Cortisone shots vs. Prolotherapy - How are they different? Why cortisone loses its effectiveness

  • Ross Hauser, MD, and Danielle Matias, PA-C discuss the difference between Prolotherapy and cortisone and why they choose to treat chronic pain with Prolotherapy injections vs. cortisone injections. As discussed, Prolotherapy is the Regenerative Medicine term for injection techniques that aim to proliferate new, healthy tissue and can include solutions like PRP (platelet-rich plasma) and autologous stem cells. They also explain the reason behind a common scenario that patients experience when subsequent cortisone injections seem to become less effective than their first one.

    This is a great video! Every sports person should watch this. There are so many older people that are debilitated with pain because they were given cortisone when they played high-level sports in their younger years.

    I had prolotherapy from Dr. Hauser years ago when he was in Chicago, I've since had prolo by other doctors, Hauser is the guru of prolotherapy. I wish he would train other MD's so his work can continue, not just a PA.

But I'm stuck in a flexed chin back position, double chin because it feels like it wants to go back out. I'm on day 6 of flexing. MRI didn't catch it

    Thanks for doing this video. It’s so important not to over use the steroids shot. I had a laminonomy for stenosis about 4 yrs ago. I saw a neurologist recommended by a friend who did the laminatintomy . After the surgery I was still in pain , and knew nothing of treatment or what I should do. I was in a lot of pain, so I thought after the laminatonmy (is the least avasive technique for stenosis. It’s basically scraping the bone to allow room for the disk. Anyway I’m no expert. So, after the surgery I naively thought it would take away all the pain that it would be the “solution” an ends to the means. I went back to this neurologist and told him how much pain I was having and I felt I needed to know so much more. I was confused and in the dark . I was also afraid to research my condition believing that I might find out that I must have something truly wrong and horrible about my back. The neurologist answered a few of my questions. I had so many questions. It was almost like he wanted to keep me in the dark. I had read some article about Cuena Equina ,and I thought I had it so I went to the emergency room. Thank god I didn’t have it. Than this neurologist passed me on to other Dr a “ pain” specialist. Ok, I thought I’ve been an addict in the past(I’m currently being treated with suboxone and it has saved my life). They wanted to get me to get off suboxone and on that oxycontin real bad! I got it they wanted my ignorant and addicted,so my back problem would take 2nd place they wanted an addict . They figure they give you theese pills than you get addicted and you no longer care about your back . You want those pills! I said no and I stayed on Suboxone. It’s saved me a long time ago from opiate addiction and my addiction to fentanyl patches. AA meetings will not help someone who has an addiction for more than 10 years. I don’t care what people say. Thank God suboxone is avalible for treatment it works, and it gave me the power to say no to this “pain doctor”. 15 years ago I would have feel for it . Not now. And I sober from Suboxone, thank god everyday for it is a miraculous supportive drug for opiate addicts who truly want to stop and have an addiction for more than 10 years . All AA and NA gave me was trouble . It put me in rooms with people sicker than I was. Psychos even go to theese meetings try to get involved in your life. They’ve broken up relationships family and I’ll say this. It is the most arcane ignorant treatment for an addiction to a drug that really does interact with your brain in a way that it changes your brain.
    Any way back to why I’m writing all this. Thank You sooo much for this video !!! Remember the second doctor I told you about the “pain specialist” the next thing he was going to do was to give me steroid shots he told me would correct the pain. After I went for my first steroid shot. It felt so good no pain. Than a couple of days later it came back and when it did I was over seas in Scotland on my honeymoon. Yikes!! So then I made another appointment to receive the shot pain free for a few days . Than 6 months later reiceived shot temporarily took away pain but not totally . So over a series of a year I was getting this steroid shots constantly. There were a lot of patients there too! This doctor had to have been making bank! I came in one day very frustrated ready to have injection. A gentleman starts talking to me telling me how this is making my back worse! He told me these shots do more bad than good! It turned out he was the Anesthesiologist!!!!! The doctor should have told me this beforehand! It turns out I believe this anesthesiologist felt bad about people like myself ignorant and given shots when they need it. I thought about it ,and if they gave the patients only 3 shots a year like they are supposed to. The people themselves would see they don’t work and stop . It’s so wrong that these “doctors do this . It’s wrong and heartbreaking I stoped going to that so called doctor and I noticed I still have pain , but for some reason it stoped going down my legs after I stoped the injections. I wish I could tell you were this was but than I thought . This type of care model were the paitent is kept in the dark and given access to multiple steroid shots is sick ,and it’s making some people a lot of money . I no longer see the neurologist or the pain doctor that was giving me the shots. They aren’t doctors in my opinion money is the motivation. I mean how else could you justify giving a patient in your care something that will make you condition worse? Explain that! I will tell you I found an excellent doctor Dr.Wind in Maryland very professional. I wrote all of this because if that Anesthesiologist hadn’t informed me no one would have,and because of that I’m not doing something that is harming my back. I hope one person reads this and it saves them from e experience like I had. Now I inform myself daily about reasearch and everything else I can educate about my spine. Bob and Brad have a great UA-cam channel about back pain and warnings about jumping the gun and having evasive back sugary. They also speak of the McKenzie method of treatment that is used by a variety of back specialists too. I’ve tried some McKenszie exercises and they provide instant relief. I’m not kidding. My favorite one for pain in my lower back and legs is to lay on my stomach and push up slowly while you are in a sphinx position. Bob and brad have information on this position, and I no longer have pain radiating down my legs. It’s gone from laying on my stomach for prolonged amounts of time. Everyone is different but I hope someone will take away from this is not to fall into system of doctors keeping patients ignorant while being paid. It’s sicker that other docs No this and do nothing! Don’t believe everything your “pain” doctor says. Step back be aware of your pain but you are going to have to educate yourself especially if you have back trouble. I learned you just can’t trust someone because they are a doctor,unfortunately. I hope this saves just one person from addiction and failed back surgery which is very common and yes it can happen if you use steroid shots more than 3 times. This video was great too. I wish I would have been able to see it 4 years ago. Anyway take care and remember the only person who is going to really take care of yourself is you. Meditation and yoga and self awareness have saved me .

