your baby. Our health sector has made sure of it and please don't feel bad especially when some people still choose to be ignorant about HIV.     Yhoo Nozi sana, I went under a lot of postpartum depression due to the fact that I wanted to experience a lot of things with my pregnancy but because I wanted to protect my baby so much, I didn’t get to experience such things.I was forced into a C-section last minute even though my viral load has been undetectable for years now and used formula because I was so protective.Its upsetting,my love but we remain strong baby.But its all passed and my baby is negative and healthy chubby thing😍😍😍

    Your feelings are valid. Trust me cc. It's not in your head and you are not over thinking. The doctor was rude. Even when you have the means you have a choice. I breastfed my first born and she was premature. And 3 months in I ran out of milk and moved to milk. She is negative. I did all the funny things of putting breast milk in the eyes for what what. You are entitled to your own experience and as long as you look after yourself. I keep thinking of the nurse who told me to take out my womb back in 2012 when I just found out of my status 😥. Now I have 2 negative healthy babies.

    With all due respect, please change this doctor sis Nozi. Approach is very important. This
    medical practitioner is not it. You can give your knowledge and expertise in a good manner. I'm already uncomfortable for you. As a mother myself, my pregnancies alone haven't been easy and it's very important to go through your pregnancy journey, labour & delivery with a Dr who has your best interests and those of your baby's at heart. A Dr who sees you, understands you and is walking this journey with you in love and respect, a Dr that gives you peace of mind that it's all going to be okay despite the obstacles and hurdles on the way. A Dr that you brings you a sense of comfort and warmth with regards to your journey of meeting the new addition you'll all be bringing into this world.❤️

    First of all i would like to say thanks for updating us on your beautiful storie and a journey to pregnancy also delivery mode to select. As a lawyer as your little sister. I would suggest you to go for C- section as an option because it is considerered to be safe for all person whether negative or positive. C-section is regarded to be safe to a new baby. Any new parent who has a close doctor must be given the pros and cons of vaginal &C-section delivery mode. Now a parent must see and foresee the risks and if she has foreseen the risk and complications on labour the answer will be a for C section to save a life of a mother and a child whether in a positive or negative condition. Delivering mode is on your hands now, please choose wisely i wish to see you 3 both of you educating us. I wish you all the best Nozi & Mayaba. Lots of Love from Tanzania!

    My sister in law gave birth naturally and breastfed. unfortunately, for some reason her medication stopped working and she only found out four months into breastfeeding. She checked her viral load and it was in the 3000 range. She immediately stopped breastfeeding after getting her viral load results, but it was too late. Her baby is currently also on medication. So the wise thing to do is continuously check your viral load as you breastfeed.

Now listen Sisi, Dr's are also human so you will find some who have not dealt with their own fears and anxieties and they end up projecting these to their patients so I get your frustration Sisi bt don't be discouraged. You have sufficient information and you have received sufficient advice. Continue with the choices you have made, don't allow that doubt to creep in, we serve a God who covers and protects🙏 You have worked hard to get here so Qinisa emkhulekweni now and you and GT will be all good, we declare this in Jesus mighty name..🙏

    I am so sorry you had to go through this and I fully understand the position it puts you in and generally the pain that comes with lack of sympathy from your doctor. All moms want la bond ya during breast feeding, we want to fill that we are incontrol of how we bring our babies into the world. It's quite invasive more then ussually for mom's living with HIV/AIDS. All though I may not fully understand ezinye izinto I can relate as a mother especially with gynecologist that are not sensitive

    I know private GPS/ Obgyn wants to make money over the well being of the client! Natural birth is very much safe. And breastfeeding is vital and benefits of Colustrum first few days of life. At public hospital we ensure that we teach breast feeding techniques and safely expressing and storing breast milk. Note beyond 6 weeks of life we start with BACTRIM which is the prophylaxis takes until a week after breastfeeding is stopped. You can give breastmilk up to 2 years

    I felt it when you said, now it seems like your hiv status is an inconvenience to your baby 😪 that must hurt. It's all that Dr's approach or conduct that got you to even think of such.     What you said at the beginning sis Nozie is true. As long the mother is virological suppressed you can breastfeed your baby and you can give natural birth. And your baby will recieve NVP to protect her/him. This Dr was just being rude. Government always promotes breastfeeding not because they can't afford formula but because it is best for the bby, n remember there are many benefits a baby gets from breastfeeding than formula feeding. But ke sis wam..ts all up to you n ur husband. What you agree on go with it🥰
