That was my question too WHY DOES TEEN LEO NOT HAVE A ARM!?💀🤣
I did a little digging into the background of this comic, and as far as I was able to see from the minimal content I skimmed through, it's basically a what if scenario where Leo loses his arm during the events of the movie, and F!Leo also gets sent to the past and ends up helping younger Leo with his recovery. Hopefully that answers your question! :D

Leo: I'm sorry I socked a pillow on your head to even wake you up.

Leonardo: ACHOO!!! For fuck sake...the last time I was somewhat like this was when my arm was still healing.
Leo: What happened that you got sick?
Leonardo: Don fixed me up as fast as he could to save our medical inventory which well I told em to do but with one of the Krang's dogs were the reason I lost it, it got infected. I was in bad shape for weeks, Donnie wasn't sure if I was gonna make it but eventually it cleared up.
Leo: That must have been awful...
Leonardo: There's a lot of things I've gone through in my adult years, ya get numb to it all for a while, when there's nothing but chaos and death around ya, ya don't have fear rather ya just don't want to stick around to see what was next...
Leo: You thought about...killing yourself?
Leonardo: Many times and even when I first got here I did to but hearing that my future didn't exist anymore took all those thoughts six feet under. I know you do too though...
Leo: W-what?
Leonardo: *coughing* You get to be in a dark place when yer forced to wait out your body to heal itself while you, yourself, have done all you can. Besides, it hurts when you know someone doesn't wanna be around you but they will never tell you or deny it...
Leo: Y-yeah...thanks...
Leonardo: Anytime kid, I say the same to you, it takes guts to boss an old man around that's twice yer size.
Leo: Hm, medicine, now.
Leonardo: Alright, give em here *downs them dry* there.
Leo: I was gonna get you some water you know to get those down!
Leonardo: Poptart, the older ya get the more pill popping ya gotta do, I don't need no drink for any meds.
Leo: Get some sleep, your fever's getting worse...