    I wish Dr. Hauser would open an office in New York City. I had a left hip replacement in 2017, after being diagnosed with AVN.. which the doctors found rare for me to have at my age..Late fifties, wasn't a Steroid user or Alcohol drinker, nor was I 28 years old. I fell in 2020 injuries both hips, lower back and right wrist.. Since my fall, my left hip pain has increased, with burning, stabbing pain that goes to my thigh and down the back of my left leg. Have pain in my groin and upper buttocks feels like it's on fire. Lower back and tailbone pain that stretches to the right hip. Problems urinating, feels like a full bladder but can't keep the stream going at times. March 23rd 2021, had a Epidural lower back, that instead of relieving the pain it made it Worst for a couple of weeks, then the pain just lingers. I have a lower Spine MRI tmrw, lets see what happened when my Pain Mgmt dr gave me 3 injections in March, I shouldn't be feeling pain like I just got an injection?
    They say I have CRPS or RDS, I personally don't think so.. Something is wrong and no one has found a way to correct my medical condition, bc they don't know what is causing it. MRI's of my left hip are normal, yet it feels and the outside of the area is warm and tender to the touch.. and it's not Bursitis. Rt hip is hurting and gets worst at times, and my lower back is hard to sit, stand lay down on back without discomfort.. I don't sleep much.
    It maybe the Piriformis Muscle, but no one wants to examine me for it.. Help!

    Thank you for this informative video! How about thinning of central TFCC? I am 41 years old female and I've been suffering from right hand tendon issue and wrist pain for the past 1 and half year. At first I was diagnosed with tendonitis and fluid retention around my wrist due to so called repeated action or something, and I was given physiotherapy for 2 months which helped a little. Then I stopped it and my doctor prescribed me a brace around my tendon which helped to some extent. But the pain never ever healed totally and I got dependent on this brace for all this time and even the brace it hurts sometimes, my wrist and even becomes red and swollen. Finally I had an MRI 2 weeks ago and I was told about thinning of central TFCC and that there weren't any tears. I looked it up for further details and I got scared by anything that I read about it. It hurts even now that I am typing this. Will I ever recover fully or will I god forbid lose my hand's ability altogether? I do not believe in temporary pain relieves like cortisone or Tylenol. I am looking for totally healing and recovering.

    Ugh, am 47, I just now stumbled onto this video after getting a steroid shot in my hip joint for FAI .. posterior with the bone narrowing; also been getting gel shots in both knees for osteoarthritis… I have been researching for three months on my condition, and this video was the most informative on UA-cam because I knew nothing about steroid shots, except the pain was 90% gone for weeks. I can get out of bed, car, walk, and climb stairs, but looking at this vid seems I did myself more damage.

    I had my 1st ever cortisone injection for ecu tendinosis about six weeks ago. I do not feel relief at all from the shot, my wrist was swelling for 2 weeks post injection and then skin rashes follow. I could not do the PT exercises I was doing just before the shot. I tried to do flexion, extension, rotation with light weight like 1-2 lb and my skin would flare up. Most times I can’t even tell if the inflammation is just from the skin or the ecu as well. The skin problem has calm down this week but I’m still feeling worse than pre-injection. Really frustrating time.

    Hallo, this is so interesting. I live in South Africa, have had, 3 back and two neck operations. Im very active. Its now 10 months after my last back operation. I am being treated by a lbiokinethisist on a weekly basis. Lots of stretching, and i walk. I suffer a lot with neck pain, unstable C6 and C7.
    Ofter have occipital neck pain and suffer with headaches all on the right side of my face, eyeballs pain, sensitive to bright lights. Have a tight periformus and gluteus medius, stretching and strengthening that area.
    It has been suggested that i have another neck operation.
    Can you help me. Im going to see if i can find a specialist who performs the polotherapy, i have been struggling for such long time, never heard of this before. I have had a few cortison injection, for trochanter bursitis.
    And ablation a few months after operations, which seem to help me cope with getting through the initial stages where the pain is constant.
    Is there anybody in South Africa that you know of who can help with such a therapy.
    Kind regards
    Ann K

      Hi Robert, Overall, we encourage people to eat fresh, whole foods as often as possible and plenty of vegetables throughout the day, but there is not one specific diet that helps everyone, in our experience. We've seen that the Mediterranean type of diet works well for many people as a starting point and work out from there. Some people who need more protein/fewer carbs (more of a keto approach) where others need less fat and more vegetables and fruit to feel their best and can even go vegetarian.

    Hi I have CRPS (RSD) & Cervical Stenosis. I’ve been suffering for over 25yrs.... had surgeries, trigger point injections, SI injections, epidurals, transferaminal (sp?) too many to count.
    My questions are : 1) If this isn’t FDA approved, does this mean it’s not being practiced? Meaning, are you the only Drs that are performing this procedure?
    2) Since it’s not FDA approved , I’m assuming insurance will not cover this? If they don’t/won’t how much will this cost a patient ? (Ballpark)
    3) What are you injecting? I’ve read Stem & PRP are not Prolotherapy. That Prolotherapy is actually either Sugar Water or Salt (25%) ,depending on what the Dr chooses.
    Thank You so very much! This is informative & gives me a little bit of hope in my battle conquering my pain!
    I look forward to hearing from you. 💜

    • @CaringmedicalProlotherapy
      @CaringmedicalProlotherapy  3 роки тому +2

      Hi karena2267, Thank you for reaching out and we're sorry to hear that you have been suffering for so long, undergoing so many procedures and not getting to the root cause of what is happening. Hope we can shed some more light on the topic for you. The FDA oversees food and drugs, not the specific techniques used in procedures. So, it is like a surgeon uses FDA-approved materials but the technique they use for a surgery may be different than another surgeon. Prolotherapy has been used for decades and the techniques can vary between providers. Prolotherapy is the umbrella term for regenerative injection treatments. So, stem cell therapy and PRP are types of Prolotherapy (therapies that proliferate the growth/strengthening of tissue). The solutions/ingredients/materials used by in our office are FDA-approved. We cannot speak for everyone who states that they practice Prolotherapy, again it would be like one surgeon speaking for all surgeons or one phlebotomist speaking for all phlebotomists, etc.. Our office has extensive experience and knowledge of almost all solutions because this has been our specialty for nearly 3 decades and our original office was one of the longest-running Prolotherapy offices in the world, directly handed down from Dr. Gustav Hemwall who practiced Prolotherapy from the 1950s-early 90s when Dr. Hauser joined his practice. We have published the most books and research on the topic, but we are not the only ones performing it. If you feel that you would like our team to review your specific case and potentially see you as a patient in our Fort Myers, Florida clinic, we'd be happy to help. Our prices are always listed on our website for transparency as well. Insurance is mixed on coverage for Prolotherapy. Many insurance companies only offer coverage for medication, steroid injections, some surgeries and some PT when it comes to chronic pain treatment options. But we do have most patients who file with their private insurance company tell us that they received at least some reimbursement. We wish you all the very best in health and finally finding a provider who can connect the dots on your case and help you get much-needed relief!

    In cases where prolotherapy may not work, e.g. trigger finger, would cortisone make any sense at all?

    • @CaringmedicalProlotherapy
      @CaringmedicalProlotherapy  3 роки тому +5

      Hi Cassian YHVH, Yes, there is a time and place for cortisone, of course. Every case would need to be advised individually. The point of this video is to explain how it is overused in a lot of degenerative pain conditions where it leads to osteoarthritis faster. Ideally, any patient's condition/symptom is assessed with the mindset of their long-term joint health and what is the best for them to preserve their joint, not just the immediate pain relief that could have worse long-term repercussions.

    Would this treat frozen shoulder in any way?

    • @CaringmedicalProlotherapy
      @CaringmedicalProlotherapy  2 роки тому

      Hi Clarence Lee, Thanks for asking. Good question. Yes, frozen shoulder is something that we help here with Prolotherapy in combination with physical therapy and other modalities that encourage restoring movement and blood flow to the joint. Prolotherapy is used to address the original injury, such as a tendon tear or ligament sprain. We did a very short video about it many years ago: ua-cam.com/video/UuKog-6xcf4/v-deo.html Like any treatment, though, it is based on the individual case and an exam would be needed to truly determine what the ideal route would be for a patient. Wish you all the best in health!

    So how does this help the disc bulging?

    • @CaringmedicalProlotherapy
      @CaringmedicalProlotherapy  3 роки тому

      Hi The Bed Doctor, Thanks for the question. The discs bulge into or herniate through ligament tissue. Since Prolotherapy is a treatment for ligament restoration/strengthening, it is used to help to stabilize the vertebral movement and prevent unstable joint motion that allows for discs to bulge and the excessive wear and tear on the discs and other surrounding tissues. We did a short video on this a few years back that may help as well: ua-cam.com/video/OgbAA4e-Yg8/v-deo.html
      Wish you all the best in health!

    • @CaringmedicalProlotherapy
      @CaringmedicalProlotherapy  3 роки тому

      @@thebeddoctor4273 With any medical treatment, not just Prolotherapy, the provider would need to see you in person for a proper exam and assessment, as well as review the imaging that you've had done. We help patients with bulging disc in many cases avoid surgery, but if surgery was necessary for someone we would recommend that. Understandably, we could not possibly know the best course of treatment for an individual without seeing them.

    Would it be best if its the first cortisone shot to have that to get out of pain quickly.. Then move on to prolotherapy.. as a longer term solution

    • @CaringmedicalProlotherapy
      @CaringmedicalProlotherapy  2 роки тому

      Hi Richard, We cannot recommend that route, but it would be up to the patient and their provider to determine the option that suits their needs best. One consideration is to have a consultation with both the doctor who would be providing the cortisone as well as the prolotherapist and then make a decision.

      I heard a story where a patient warned her orthopedic surgeon not to give her cortisone since the last time she had it she ended up in a psychiatric hospital because of the corticosteroid psychosis it caused . He reassured her that since he was injecting "inside" her knee joint that it would be safe for her and guess what happened after she agreed to that? That's right she ended up right back into the psych hospital and when she was being driven to the there she begged her driver to pull over on the bridge they were crossing so she could jump off of it. Luckily, she didn't do so and she eventually recovered. It's not a benign drug, granted not everyone gets those nasty side effects but it is hard to predict who will suffer these side effects.

      Steroids are not for everybody ! I refuse to have one . I worked with old people And they suffer constantly from pain , they get steroid shots but the have a lot of problems with autoimmune disease, skin , infection, kidney problems etc .etc
      Steroids can work for years or months , they don’t cure you . They only stop the pain so you can keep moving .
      I visited 3 doctors and have different opinions
      1- said I need for my herniated disks. Not side effects unless I have many .
      2- you are too young to have it . It will only mask the the pain but not cure .
      3- be aware it will kill your inmune system, you will have some osteoporosis later on , steroids kills the cells tissues where the steroid is injected. ( as I was getting in my spine ) he told me is bad to get near the spine

      Hi @patriciajump9511 - it's great to hear that your sciatica is gone, praise God!
      Unfortunately, the differences between Prolotherapy and Cortisone are extensive. Over the years we've seen many patients who have received corticosteroid (cortisone) injections for joint pain and for many, excessive cortisone treatments lead to a worsening of chronic pain. While some people do benefit from cortisone in the short term, the evidence points to cortisone causing more problems than it helps.

      @CaringmedicalProlotherapy We need you in the uk. Our health system is in rapid decline. 7.4 million people in England alone awaiting elective surgery. I think how many people if offered prolotherapy would not need surgery or be in pain. Not all surgery is successful either. One of my neighbours over ten years ago had double hip replacement but was no better afterwards. Now he has to use a scooter to transport himself around. He paid the equivalent today of about £26,000 for private surgery.

      Hi leah, Just like with any medical treatment, not all providers or centers are the same. Most Prolotherapy offices do not have digital motion x-ray or even treat the upper cervical spine. We run a highly specialized neck center for these types of cases, so that is the experience we share. While there are many great Prolotherapists, not all treat the spine or complex cases. Ultimately, you also want to be treated by a doctor who you feel is on your team. We wish you all the best in healing!
